    Description / Table of Contents: Summary The liver of 45 cases of hemochromatosis was thoroughly studied pathologically with special attention paid to the cirrhotic changes. In 15 of the 45 cases there was a particularly regular perilobular cirrhosis, which usually was characterized macroscopically by a massive hypertrophy, by finely granular and uniform external and cut surfaces, and by a notably intense brown discoloration. Microscopically there was a perilobular, non-destructive cirrhosis with strikingly little parenchymatous damage. It is presumed, that this perilobular cirrhosis represents the typical picture of theprimary hemochromatosis. Except for the unusual pigmentation, the livers of the 30 remaining cases of hemochromatosis resembled non-specific septal, post-necrotic, and Laennec's cirrhosis, thus representing probablysecondary hemochromatosis associated with primary hepatic cirrhosis of various types.
    Notes: Zusammenfassung Bei 45 HÄmochromatosefÄllen werden die Lebern pathologisch-anatomisch eingehend untersucht, wobei besonders das cirrhotische Bild berücksichtigt wird. 15 dieser 45 FÄlle weisen eine besondere perilobulÄre Cirrhose auf, die makroskopisch in der Regel durch eine massive Hypertrophie, ferner durch eine feingranulierte und regelmÄ\ige Ober- und SchnittflÄche und durch eine besonders intensive BraunfÄrbung charakterisiert ist. Mikroskopisch liegt eine perilobulÄre, nicht destruierende Cirrhose mit auffallend geringem Parenchymschaden vor. Es wird vermutet, da\ es sich bei dieser perilobulÄren Cirrhose um das typische Leberbild derprimÄren HÄmochromatose handelt. Die Lebern der übrigen 30 HÄmochromatosefÄlle entsprechen, abgesehen von der ungewöhnlichen Pigmentierung banalen septalen, postnekrotischen und Laennecschen Cirrhosen, wahrscheinlichsekundÄren HÄmochromatosen bei primÄren Lebercirrhosen verschiedenster Art.

    Notes: Zusammenfassung Es wird über Versuche berichtet, mit Hilfe von Desferrioxamin histologisch mit der Turnbull-Reaktion nachweisbares Eisen, vor allem Hämosiderin, aus fixiertem und frischem Gewebe von Hämochromatoselebern zu entfernen. Schnitte von 10 µ Dicke werden in 2Tagen durch eine 2%ige Desferrioxamin-Lösung histologisch praktisch vollständig von färbbarem Eisen befreit. Werden die herausgelösten Eisenmengen bestimmt, so werden aus Schnitten, die 24 Std inkubiert sind, bis zu 57% der berechneten, darin enthaltenen Eisenmengen herausgelöst. Das Desferrioxamin erweist sich damit auch in vitro als geeignetes Mittel, Eisen, besonders Hämosiderin, in größeren Mengen aus dem Gewebe herauszulösen.

    Notes: Summary In this randomized, controlled multicenter trial we evaluated the effects of recombinant interferon-α2b on galactose elimination capacity and histological activity index in 88 patients with chronic active hepatitis non-A/non-B. Forty-five patients were randomly assigned to treatment with interferon at 1.5 × 106 U three times per for 1 year; 43 patients were assigned to no treatment. A complete response (normalization of alanine aminotransferase) was observed, respectively, in 47% and 5% of the two groups (P〈0.006); 47% of these patients suffered a relapse. Thus 22% of patients had a sustained response. Histological activity decreased significantly in responders (P〈0.04) while the biopsy score did not change significantly in nonresponders. In contrast, galactose elimination capacity — a surrogate marker for survival in chronic active hepatitis — was not affected by response to treatment. None of the parameters evaluated, including hepatitis C virus RNA, was able to predict response or relapse. We conclude that low-dose interferon treatment for 1 year is as effective as the recommended treatment schedule.
