    Notes: Summary A fatal case of a postsplenectomy sepsis is presented which occured in a 5-year-old boy 11 month following splenectomy due to trauma and reimplantation of splenic tissue. The patient died 4 h after admission to the hospital. The post mortem revealed an encephalitis and a sepsis although splenic regenerates were found in the omentum pouch. Our report increases the number of cases described to date in the literature to a total of 18. Autologous reimplantation of splenic tissue does not offer complete protection against overwhelming infection.

    Notes: Summary The following case report presents the diagnostic procedures, laparoscopic therapy, and postoperative course of a 48-year-old patient with pheochromocytoma. During the previous 15 years, he had occasionally presented with hypertension, intermittent attacks of severe perspiration, and tachycardia; no diagnostic measures were performed at the time. During an ultrasound examination of the abdomen performed due to gastrointestinal complaints, a 5-cm adrenal tumor was discovered incidentally. Further diagnostic procedures then indicated the presence of a pheochromocytoma which was resected laparoscopically. The anesthesia was tolerated well, although isolated systolic blood pressure peaks to 200 mmHg were observed. The laparoscopic tumor resection presented no problems, although identifying the tumor proved to be difficult and resulted in an extended operation time of 4 h and 20 min. The postoperative course was unremarkable. This case report presents our laparoscopic technique and confirms that techniques proven in the “open” resection of a pheochromocytoma can also be utilized in the laparoscopic approach.

    Notes: Abstract The effects of motilin on gastrointestinal muscles show great variations in different organs and different species. For a precise regional differentiation, we recorded the mechanical activity of longitudinal and circular strips from fundus, corpus and antrum and of circular preparations from the inner and outer layer of the pyloric sphincter and from the duodenum (20 human stomachs). Motilin produced excitatory effects on the mechanical activity of the circular muscle strips from all regions of the human stomach including the pylorus. The effects on longitudinal preparations and on duodenal strips were weak. The most striking effect was an increase of phasic activity (amplitude) in circular antrum preparations, which exceeded the acetylcholine- and bombesin-induced activity. In pylorus preparations, a strong stimulation of phasic activity was observed with a transition to tonic activity in the inner layer of the pyloric ring at high motilin concentrations. The motilin-induced activity of the pyloric preparations was greater than the acetylcholine-induced contractions and even exceeded the bombesin-induced responses in the outer pylorus. The responses of the muscle strips of the proximal stomach (fundus and corpus) were weaker and did not exceed the acetylcholine-induced activity. All effects remained unaltered by atropine and tetrodotoxin application. The study confirms that motilin can interact directly with the smooth muscle of human stomach.

    Description / Table of Contents: Zusammenfassung Von 1960 bis 1986 wurde an unserer Klinik bei 82 Patienten eine Resektion eines Meckelschen Divertikels vorgenommen. Eine Auswertung der Krankheitsverläufe erbrachte folgendes: Die Diagnose ist präoperativ nur selten bekannt (4%), allenfalls im Blutungsfall kommen Angiographie und Szintigraphie zum Einsatz. Heterotopes Gewebe findet sich häufiger bei Komplikationsfällen. Insbesondere bei negativem Appendix-Befund sollte u. E. routinemässig nach dem Divertikel gesucht werden. Die geringe Zahl postoperativer Komplikationen auf der einen und das Komplikationsrisiko eines belassenen Divertikels auf der anderen Seite (Hauptrisikogruppe: Kleinkinder) rechtfertigen die prophylaktische Resektion.
    Notes: Summary Between 1960 and 1986, a resection of a Meckel's diverticulum was performed on 82 patients in our hospital. The course of the disease was retrospectively evaluated: The preoperative diagnosis is seldom known (4%) and only in cases of transanal haemorrhage are angiography and scintigraphy useful. Ectopic tissue was found more often in cases with complications. In cases with no inflammatory changes of the appendix, Meckel's diverticulum should be routinely checked. Prophylactic removal of the diverticulum is indicated, as there are few postoperative complications and a risk that the left diverticulum may be the cause of later complications.

    Description / Table of Contents: Zusammenfassung 41 Patienten, bei denen wegen einer traumatischen Milzruptur eine Splenektomie (n = 21) oder eine autologe Replantation von Milzgewebe (n = 20) durchgeführt worden war, stellten sich einer Nachuntersuchung.Ergebnisse: 1. Nach autologer Replantation läßt sich Milzgewebe szintigraphisch nachweisen. z. Bei der traumatischen Milzruptur ist die Splenosisincidenz sehr hoch. 3. Replantiertes Milzgewebe und Splenosis gewährleisten eine ausreichende Clearancefunktion. 4. Durch den Wegfall der Speicherfunktion der Milz erklärt sich eine Erhöhung der B-Lymphocyten und T-Suppressorzellen im peripheren Blut. 5. Die Replantation von Milzgewebe ist eine Alternative zur Milzerhaltung bei schweren Milzverletzungen.
    Notes: Summary A follow-up examination was performed on 41 patients who had had a traumatic rupture of the spleen (autologous replantation,n = 20; splenectomy,n = 21). In all replantation cases spleen tissue was demonstrated scintigraphically. After traumatic ruptures of the spleen, the incidence of splenosis was very high. There was good clearance function in all replantation and splenosis cases. The lack of the spleen's storage function is responsible for elevated B-lymphocyte and T-suppressor-cell levels in peripheral blood. In severe traumatic ruptures, the replantation of splenic tissue is an alternative procedure for preserving the spleen.
