Die Verriegelungsnagelung ermöglicht eine stabile Osteosynthese infizierter Pseudarthrosen des Ober- und Unterschenkels sowohl im Schaftbereich als auch in Gelenknähe. Nach knöcherner Überbrückung des Falschgelenkes und Metallentfernung macht die Sanierung des osteomyelitischen Infektes meist keine Schwierigkeiten.
    Notes: Summary Infected pseudarthroses, often with bone defect, present particular problems in the treatment of post-traumatic osteomyelitis. The interlocking nail permits stable internal fixation of infected pseudarthroses of femur and tibia even close to the joints. The treatment of the osteomyelitic infection is straightforward after bony bridging of the pseudarthrosis and removal of the metal implant.

    Description / Table of Contents: Résumé Dans l'ostéomyélite chronique, I'implantation de billes de Gentamycine PMMA après excision des tissus infectés permet des concentrations locales de l'antibiotique qui dépassent de loin les concentrations qu'on peut obtenir lors d'une antibiothérapie systémique. Les concentrations sériques et urinaires de Gentamycine sont très basses. Des effets secondaires toxiques ne sont donc pas à craindre. La technique opératoire est décrite en ce qui concerne l'ostéomyélite séquestrante, les ostéosynthèses infectées, les pseudarthroses et les endoprothèses articulaires infectées.
    Notes: Summary In chronic osteomyelitis very effective local antibiotic therapy following surgical debridement can be achieved by implantation of Gentamycin-PMMA-chains. The high local concentrations of the antibiotic are not achieved by any systemic antibiotic therapy. Because of the low serum and urine levels there is no risk of nephrotoxic and ototoxic side effects. The operative procedure in sequestrating osteomyelitis, infected osteosyntheses, infected nonunions and infected endoprostheses is described.

Nach radikaler Entfernung von Nekrosen, Sequestern und alloplastischen Fremdkörpern werden die infizierten Hohlräume mit den Ketten ausgefüllt und verschloBen. Das Antibioticum wird in Konzentrationen weit über der MHK und MBK der ätiologisch wichtigsten Erreger freigesetzt. Das neue Verfahren erlaubt frühe Mobilisierung des Patienten, erleichtert die Pflege und vermindert die Krankenhauskosten. Systematische Antibiotica-Therapie ist nicht erforderlich.
    Notes: Summary Gentamicin-PMMA chains represent a new form of local antibiotic therapy of chronic bone and soft tIssue infections. After radical removal of sequestered bone, soft tIssue necroses, and alloplastic material the infected cavity is filled with the chains and closed. The antibiotic is released in high concentrations far above the MIC and MBC of the etiologically important pathogens. The new method permits very early mobilization of the patient, eases the nursing care, and reduces costs because of shorter hospital stay. Systematic antibiotic therapy is not neceBary.

    Description / Table of Contents: Zusammenfassung Es wird über 31 Patienten mit infizierten Pseudarthrosen des Oberschenkelknochens berichtet, bei denen zur Osteosynthese der Verriegelungsnagel verwendet wurde. Bei 26 Patienten konnte knöcherne Konsolidierung erzielt werden, bei 3 Patienten verblieb ein straffes Falschgelenk, so daß ein Schienenhülsenapparat verordnet werden mußte. In einem Fall konnte durch nochmalige Osteosynthese mit dem Verriegelungsnagel schließlich Überbrückung der Pseudarthrose erreicht werden. Wegen fortbestehender Pseudarthrose und der chronischen Osteomyelitis auch nach Nagelentfernung war zweimal Amputation erforderlich.
    Notes: Summary 31 cases of infected pseudarthrosis of the femur were re-osteosynthesized with the interlocking nail. Bony consolidation was obtained in 26 patients, in all cases the chronic osteomyelitic infection subsided with removal of the nail. Pseudarthrosis persisted in 3 cases, after removal of the nail the patients could walk with a walking splint. In one of this group bony consolidation was obtained after repeated interlocking nailing. Persisting pseudarthrosis and infection was the indication for amputation in 2 cases.

Mögliche Ursachen eines Strukturdefizits werden diskutiert und eine seltene Komplikation nach Totalendoprothesenimplantation bei Osteoradionekrose vorgestellt.
    Notes: Summary In the environment surrounding the total femoral head prosthesis, the biological reaction of living bone tissue is influenced not only by the irritating activity of the foreign body, i.e. by the thermic, chemical or bacterial damages. The reaction is also determined by the primary condition of the osseous groundwork in the area of implantation. Any possible causes of structural faults are discussed and a rare complication is demonstrated: In a case of radioosteonecrosis of the hip, the delayed result of a prosthesis-implantation was disappointing.
