L'étude est de déterminer d'effect de la manipulation pharmacologique de la motilité de l'intestin postopératoire sur la résistance des anastomoses coliques. Materiel et methode: Septante-et-un (ou soixante-seize) rats Sprague-Dawley ont été répartis en trois groupes: groupe 1 (n = 20; anastomose colique + injection quotidienne sous-cutanée d'1 ml de sérum); group 2 (n = 29; anastomose colique + 1,2 mg/100 g de poids corporal de metoclopramide dans 1 ml injecté par voie sous-cutanée quotidienne); et groupe 3 (n = 22; anastomose colique + 2 mg/100 g de poids corporel de hyoscine N-butylbromide dans 1 ml injecté quotidiennement par voie sous-cutanée). Les rats survivants (20 dans chaque groupe) ont été sacrifiés au 4e jour après chirurgie et ont fait l'objet d'une évaluation des adhérences. Chaque segment intestinal contenant l'anastomose a été prélevé et la pression de distension maximale a étéévalué. Resultats: Dans huit cas, le décès, dans la période postopératoire immédiate, résulte d'une déhiscence anastomotique (7 dans le groupe 2 et 1 dans le groupe 3). Le taux global d'adhérence est plus élevé dans le groupe 2 que dans le groupe 3 (P = 0,003). Le taux d'adhérence à l'anastomose est plus élevé dans le groupe 1 que dans le groupe 2 mais cette différence n'est toutefois statistiquement pas significative. La pression de distension maximale tolérable est significativement plus basse dans le groupe 2 que dans les deux autres groupes (P = 0,001). Dans tous les autres cas, une fuite de produit de constraste a pu être observée su la ligne de suture. Conclusion: L'emploi de métoclopramide (un simulateur de la motricité intestinale) au cours de la période postopératoire immédiate est associéà une augmentation du nombre de déhiscences des anastomoses coliques et, lorsque les animaux survivent, avec une diminution significative de la résistance de l'anastomose. La hyoscine (un inhibiteur de la motricité gastrointestinale) n'améliore pas la guérison des anastomoses.
    Notes: Abstract. Introduction: The aim of the study was to determine the effects of pharmacological manipulation of postoperative intestinal motility on the resistance of colonic anastomoses. Materials and methods: Seventy-one Spraque-Dawley rats were divided into three groups: Group 1 (n = 20; colonic anastomosis + 1 cc of saline solution subcutaneously, daily); Group 2 (n = 29; colonic anastomosis + 1.2 mg/100 g body weight metoclopramide in 1 cc subcutaneously, daily); and Group 3 (n = 22; colonic anastomosis + 2 mg/100 g body weight hyoscine N-butylbromide in 1 cc subcutaneously, daily). Surviving rats (20 in each group) were sacrificed 4 days after surgery and adhesions were evaluated. Each segment containing an anastomosis was removed and the bursting pressure was determined. Results: The cause of death during the early postoperative period was dehiscence in 8 cases (7 in Group 2 and 1 in Group 3). General adhesion scores in Group 2 were higher than in Group 3 (P = 0.003). The score for adhesions to the anastomosis in Group 1 was higher than in Group 2, but no statistically significant difference was found. Bursting pressure was significantly lower in Group 2 than in other groups (P = 0.001). In all cases leakage of dye was observed at the anastomosis. Conclusion: The use of metoclopramide (a gastrointestinal prokinetic agent) during the early postoperative period was associated with an increase in dehiscence in colonic anastomosis and, when animals survived, there was a significant decrease in anastomotic resistance. Hyoscine (an inhibitor of gastrointestinal motility) did not improve the healing of anastomoses.
