    Notes: We report the case of a 28-year-old homosexual man with advanced HIV disease (CDC classification group IV A) who developed erythrodermic psoriasis which responded to calcipotriol, a topical vitamin D analogue. We believe this to be the first reported case of HIV-related psoriasis responsive to this form of treatment. Systemic therapy for HIV-related psoriasis is limited because of immunosuppressive effects. We suggest that calcipotriol may prove to be a useful therapeutic option in these patients.

    Notes: The relationship between sebum excretion rate (SER) and clinical improvement was investigated in 255 acne patients treated For 6 months with either oral erythromycin (1 g/day), minocycline (100 mg/day), oxytetracycline (1 g/day) or cotrimoxazole (400 mg/day); topical therapy was 5% benzoyl peroxide. In all but the cotrimoxazole treated group, there was a significant correlation between a high SER and reduced clinical response. This was particularly evident in those patients with an SER of greater than 2–5 μg/cm2/min. These patients showed only 17% improvement compared with 100% improvement in those subjects with an SER of 1.0 μg/cm2/min or less. The presence of obvious seborrhoea in a patient who has failed to respond to an adequate 6–month course of antimicrobial therapy, should indicate the earlier rather than later use of isotretinoin for their acne.

    Notes: Notalgia paraesthetica is a rare entity which involves the posterior primary rami of thoracic nerves T2-T6. Patients present with a localized discomfort or pruritus on the back. The condition runs a benign course and usually resolves spontaneously. However, whilst present the symptoms can be relentless and disturbing to patients.We report three patients with notalgia paraesthetica, all of whom were helped symptomatically by the topical application of the local anaesthetic cream, EMLA® (2.5% lignocaine and 2.5% prilocaine).

    Notes: The tetracycline group of antibiotics still remains the most successful oral treatment for acne. They are relatively free from side-effects apart from the occasional gastrointestinal upset or vaginal candidosis. Rarer side-effects include drug rashes,1 pigmentation with minocycline2 and a light-sensitive eruption with doxycycline.3,4 The incidence of light-sensitive rashes with doxycycline at a dose of 100 mg daily, is in the order of 3%.5 Acne does not always respond to conventional regimens of antibiotics and higher dosages may be required.6 We report a highly significant incidence of light-sensitive eruptions in patients receiving doxycycline at a daily dose of 150 mg or above.

    Notes: The relationship between hidradenitis suppurativa (HS) and hyperandrogenism is largely based on the finding of an increased free androgen index due to a low sex hormone binding globulin (SHBG). As SHBG is now believed to be regulated by factors that influence body weight, and previous studies were not controlled for body weight, we have re-evaluated the androgen status of female patients with HS. We have studied the endocrine status of 66 women with HS. Twenty-three had acne, and 23 were significantly obese (body mass index: BMI 〉30). There was no relationship between obesity and disease duration. Nineteen of 56 women were hirsute. A premenstrual flare in disease activity was reported by 32 women, but this was not related to menstrual disturbances. No consistent relationship was reported with pregnancy. Eight women with HS were menopausal at presentation, and one developed her disease 6 years after the menopause. Plasma androgens in women with HS were compared with controls matched for BMI and hirsuties. There was no difference between HS and controls. Testosterone and dehydroepiandrosterone sulphate were normal in all subjects with HS. In obese subjects, SHBG was reduced, consistent with BMI-matched controls. We have found no supporting evidence for biochemical hyperandrogenism in women with HS when compared with age-, weight- and hirsuties-matched controls. We report the continuation and primary development of HS in postmenopausal women.

    Notes: We investigated 299 patients treated 5-10 years ago with isotretinoin, and followed them for 5 years post-treatment. Of the 299 patients 22·7% required repeat courses of treatment; 17% had two courses, 5% had three courses and 1% had 4-5 courses. Response to further treatment was predictably successful, was not associated with any additional adverse reactions, and produced no persistent side-effects.Factors contributing to the need for further courses of treatment included lower dose regimens (0·1 and 0·5 mg/kg), the presence of severe acne, being a female over the age of 25 at the onset of therapy, and having a prolonged history of acne.

    Notes: A 53-year-old woman presented with erosive pustular dermatosis of the scalp 6 weeks after a bifrontal skin flap and right frontal craniotomy had been performed for removal of a suprasellar meningioma. Although some authors consider that acute local trauma is a precipitating factor for this condition,1–5 we believe that this is the first reported case following a neurosurgical procedure.
