    Notes: Although the pathogenesis of chronic ulcer still remains undefined, one of the characteristics is poor granulation of wound bed, in other words, disruption of wound angiogenesis. Experimental studies have shown that bone marrow (BM) derived endothelial progenitor cells take part in postnatal neovascularivation in cutaneous wound healing. BM cells also contain mesenchymal stem/progenitor cells with pluripotency differentiating into myofibroblasts and fibroblasts. We treated nonhealing leg and foot ulcers sustained over one year by topical transplantation of autologous fresh unfractionated BM-impregnated collagen sponge (BMiCS). In all patients, the treatments led to rapid generation of well-vascularized granulation tissue. All of the wounds were healed up completely with conservative treatment or skin grafting. We suggest that direct transplantation of autologous bone marrow cells on the wound may represent a novel procedure for regeneration of disrupted wound healing on recalcitrant cutaneous ulcers.

    Notes: Seven cases of angioma serpiginosum were examined in our clinic during a period of 15 years. A histological study performed in six of them revealed an increased number of ectatic capillaries in the dermal papillae. One of the cases, a 31-year-old woman, was studied by electron microscopy. The dilated capillaries consisted of endothelial cells, pericytes and veil cells. Walls of capillaries showed a heavy precipitation of fine fibrillar material admixed with collagen fibers. Most vessel walls were composed of two layers. The inner layer consisted of fine fibrillar material and the outer layer consisted of bundles of collagen fibers of about 40 nm in diameter. Characteristic findings in the endothelial cells were the formation of accessory lumen or slit-like protrusion of lumen into the endothelial lining. It appears that angioma serpiginosum is not a simple telangiectasia of preexisting vessels but represents a type of capillary nevus, which has a tendency to manifest itself in the form of capillary dilation and proliferation.

    Notes: Electron dense membrane limited granules were observed in basal cells of the outer root sheath and in connective tissue sheath of the anagen sinus hair of mice. These granules were oval or round in configuration, and were 150–180nm in size. They were considered to represent structures related to collagenolytic activity to accelerate elongation of anagen hair.

    Notes: Six examples of basal cell epithelioma were compared to fetal and adult skin. Five parameters were used in this study: abnormally large aggregates of melanosomes in epithelial cells; the paucity of half-desmosomes; the increase of microfilaments in the cell periphery; clustered membrane-limited granules; and desmosome-like structures of stromal fibroblasts. All of these indicators were positive in both basal cell epithelioma and fetal hair follicle; but they were negative in both fetal and adult epidermis. Therefore, it was concluded that basal cell epithelioma seems to mimic fetal primary epithelial germ. Interestingly, stromal fibroblasts of basal cell epithelioma phagocytize collagen fibers, and membrane-limited granules were found both inside and outside the tumor cells facing the digested collagen stroma. In view of the collagenolytic activity of the homogenate of basal cell epithelioma, membrane-limited granules might represent zymogen granules for collagenases.

    Notes: A 35-year-old Japanese woman developed vitiligo with an inflammatory erythema on the face two years after the onset of bilateral uveitis and dysacusis. Histological studies of the lesions revealed the loss of pigment and dense mononuclear cell infiltration around the blood vessels. At the dermoepidermal junction area, a few eosinophilic masses were found. Electron microscopy disclosed that the masses were filamentous masses (colloid bodies) and amyloid. After the oral administration of prednisolone, the erythema subsided, and the vitiligo showed some regimentation.

    Notes: A 14-year-old Japanese male showed numerous small cutaneous hemangiomas and severe anemia produced by internal hemorrhages from intestinal hemangiomas. Histological studies revealed ectatic vessels lined by a single layer of endothelial cells. Six hemangiomas, with fibrous walls of variable thickness, showed characteristics of vegetant intravascular hemangioendothelioma (Masson) in an organizing thrombus. Electron microscopy confirmed that the dilated cavities, were lined by a layer of flattened endothelial cells, surrounded by a few pericytes and/or smooth muscle cells. No appreciable abnormalities were found in the blood vessels located in the clinically normal skin.A few amyloid deposits in the subepidermal connective tissue were also observed.

    Notes: Pigmentation is a characteristic clinical feature of basal cell carcinomas (BCCs) in Japanese patients. The pathogenesis of melanin pigment in pigmented BCCs is poorly understood.Methods: We have combined the techniques of morphometric analysis and electron microscopy to assess accurately the morphologic aspects of melanocytes that occurred in pigmented and non-pigmented areas of pigmented BCCs.Results: In the pigmented areas melanocytes were not only located along the basal membrane but also interspersed between tumor cells in the central parts of the tumor nest, and had large and numerous dendrites. Those in a supra-basal location displayed some degree of degeneration due to mitochondrion and melanosome swelling. In the non-pigmented areas melanocytes were only basally located, showed fewer dendrites, and frequently showed abortive melanosomes. However, melanocytes in these two different portions were in the active state of melanogenesis and proliferation. Ultrastructural cytomorphometric analysis also showed significant differences in most of the nuclear and cell parameters including nuclear and cell area, the nuclear/cell area ratio, cell perimeter and cell form factor between these two types of melanocytes. Particularly melanocytes in the pigmented areas were twice the cell size of the latter. In addition, the melanosomes remained almost completely in the apoptotic tumor cells, and the phagocytosis of the melanosome-containing apoptotic cells by the neighboring tumor cells appeared to be followed by the formation of the melanosome complexes.Conclusions: These findings suggest that different populations of melanocytes are probably present in pigmented BCCs, and repeated cycles of phagocytosis of melanosome-containing apoptotic cells may represent the predominant way of forming large melanosome complexes. The present morphological observation and quantitative analysis provide a morphological basis for further studies to interpret other pathologic changes in pigmented BCCs.

    Notes: Background:  Angiosarcomas arise in the scalp and face in the elderly, in association with chronic lymphedema (Stewart–Treves syndrome), and in irradiated areas. Rarely in these settings, angiosarcomas exhibit pure spindle cell phenotype.Methods:  Herein, the clinicopathologic features of a 72-year-old-woman with spindle cell angiosarcoma are described.Results:  A 72-year-old woman presented with numerous nodules and diffuse induration from the lower abdomen to the right buttock, corresponding to the area exposed to 60Co-irradiation during treatment for cervical carcinoma 10 years earlier. Histopathological examination revealed inflitrative, atypical, spindle cells that labeled with antibodies to CD31, CD34, and factor VIII-related antigen. Ultrastructurally, these malignant spindle cells contained Weibel–Palade bodies. No features suggesting radiation dermatitis (sclerosis or bizarre, large fibroblasts) were identified, but lymphangiectases and widely spaced collagen bundles(lymphedema) were prominent in the skin surrounding the angiosarcoma. Computed tomographic scan of the abdomen highlighted this histologic finding by demonstrating tumor masses limited to areas of lymphedema. Treatment with intravenous and local injections of recombinant interleukin 2 (rIL 2) followed by electron beam irradiation were initially effective with tumor remission for 2 months. However, the recurrent tumor did not respond to a second course of one-shot injection of rIL 2 through the abdominal aorta and the patient succumbed to her angiosarcoma 19 months after diagnosis.Conclusions:  Radiation-induced lymphedema may be a factor in angiosarcoma associated with radiotherapy.
