    Notes: The rate constant of a bimolecular gas-phase chemical reaction is determined by the energy distribution of the reactants and the cross section for the reaction. Systems in which photolysis and other nonequilibrium processes occur may have energy distributions which differ significantly from the equilibrium distribution. Analytical solutions are derived for the rate constant as a function of temperature for certain systems in which nonequilibium processes occur. The analytical solutions are compared with the numerical solutions and the agreement is found to be excellent, confirming the validity of the approximations that were made. Chemical reactions are simulated with the Monte Carlo method and the results again confirm the validity of the analytical solutions. Under certain nonequilibrium conditions, it is shown that the complex expression for the rate constant as a function of temperature reduces to a simple form, which resembles the Arrhenius equation. In this special case, the dependence of the results on the shape of the reaction cross section is investigated.

    Notes: The feasibility of using a photoionized, low-ionization potential organic seed gas to initiate a high pressure plasma discharge is examined and compared to radio frequency breakdown of high pressure argon alone. The seed gas, tetrakis(dimethylamino)ethylene, which has an ionization potential of 6.1 eV is ionized by an ultraviolet laser through 6.4 eV photon absorption, and forms a plasma column inside a vacuum chamber. The plasma absorbs additional power through inductive coupling of 13.56 MHz helical antenna radio frequency wave fields to the plasma through electron acceleration, ionization, and collisional damping. Laser initiation of 2–6 mTorr of the seed gas in 1–150 Torr of argon is accomplished and produces steady-state line-average plasma densities of ne(approximate)4×1012 cm−3 in a volume of 300 cm3. The two-body recombination coefficient of the organic seed gas and its optimum partial pressure when mixed with argon are experimentally determined and analyzed. Particle loss and power requirements for maintaining the discharge are evaluated by examining ionization, diffusion, and recombination processes. © 2002 American Institute of Physics.

    Notes: A pulsed, 193 nm excimer laser is utilized to photoionize the organic gas tetrakis- dimethylamino-ethylene (TMAE). The laser ionizes a plasma sheet with a width of 7.8 cm and an adjustable thickness of 0.7–1.4 cm. The axial scale length of the plasma density is a function of TMAE neutral pressure and is typically 50 cm. X-band (10 GHz) microwaves are incident on the plasma with the electric field polarized parallel to the laser beam axis. The power reflection coefficient and the phase of the reflected signal are studied as a function of time. A monostatic homodyne detection system with a response time of 10 ns is utilized to determine the amplitude and phase of the reflected wave. The peak plasma density is ne(approximate)4×1013 cm−3, sufficiently above the critical density (ncrit=1.2×1012 cm−3) to produce reflections comparable to a conducting sheet placed in the same position as the plasma. A computer model is developed to interpret and optimize the plasma conditions which provide the highest backscatter and phase-stable reflection coefficient for the longest period of time. The presence of axial density gradients causes the reflected wave to be scattered through a wide angle. As the gradients relax, the backscatter reflection coefficient increases to a value of nearly 100%. The plasma density and two-body recombination coefficient are measured by means of microwave backscatter plasma reflectivity and Langmuir probes. © 1999 American Institute of Physics.

    Notes: This is a case of a large lactating adenoma which developed in a 26 year old primagravida during the third trimester of her pregnancy. The presentation was that of a grossly enlarged and engorged breast with breast erythema, warmth and tenderness. Radiologic and pathologic examinations were difficult because of the lactational changes in the breast and areas of infarcted tissue within the large tumor. Skin biopsies and core biopsies of the mass were performed to exclude malignancy. Surgical resection of the mass was necessary for definitive diagnosis. The pathology proved to be a lactating adenoma, which is the most prevalent breast mass in young pregnant females. The large size of this tumor, and the presentation of breast erythema and edema raised the possibility of inflamatory breast cancer. Following surgical resection and definitive diagnosis of this tumor, the patient required plastic surgical reconstruction of the breast because of redundant breast tissue. Although most lactating adenomas spontaneously involute, the diagnosis is not always straight-forward and surgical resection may be required for definitive diagnosis and exclusion of other pathologic processes.

    Notes: Abstract: Black women with breast cancer have significantly poorer survival rates, a more advanced stage distribution, and are diagnosed at younger ages compared to white patients in the United States. We evaluated tumor response and survival with respect to race and age after induction chemotherapy. The study population consisted of 303 patients (229 white, 74 black) registered in two prospective trials of induction chemotherapy for locally advanced breast cancer [stage II (T 〈inlineGraphic alt="geqslant R: gt-or-equal, slanted" extraInfo="nonStandardEntity" href="urn:x-wiley:1075122X:TBJ99071:ges" location="ges.gif"/〉 4 cm), stage III (noninflammatory), and stage IV (supraclavicular lymph node involvement only)] between 1989 and 1996. Chemotherapy regimens utilized 5-fluorouracil, cyclophosphamide, and doxorubicin (FAC). Response was defined as complete (CR, no clinical/radiographic detectable disease), partial (PR, 〈inlineGraphic alt="geqslant R: gt-or-equal, slanted" extraInfo="nonStandardEntity" href="urn:x-wiley:1075122X:TBJ99071:ges" location="ges.gif"/〉50% reduction in disease), minor (MR, 〈50% reduction), no change (NC), or progressive disease (PD). Median follow-up was 58 months; survival was calculated using the Kaplan–Meier method. There was no significant difference in age at presentation (54% of black patients compared to 58% of white patients 〈50 years of age). The black patients had significantly more advanced stages of disease at diagnosis (50% of black patients compared to 30% of white patients with stage IIIB disease; p = 0.03). For both age groups together, tumor response, 5-year overall survival (OS), and 5-year disease-free survival (DFS) rates were similar between the black and white patients. A trend was noted that the younger black patients were more likely to have a clinical CR or PR; this did not translate into a survival advantage. Despite the more advanced stage distribution for black women with breast cancer, induction chemotherapy yields high response rates (especially for younger black patients) and survival rates equivalent to white patients.

An assessment of peritoneal lavage fluid CA125 as a tumor marker in epithelial ovarian cancer, Int J Gynecol Cancer 1991; 1: 215–222.CA125 levels in peritoneal lavage fluid (PLF) has been evaluated as a tumor marker in EOC. PLF samples were obtained by performing peritoneal lavage with 1L 0.9% saline, usually via temporary percutaneous cannulae. The study group comprised 87 EOC and 40 control patients; in controls, peritoneal lavage was performed at laparoscopy. Repeated access was associated with significant problems which curtails the potential of IP monitoring. Overall 25% of peritoneal lavage attempts were unsuccessful. A normal upper limit of 60 U ml−1 was established for PLF CA125. In patients with postoperative disease, pre-treatment PLF and serum CA125 (using a cut-off point of 30 U ml−1) levels were elevated in 66 and 87% of cases, respectively. PLF CA125 had a stronger association with the presence of ascites than with the amount of residual disease. PLF CA125 levels correlated with observed response in 71% of 51 patients with evaluable response, which was not significantly less than the 83% observed for serum CA125. Serial measurement demonstrated that rising PLF CA125 levels can predict relapse but that serum CA125 was at least as good in this respect. PLF CA125 is not a more sensitive tumor marker than its serum counterpart and will contribute little to the management of EOC.

    Notes: We demonstrate by using simulations that spatial embedding of single-variable time series data does not reliably reconstruct state-space dynamics. Instead, correlation dimension estimated from spatially embedded data is largely a measure of linear cross-correlation in the data set. For actual electroencephalographic (EEG) data, we demonstrate a high negative correlation between spatial correlation dimension and the average amount of lag-zero cross-correlation between “nearest-neighbor” embedding channels (the greater the cross-correlation, the lower the dimension). We also show that the essential results obtained from spatially embedding EEG data are also obtained when one spatially embeds across a set of highly cross-correlated stochastic (second-order autoregressive) processes. Although, with appropriate surrogate data, correlation dimension estimated from spatially embedded data detects nonlinearity, its use is not recommended because correlation dimension estimated from temporally embedded data both reconstructs state-space dynamics and, with appropriate surrogate data, detects nonlinearity as well.

    Notes: Objective To review the incidence of Stage I epithelial ovarian carcinoma in the West Midlands region and to identify prognostic factors that have a significant effect on survival.Design A retrospective review of all Stage I ovarian cancer patients registered from 1.1.80 to 31.12.84.Setting West Midlands Regional Cancer Registry.Population 457 patients with Stage I ovarian cancer—373 with epithelial ovarian carcinoma.Main outcome measures Survival at censor date of 30.6.89.Results 28% developed recurrent disease, and the overall 50-year survival of the group was 70%. Univariate analysis of all possible prognostic factors showed that stage, adjuvant chemotherapy, histological grade and type, surgical rupture of the tumour, intact capsule histologically, clinical finding of ascites and the performance of peritoneal washings were significantly associated with survival. Adequate surgical staging as defined by FIGO was not significantly associated with survival. A multivariate analysis based on the Cox proportional hazard model identified histological grade, adjuvant chemotherapy, patient age and peritoneal washings as having independent prognostic effects. Surprisingly adjuvant chemotherapy and peritoneal washings had negative effects on survival.Conclusions Although a retrospective review has limitations, it would appear that adequate surgical staging and adjuvant chemotherapy confer no benefit in terms of survival in Stage I disease.
