    Notes: Zusammenfassung Im Vergleich der Schwangerschaftsraten sowie der Tubendurchgängigkeit nach der Operation liegen die mikrochirurgisch operierten Patientinnen deutlich besser. Die Domäne der Mikrochirurgie liegt auch nach unseren Ergebnissen bei der Anastomose. Bei der Refertilisierung nach Sterilisatio sind die Ergebnisse am besten. Bei den heute noch relativ schlechten Erfolgsraten der IVF wird in allen Fällen eine mikrochirurgische Sterilitätslaparotomie vorangehen. Dabei kann man den Zustand der Tuben am besten beurteilen und den Operationserfolg abschätzen. Bei aussichtslosem Befund sollten die Ovarien funktionsgerecht im kleinen Becken plaziert werden, um möglichst eine ultraschallkontrollierte Follikelpunktion für die spätere IVF zu ermöglichen.

    Notes: Schlußfolgerung Nach unserer Auffassung ist das laparoskopische Operieren von Dermoiden möglich, wenn kein Malignomverdacht besteht, wenn die Größe eines Tumors nicht einer laparoskopischen Operation widerspricht, bei gutem laparoskopischen Training, bei optimaler nachfolgender Überwachung der Patientin (Sonographie). Die Indikation zur Re-Laparoskopie scheint uns bei Kinderwunsch gegeben.

    Notes: Zusammenfassung Die laparoskopisch durchgeführte Bildung einer Neovagina bedeutet eine wesentliche Verbesserung der Vecchietti-Methode. Dank des geringen Traumas erfordert dieser Eingriff nur einen kurzzeitigen Krankenhausaufenthalt, was besonders in Anbetracht dieses psychischen und physischen Problems von großer Wichtigkeit ist. Die Ergebnisse sind identisch, so daß dieses Verfahren der konventionellen Methode nach Vecchietti immer vorgezogen werden sollte.

    Notes: Abstract Background: This study was undertaken to assess the value of laparoscopic surgery as treatment for benign mature teratomas of the ovary. Methods: A total of 70 patients treated exclusively with laparoscopic surgery for ovarian mature teratoma were studied. The tumors were either enucleated with preservation of the ovary or removed by salpingo-oophorectomy. Results: Ovary-preserving surgery was performed in 60 cases; salpingo-oophorectomy was accomplished in 10 cases. All tumors removed were histologically benign. No surgical complications were encountered throughout the series. In eight patients who underwent primary ovary-preserving surgery, a second-look laparoscopy was undertaken with virtually no pathological findings noted. Conclusions: Our findings confirm those of other authors—when performed by experienced surgeons, laparoscopic removal of ovarian mature teratomas is a safe and recommendable alternative to laparotomy. Preoperative diagnosis and intraoperative inspection of the tumor must be as thorough as possible.

    Notes: Abstract Background: This paper addresses the value of laparoscopic surgery for the removal of gonads in patients with Y-chromosome-bearing gonadal dysgenesis and androgen insensitivity syndrome, who are otherwise faced with a high rate of gonadal malignancy. Methods: Three patients with Y-chromosome-bearing gonadal dysgenesis and one patient with androgen insensitivity syndrome were operated upon laparoscopically. Removal of gonads was accomplished by their mobilization and dissection from the pelvic side walls, with ligation and transection of the utero-ovarian and infundibulopelvic ligaments. Results: Surgery was without complications. Histological examination of the gonads showed complete removal and absence of malignancy in each patient. Patients were discharged the day after surgery. Conclusions: The laparoscopic approach is a safe and effective alternative to laparotomy in the management of patients with Y-chromosome-bearing gonadal dysgenesis and with androgen insensitivity syndrome.

    Notes: Zusammenfassung Neuentwicklungen auf dem Gebiet der operativen Endoskopie führten zu einer wesentlichen Erweiterung der Behandlungsmöglichkeiten bei der Endometriose. Bei entsprechender Ausrüstung und Ausbildung der Operateure können Endometriosebefunde bis Stadium IV nach AFS laparoskopisch behandelt werden. Grundprinzip der Therapie ist die radikale Entfernung aller sichtbaren bzw. tastbaren Endometrioseherde. Eine exakte präoperative Diagnostik ist bei ausgedehnten Befunden unerläßlich. Die Vorteile der Laparoskopie liegen in der geringeren postoperativen Morbidität und besseren Lebensqualität. Hinsichtlich der Radikalität und postoperativen Fertilität ist die Laparoskopie gegenüber der Laparotomie als gleichwertiges Verfahren anzusehen.
