Summary The author gives a report of the technic of the electrocorticography during the operation and of the subcortical electrography that neither disturbs the neurosurgeon nor damages the patient's brain. The flexible conductors of the electrodes are fixed in pairs on the boneedge with clamps. Hemostasis and coagulation are possible with the already placed electrodes; the operation-field is not covered by the rigid clamps. The electrodes may be removed from the operation-field towards the bony edge at any time, so thatthe neurosurgeon can operate with placed clamps for the electrodes, and after a pause new ECG records can be derived. 1. The electrocorticography (ECG) is carried out through a bore on the open dura or on the pial brain surface with the described electrodes, and it may be completed with recording by needles. a) Theepidural ECG is recorded after elevation of the bone flap from the dura before opening it and, while preserving the cerebral cortex, it already permits to establish a good localization of the cortical focus of epilepsy and superficial tumors. b) Thedirect ECG is performed after the opening of the dura and after the application of the electrodes on the cortex. Both methods are generally combined with derivations by means of needles from deeper lying cerebral regions. 2. For thesubcortical electrography two needle-systems are used: a) The deep electric derivation is repeatedly carried out with anelectrodeprobe 1,5 mm thick, containing 5 to 8 silver wires in a container of plexiglas. b) For the less deep endocortical and subcortical derivations 0,6 mm thick double needles are used. With the electrode-probe it is possible to ascertain the exactexpansion in cm. of the deepness of thecerebral tumors by means of the electric silence, whereas by means of the slow waves in the periphery we can localize the cerebral edema which accompanies the tumor. Previously used free derivations in the cortical and subcortical space by means of drill-holes is improved, as far as localization is concerned, with the help of aprecision apparatus. Drilling and derivation were performed in two times.

Résumé Les auteurs donnent la description d'une technique nouvelle de l'électrocorticographie et de l'électrographie souscorticale. La méthode ne dérange pas le neurochirurgien et évite l'altération du cerveau. La fixation des paires d'électrodes flexibles se réalise par des porte-électrodes spéciaux sur le bord de l'os. Il est possible de faire de petites manipulations comme l'hémostase et l'électrocoagulation avec les électrodes en place. Le champ opératoire n'est pas encombré par des porte-électrodes étendus et raides. On peut retirer les électrodes à volonté de la zone opératoire jusqu'au bord de la trépanation, le neurochirurgien peut continuer l'opération laissant les porte-électrodes en place et refaire un nouvel enregistrement au cours d'une pause de l'opération. 1. L'électrocorticographie est faite ou sur la dure mère (ECG épidurale) ou sur la surface piale du cortex (ECG directe) dans la manière décrite et peut Être complétée par des dérivations des aiguilles. L'électrocorticographie épidurale dérivé après l'enlèvement du volet et évitant une altération du cerveau permet une bonne localisation des foyer épileptiques corticales et des tumeurs superficiels. 2. Pour l'électrographie souscorticale on applique deux sortes d'électrodes. a) Ladérivation souscorticale multiple est faite par unesonde d'électrodes à diamètre de 1,5 mm consistant d'une baguette de plexiglas qui contient 5 à 8 fils d'argent. b) Pour la dérivation intracorticale et souscorticale moins profonde on utilise des aiguilles doubles d'un diamètre de 0,6 mm. La sonde-électrode permet dedéterminer la profondeur des tumeurs cérébrales par la zone de silence électrique avec une précision de l'ordre du centimètre et de reconnaÎtre l'oedème autour de la tumeur par les ondes lentes. La dérivation directe des régions corticales et souscorticales par des trou de trépane sans grande trépanation décrite dans une publication antérieure est complétée par l'appliquation d'une appareil stéréotaxique. La trépanation et la dérivation se fait en deux temps, dans un premier temps on pratique le trou de trépane, dans un second temps, après guérison de l'incision on ponctionne et dérive les potentiels.

Zusammenfassung Es wird über eine neue Technik der operativen Elektrocorticographie und subcorticalen Elektrographie berichtet, die den Hirnchirurgen nicht stört und das Gehirn des Patienten schont. Die flexiblen Elektrodenleitungen werden paarweise mit Krallenhaltern am Knochenrand befestigt. Kleine Manipulationen, wie Blutstillung und Koagulation, sind bei liegenden Elektroden möglich und das Operationsfeld wird nicht durch starre Elektrodenhalter verdeckt. Die Elektroden können jederzeit aus dem Operationsfeld an den Knochenrand zurückgezogen werden, so da\ derHirnchirurg bei liegenden Elektrodenhaltern weiter operieren und in einer Pause erneut ableiten kann. 1. Die Elektrocorticographie (EEG) kann nach der Trepanation auf der freigelegten Dura oder auf der pialen Hirnoberfläche mit den beschriebenen Elektroden durchgeführt und durch Nadelableitungen ergänzt werden. a) Dieepidurale ECG wird nach Aufklappen des Knochens vor Duraeröffnung von der harten Hirnhaut abgeleitet und erlaubt bei maximaler Schonung der Hirnrinde bereits eine gute Lokalisation corticaler Krampffoci und oberflächlicher Tumoren. b) Diedirekte ECG geschieht nach Abziehen der Dura und Auflegen der Elektroden auf die Hirnoberfläche. Beide Methoden werden meist mit Nadelableitungen aus tieferen Hirnregionen kombiniert. 2. Für diesubcorticale Elektrographie werden zwei Nadeltypen verwendet: a) Die mehrfache elektrische Tiefenabtastung erfolgt durchElektrodensonden von 1,5 mm Dicke, die in einem Plexiglashalter 5 bis 8 Silberdrähte enthalten. b) Für die intracorticale und subcorticale Ableitung in geringerer Tiefe werdenDoppelnadeln von 0,6 mm Durchmesser verwendet. Mit der Elektrodensonde kann man dieTiefenausdehnung von GroΒhirntumoren durch die elektrische Ruhe in Zentimetern genau angeben und das begleitende Hirnödem im Randgebiet durch die langsamen Wellen lokalisieren. Die früher angewandte freie Nadelableitung aus corticalen und subcorticalen Hirnregionen durchTrepanlöcher wird mit Hilfe einesZielgerätes lokalisatorisch verbessert. Trepanation und Ableitung wird zweizeitig vorgenommen.
