    Notes: To understand the impact of earthquake experience and education on awareness, a survey was conducted with 1,065 high school first grade students from five prefectures of Japan. Results showed that earthquake experience is not the prime factor to enhance awareness. Education, when it is confined to school education, can provide useful information as the knowledge base for earthquake. However, in the gradual path of knowing, realizing, deepening, decision and action, family, community, and self education are found to be more prominent. While, self education is important for realizing and deepening, family and community education play the most vital role for decision and actions. In school education, more active ways of disaster education through conversation, experiencing, and visual aids are found to be more effective. It is believed that school education, coupled with self, family and community education can help a student to develop a "culture of disaster preparedness", which, in turn, will urge them to take right decisions and actions as an adult.

    Notes: Abstract Non-small cell lung cancer with carcinoma-tous pleuritis is considered to be a contraindication of surgical resection. The objective of this study was to clarify the prognosis of patients with non-small cell lung cancer in whom carcinomatous pleuritis was found at thoracotomy. A questionnaire survey on the survival of patients with carcinomatous pleuritis found at thoracotomy between January 1985 and December 1994 was conducted by the Japan Clinical Oncology Group. According to the data collected from 21 hospitals, 8 813 patients with non-small cell lung cancer underwent thoracotomy, 284 (3.2%) of whom were found to have carcinomatous pleuritis. Information on survival was available for 227 of these patients, 34 (15%) of whom underwent thoracotomy alone without resection, whereas 193 (85%) underwent surgical resection. Of the 193 resected patients, 155 had no macroscopical residual tumor apart from the carcinomatous pleuritis. The 5-year survival rate was 14%. According to a univariate analysis, female sex, the presence of adenocarcinoma, a tumor size of less than 3.0 cm, no clinical lymph node metastasis, and no macroscopical residual tumor had a significantly favorable impact on survival. A multivariate analysis revealed that the extent of clinical lymph node metastasis (P = 0.006), histology (P = 0.028), and the absence or presence of a macroscopic residual tumor after the operation (P = 0.045) were predominant prognostic factors. The 5-year survival rate of 83 patients with three positive variables was 24%. The prognosis of patients with adenocarcinoma found to have carcinomatous pleuritis at thoracotomy was not necessarily unfavorable if there was no clinically detected lymph node metastasis and no residual tumor apart from the carcinomatous pleuritis.

    Notes: Abstract The major issue in treating metastatic liver cancer is: how far should we perform resection? We believe that only reports of long-term survival afford an answer to this problem. We report three such patients. The first patient underwent pancreatoduodenectomy for cancer of the papilla of the duodenum and resection of metastatic liver cancer. She is alive without recurrence 15 years and 1 month after the initial surgery. The second patient received low anterior resection for rectal cancer, extended right lobectomy for liver metastasis, and pancreatoduodenectomy for metastasis at the common bile duct. She survived 6 years and 9 months after the initial surgery. The third patient underwent right nephrectomy for Wilms' tumor (adult type), extended right lobectomy for liver metastasis, and repeat resection of recurrences at the mediastinum and in the thoracic and abdominal walls. She is alive 21 years and 2 months after the initial surgery. These experiences have prompted us to carry out resection when surgery is deemed feasible.

    Notes: A non-linear model is proposed to characterize the mechanical behaviour of compact bone and has been validated by mechanical tests on bovine femoral compact bone. The model consists of a non-linear spring and a non-linear dashpot in parallel. The method of identifying the viscoelastic parameters in the model is discussed. Impact and quasi-static compressive mechanical tests have shown that the model can be applied to bone to represent its mechanical behaviour at strain rates up to 100 s-1. The directional dependence of the viscoelastic parameters for the bone are determined and discussed.

    Notes: Abstract An experimental study was conducted to investigate whether a segment of autogenous esophagus with its lumen supported by an expandable metallic stent (EMS) could be successfully used as a tracheal substitute. Seven rings of the cervical trachea were circumferentially removed and reconstructed by interposing a pedicled segment of the esophagus with an EMS in six mongrel dogs. The interposed esophagus was observed endoscopically at various stages after the operation. By 1 month, the EMS was found to be buried under the esophageal mucosa, and the lumen was patent. The dogs died 5, 17, 61, 92, 210, and 478 days after the operation, but the cause of death could not be determined by postmortem examination and no respiratory tract complications were detected. The results of this study indicated that a segment of the esophagus with its lumen supported by an EMS could work as a reliable tracheal substitute, though its practical use is not clinically feasible. Nevertheless, we conclude that some autogenous material other than the esophagus with its lumen supported by an EMS could provide a feasible method for tracheal replacement.

    Notes: Abstract Pulmonary angioplasty was successfully performed in two patients using the vascular clip system (VCS). The portion of the pulmonary artery which was directly invaded by primary lung cancer was partially resected. The VCS was used to quickly and easily repair the vascular defect. No bleeding was noted from the clipped vascular walls. Both patients had uneventful recoveries.
