Résumé Les pays germanophones (Autriche, Allemagne, Suisse) ne sont pas les plus avancés pour ce qui ressort du contrôle des maladies liées au tabagisme. Dans ces pays, il reste à convaincre les instances médicales que l'arrêt du tabagisme est non seulement nécessaire mais réalisable. C'est la raison pour laquelle le dévelopement d'un consensus a été initié pour fournir un document de base en vue d'activitiés futures. Le dévelopement de ce consensus s'intègre dans les activités réalisées par le «Projet pour l'Arrêt du Tabagisme» de l'UICC (Union Internationale Contre le Cancer), un projet rentrant lui-même dans le cadre du Programme sur le Tabac et le Cancer de l'UICC. Le rapport de 1988 de l'US Surgeon General établit clairement que l'usage de produits tabagiques n'est pas une question de choix mais résulte d'une accoutumance comparable à l'accoutumance à l'héroine et à d'autres narcotiques. La littérature scientifique abonde d'informations sur les méthodes et techniques pour arrêter de fumer, tant pharmacologiques que non pharmacologiques. Plusieurs approches pharmacologiques ont été essayées pour traiter la dépendance à la nicotine chez l'homme. Il ressort de travaux scientifiquement valides et d'études contrôlées que la nicotine ellemême est la seule substance efficace pour traiter la dépendance à la nicotine. Plusieurs techniques existent actuellement pour contrôler l'accoutumance à la nicotine, allant de l'usage de la simple volonté de s'en sortir à des thèrapeutiques combinées sophistiquées incluant des interventions pharmacologiques. L'arrêt du tabagisme nécessite du temps, des efforts et d'autres ressources si bien que la question du remboursement pour ces services doit être considérée. De même que pour n'importe quel autre traitement, ces coûts incombent soit à l'individu, au client et/ou á d'autres sources incluant les systèmes d'assurances privés ou nationales. Les systèmes de santé devront apprendre à gérer ces nouveaux types d'intervention et les intégrer aux autres aspects «classiques» des interventions médicales qui incluent mesures de diagnostic, traitement, évaluation du traitement, contrôle des rechutes et remboursement. Cependant, l'arrêt du tabagisme ne sera jamais qu'un seul des moyens disponibles dans le cadre d'une approche globale pour contrôler la plus importante des épidémies modernes, à savoir l'atteinte à la santé causée par la consommation de tabac. D'autres mesures sont en effet nécessaires telles que l'introduction par la loi d'espaces sans fumée dans les lieux publics, l'interdiction de la publicité pour les cigarettes, l'obligation d'inclure des notices d'avertissements sur les boîtes de cigarette.

Summary The german speaking countries (Austria, Germany, Switzerland) are not the most advanced in the world when it comes to the control of tobacco related diseases. The medical system in these countries has yet to be convinced that smoking cessation is not only necessary but feasible. In order to provide a basic document for further activities, a consensus development process was initiated. The consensus development process forms part of the activities performed by the UICC (International Union Against Cancer) Project Smoking Cessation, which is a project within the framework of the UICC Tobacco and Cancer Programme. The 1988 Report of the US Surgeon General states very clearly, that the use of tobacco products is not a matter of free choice, but is the result of an addiction as scientifically valid as the addiction to heroin and other narcotics. The scientific literature provides informations on many methods and techniques for a smoking cessation, both pharmacological and non-pharmacological. Many pharmacological approaches have been tried to treat nicotine dependence in man. According to scientific standards and many controlled studies, nicotine has been the only drug found to be effective in treating nicotine dependence. Many techniques, ranging from self help to sophisticated combined therapeutic approaches including pharmacological interventions, are now available to deal with the nicotine addiction. Smoking cessation treatment uses time, effort and other resources and the question of reimbursement for these services has to be considered. As with any other form of treatment, costs needed to be paid either by the individual patient/client and/or by other sources including private and national health insurance systems. The health care system will learn how to use this new kind of intervention and to incorporate it into the “classical” ingredients of medical intervention which include diagnosis, treatment, evaluation of therapy, relapse control and reimbursement. However, smoking cessation will always form only one aspect within the framework of a comprehensive approach to control the most important modern epidemic, that is the damage to health caused by tobacco consumption. Additional important measures such as the introduction of smokefree areas on public places by legislation, ban of tobacco advertisement, health warnings on cigarette-packets are necessary.

Zusammefassung Die deutschsprachigen Länder (Österreich, Bundesrepublik Deutschland, Schweiz) zählen in bezug zur Kontrolle tabakassoziierter Erkrankungen nicht gerade zu den fortschrittlichsten Staaten der Welt. Das medizinische System dieser Länder ist bis heute nicht überzeugt, dass die Raucherentwöhnung nicht nur notwendig sondern auch machbar ist. Um ein entsprechendes Basisdokument für die weitere Vorgangsweise zu schaffen, wurde ein Konsensusprozess eingeleitet. Dieser Konsensusprozess stellt eine Teilaktivität des UICC-(International Union Against Cancer) Projekts “Raucherentwöhnung” dar, das im Rahmen der UICC-Tabak- und Krebs-Programme durchgeführt wird. Der Bericht der US Surgeon General von 1988 besagt deutlich, dass der Konsum von Tabakprodukten nicht nur eine Sache der freien persönlichen Entscheidung ist, sondern dass sehr wohl auch eine Abhängigkeit vorhanden ist, die wissenschaftlich genauso belegt werden kann, wie die Abhängigkeit von Heroin oder anderen Narkotika. Die wissenschaftliche Literatur liefert Informationen über zahlreiche Methoden und Techniken zur Raucherentwöhnung, sowohl pharmakologischer als auch nicht-pharmakologischer Art. In der Pharmakologie wurde vieles erprobt, um die Nikotinabhängigkeit behandeln zu können. Aufgrund des heutigen Wissensstandes und zahlreicher kontrollierter Studien wird Nikotin als die bislang einzige Substanz erachtet, von der wissenschaftlich belegt ist, dass mit ihrer Hilfe die Nikotinabhängigkeit effektiv behandelt werden kann. Viele Methoden, beginnend bei der Selbsthilfe bis zu sehr umfassenden kombinierten therapeutischen Ansätzen reichend — einschliesslich pharmakologischen Interventionen —, stehen nunmehr zur Verfügung, um die Nikotinabhängigkeit zu therapieren. Rauchertherapie benötigt Zeit, Einsatz und andere Ressourcen und es stellt sich die Frage, wie diese Leistungen entsprechend honoriert werden können. Wie auch bei anderen Behandlungsverfahren, müssen die Kosten entweder vom Patienten/Klienten selbst übernommen werden oder durch andere Quellen einschliesslich privater und staatlicher Krankenversicherungen getragen werden. Das Gesundheitssystem wird lernen, mit diesen neuen Interventionsmethoden umzugehen und es mit den “klassischen” Verfahren medizinischer Interventionen — Diagnose, Behandlung, Evaluation, Rückfallskontrolle, Honorierung — zu versehen. Die Raucherentwöhnung wird aber immer nur ein Aspekt im Rahmen eines umfassenden Ansatzes der Kontrolle dieser bedeutenden modernen Epidemie, wie es die Gesundheitsschädigung durch Tabakkonsum darstellt, bleiben. Auf andere flankierende Massnahmen (z. B. gesetzliche Einführung rauchfreier Zonen in öffentlichen Gebäuden, Verbot der Tabakwerbung, Warnhinweise auf Zigarettenpackungen etc.) darf in diesem Zusammenhang nicht verzichtet werden.
