    Notes: Summary Stress symptoms, burnout and suicidal thoughts in Finnish physicians were studied using a questionnaire. The questionnaire, containing 99 questions or groups of questions, was mailed to 3,496 physicians representative, as regards age, sex, specialization and employment, of all active physicians in Finland. Altogether, 2,671 physicians (76%) responded. Stress symptoms in male physicians were similar to those in male managers. In female physicians they were slightly less than in female white-collar workers. In male specialists highest burnout indices were found in general practice and occupational health; psychiatry and child psychiatry; internal medicine, oncology, pulmonary diseases, and dermatology and venereology. In female specialists they occurred in general practice and occupational health; radiology; internal medicine, neurology, pulmonary diseases, and dermatology and venereology. Non-specialists had higher burnout scores than specialists for both sexes. Highest burnout scores in both men and women occurred in those working in municipal health centres. Lowest scores occurred in those working in private practice, universities, research institutes, and public offices and organizations. Suicidal intent tended to be commoner in physicians than in the general population. It was also commoner in female (26%) than in male (22%) physicians. The results indicate a polarization between “higher burnout specialties”, often dealing with chronically ill, incurable or dying patients (e. g. specialties such as oncology, pulmonary diseases and psychiatry), on the one hand and “lower burnout specialties”, often dealing with curable diseases and favourable prognoses (e. g. specialties such as obstetrics and gynaecology, otorhinolaryngology and ophthalmology), on the other.

    Notes: Summary Muscle blood flow and capillary diffusion capacity were determined in 21 Type 1 (insulin-dependent) diabetic patients and in 12 age-matched healthy subjects by measuring the simultaneous clearance of 133xenon and 131iodide from hyperaemic anterior tibial muscle. Blood flow was significantly lower (mean ± SD: 46.7±14.1 versus 59.4±12.9 ml 100 g-1 min-1, p〈0.02) and capillary diffusion capacity was significantly greater (mean ± SD: 8.0±2.1 versus 5.9±1.3 mol/min, p〈0.005) in the diabetic patients than in the control subjects. Ultrastructural dimensions and density of capillaries in the gastrocnemius muscle of 11 diabetic patients and six control subjects were also studied. Diabetic and control capillaries did not differ in total capillary area. Compared with normal capillaries, the percentage area of basement membrane and the apparent basement membrane thickness were significantly greater (1.21±0.6 versus 0.78±0.2 μ, p〈0.05) in diabetic capillaries, while there were no significant differences in luminal, endothelial and pericytial areas. There was no difference in capillary density between the two groups. No correlation was found between basement membrane thickness and capillary diffusion capacity in the diabetic patients. We conclude that the greater capillary diffusion capacity is due to increased permeability of diabetic capillaries, and that the basement membrane is probably not ratelimiting in the transcapillary transport of hydrophilic substances in diabetic subjects.

    Notes: Summary Intramuscular nerves in the anterior tibial muscle of rats with carbon disulphide (CS2)-induced polyneuropathy were studied ultrastructurally and after histochemical demonstration of non-specific cholinesterase (Ns. ChE; E.C.3.1.1.8) activity. After 5 weeks' exposure to CS2 the intramuscular nerves showed occasional weak Ns. ChE activity, which did not appear in the normal intramuscular nerves. Axoplasmic ultrastructure was normal in the majority of the nerves. After 5 months' exposure to CS2 there was intense Ns. ChE activity along intrauscular nerve trunks. The axoplasm of the nerves showed marked degeneration: distended smooth-surfaced vacuoles, dense bodies, disappearance of neurotubules and appearance of increased amounts of neurofilaments. The present findings suggest that histochemical demonstration of Ns. ChE activity can be employed as an indicator of early axonal degeneration in progressive polyneuropathy.

    Notes: Summary Peripheral nerves and myoneural junctions of the tibialis anterior muscle of the rat were studied histologically and electrophysiologically after various periods of peroral ethanol treatment. Histochemical distributions of non-specific cholinesterase (ns. ChE; E.C. 3.1.1.8) and acetylcholinesterase (AChE; E.C. 3.1.1.7) activity of the muscle were normal during the first 3 months of daily ethanol drinking. After 5 months of exposure to 10–25% (v/v) ethanol as the sole drinking fluid, pathological ns. ChE activity was seen sporadically along the intramuscular nerves with slight ultrastructural changes in the Schwann cells. After 7 months of ethanol treatment there was further increased pathological ns. ChE activity in the intramuscular nerves while the AChE activity remained normal in the muscle. More prominent ultrastructural changes were seen in the Schwann cells namely swelling and vacuolization of the cytoplasm and dilatation of the rough endoplasmic reticulum. Increased numbers of small axons were also seen. After 9.5 months on alcohol marked increase in the ns. ChE activity was observed along most of the intramuscular nerves. AChE activity of the myoneural junctions was only sporadically weakened. A slight slow-down in the conduction velocity of the large myelinated size A fibers was observed in the animals on alcohol from 7–9.5 months, whereas the conduction velocity of the smaller myelinated B fibers was not appreciably changed. The present experiment indicates that progressive neuropathy can be induced in rats by oral alcohol feeding along with the normal laboratory diet. The first pathological changes were seen in the Schwann cells and could well be followed by the methods employed. The present experimental model can possibly be used in future studies concerning the development of toxic polyneuropathy.

    Notes: Summary The structure of myoneural junctions in the tibialis anterior and intercostal muscles was studied after histochemical reaction for myoneural acetylcholinesterase (E.C. 3.1.1.7) in human embryos. Myoneural junctions of a primitive form were first seen in the intercostal muscle at the age of 8.6 weeks (crown-rump length of 3.2 cm), and in the tibialis anterior muscle at the age of 10 weeks (4.3 cm). The postsynaptic membrane was devoid of any junctional folds typical of adult synapses up to the age of about 19 weeks. At first small, the junctional folds gradually became deeper and more prominent during the following weeks, and some ramification of the previously coherent postsynaptic area took place. Myoneural morphogenesis was not completed at birth, although well developed postsynaptic Moldings were present.

    Notes: Summary Forty-eight persons (age 44, SD 9 years) exposed to paint solvents and 40 nonexposed referents (age 45, SD 9 years) were examined. The duration of exposure was 4 to 30 (mean 20) years and the life-time exposure level was estimated to have been 10 to 330 (mean 60%) of the hygienic standard. The methods used were a neurological examination, electroencephalography, brainstem auditory-evoked potentials, electronystagmography, and posturography. Lifetime exposure was estimated on the basis of a detailed occupational history and environmental measurements made at the work places over several years. The average weekly alcohol consumption was established in a detailed interview. Due to the small number of subjects examined, the study was inconclusive. The findings classified as abnormal were slightly more common in the exposed than in the referents, and in the persons who used alcohol than in the non-users. The differences were not statistically significant, and no exposure-response relationships were found.
