Notes: We present a case of widespread reactive perforating collagenosis in a 63-year-old woman undergoing haemodialysis after diabetic nephropathy, who was treated successfully with allopurinol. The patient responded well and rapidly to a dose of 100 mg allopurinol daily. It is suggested that more patients with reactive perforating collagenosis may benefit from allopurinol therapy.

Notes: Background/aims: The objective measure of mechanical properties of the skin is relevant to the clinician as well as to the researcher concerned with skin physiology. The lack of standardization among methods used to obtain visco-elastic data from the skin is however, apparent and represents an obstacle to comparison of the results derived from the various studies concerning the subject. Methods: The mechanical properties of the ventral forearm were studied in 35 healthy volunteers. The Dermaflex® and the DermaLab® (Both Cortex Technology, Hadsund, Denmark) were compared. The results were analyzed with descriptive statistics and correlations using non-parametric methods. Measurement accuracy was assessed using coefficient of variation. Sub analysis was made in groups according to gender and age. Results: The overall correlations between the two methods were moderate (r = 0.383–0.437). A greater coefficient of variation was found in the DermaLab® machine. Significant differences were found between all values of both methods when comparing the mechanical properties of the skin in 30–39 years olds versus the group of 40–58 years olds. No differences were found between the genders. Conclusions: The results indicate that both methods are useful in the study of the mechanical aspects of the skin, especially for detecting age-related changes. The Dermaflex® seems to produce more accurate for data. This is thought to be due to the conceptual design differences between the two methods. The methods measure related but not identical values of the mechanical properties of the skin, as reflected by the limited positive correlation between them.

Notes: In order to establish the possible role of androgen in the development of hidradenitis suppurativa the symptomatology of the disease was studied in a group of 70 female patients. The results were compared with those obtained from an age-matched control group of healthy women. The incidence of women with signs of androgenization in the two groups did not differ significantly. The only significant differences found were a shorter menstrual cycle and a longer duration of menstrual flow in the women suffering from hidradenitis, and that women with hidradenitis were more likely to have a positive family history of hidradenitis. The prevalence of hidradenitis in the control group was 4%.The results show that hidradenitis is not accompanied by other signs of androgenization. The disease may be due to local changes in the aprocrinc glands of predisposed individuals.

Notes: Summary The excretion of collagen metabolites and circulating androgens was measured in ten males suffering from progressive systemic sclerosis. Significantly higher levels of sex hormone-binding globuline, total testosterone and dihydrotestosterone (DHT) (P〈0.01), and total oestradiol (P〈0.05), were found in patients when compared with age-matched controls. Urinary excretion of hydroxyproline in patients was found to correlate significantly with total testosterone (P=0.035), with DHT (P=0.005) and dehydroepiandrosteronsulphate (DHEAS) (P=0.034). Similarly the hydroxyproline peptide fraction was found to correlate significantly with total testosterone (P=0.037), with DHT (P=0.005) and with DHEAS (P=0.008). Hydroxylysine peptide in the urine correlated significantly with free testosterone (P=0.035) and DHT (P=0.040). Oestrogens did not correlate with urinary excretion of collagen metabolites. These findings suggest that androgens may play a role in the pathogenesis of scleroderma in male patients.
