    Description / Table of Contents: Zusammenfassung Obwohl der Operateur geschickt zu Werke geht, sind Zuggurtungen, die mit starren Cerclagedrähten ausgeführt werden, nicht sicher übungsstabil. Starre Cerclagedrähte müssen daher als Osteosynthesematerial eliminiert werden. An ihrer statt werden Drahtseile eingeführt. Sie sind schmiegsam, leicht zu handhaben und fest zu verankern. Sie werden mit einem neuen Gerät, das ein sicheres Gefühl vermittelt, verspannt und in diesem Zustand verplombt. Die mechanischen Kenndaten einer Drahtseilosteosynthese sind wesentlich höher als die bei Verwendung homogenen Drahtes. Das OP-Set wird in einer kompletten Kassette von Aesculap geliefert.
    Notes: Summary Tension wirings, performed with conventional rigid cerclage wires, are not reliably stable due to loosening when kinks and loops in it are forcefully straightend out during aftertreatment. Instead of rigid wires a new set is recommended, consisting of standardized wire ropes, mechanical clips and a newly developed tension device, which mediates a proper sensation to the operator for the obtained tension. Er läßt sich nur schwer positionieren und nicht übungssicher verspannen. Oft bricht der, Zwirbel beim Verdrillen. Das neu konzipierte Zuggurtungsset, das in einer kompletten Kassette von Aesculap geliefert wird, besteht aus konfektionierten Drahtseilen, die mit einem neuartigen Gerät gut dosierbar verspannt und anschließend verplombt werden. Die einzelnen Schritte seiner Anwendung werden durch OP-Fotos an je einer Patella- und Olecranonfraktur gezeigt.
    Notes: Summary Rigid cerclage wire has disadvantages which oppose the realization of the principle “tension band wiring”. It is difficult to position and becomes loose when kinks and loops in it are forcefully straightened out during aftertreatment. For the reason a new set is recommended, which is delivered complete by Aesculap, consisting of standardized wire ropes, mechanical clips and a newly developed tension device. With this equipment — as shown in photographs — the tension band operation becomes technically simpler and permanent compression is granted.

Patella- und Olecranonfrakturen und -pseudarthrosen heilen durch laterale Zuggurtung stufenlos knöchern aus, well sie dauerhafte, hohe interfragmentäre Kompression gewährleistet.Starrer Cerclagedraht ist für Zuggurtungen allerdings ungeeignet, well sich die Knickbildungen und Schlaufen unter Beübung strecken und die Verzwirbelung rückdrehen kann, so daß die primär erreichte Stabilität durch Abfall der Drahtspannung verloren geht. Deshalb wird anstelle starren Drahtes ein komplettes Set für Zuggurtungen mit normiertenDrahtseden, mechanischen Klemmvorrichtungen und neu konzipiertem Drahtspanner vorgestellt, das die Anlegung einer Zuggurtung technisch vereinfacht und permanente Kompression garantiert.
    Notes: Summary The lateral tension band is a very reliable method for the osteosynthesis of the patellar and olecranal fractures and pseudarthrosis. The prior condition for its effectiveness is permanent interfragmentary compression. A rigid cerlage wire becomes loose when the kinks and loops in it are forcefully straightened out during aftertreatment. For this reason, instead of a rigid wire, a set is recommended, consisting of a standardized wire rope (made out of many filaments twisted together), mechanical clips, and a newly developed tension-device. With this equipment, the tension band operation becomes technically simpler and permanent compression is garanteed.

Die Drahtzuggurtung (Weber, AO) gilt weltweit als „dynamisches“ Standardverfahren für die Patellafraktur und vergleichbare, unter starkem Muskelzug klaffende Brüche. Ihr theoretisches Konzept liegt in der Exzentrizität der Krafteinleitung und einer Umwandlung von Zug- in Druckkräfte durch Bewegung. Daraus resultieren Nachteile, die sich in mäßigen funktionellen Ergebnissen und präarthrotischen Veränderungen niederschlagen: Gelenkstufen, Verkantung und Auswalzung der Gelenkfläche, Verlängerung durch Distraktion, verzögerte Heilung, Pseudarthrosen. Hier wird eine statische Zuggurtung beschrieben, deren Prinzip in axialer Krafteinleitung liegt. Sie wird durch 2 Seilverspannungen an den Patellaseitenflächen realisiert. Ihre röntgenologischen und die sehr guten und guten funktionellen Ergebnisse liegen mit 78 % deutlich über denen der konventionellen Drahtzuggurtung.
    Notes: Summary. The tension band, performed with rigid cerclage wire, is known worldwide as a standard operation technique for the patella fracture as well as for all fractures gaping under strong muscular tensile forces. Eccentricity of power input and transformation of tension into pressure forces are its theoretical rationale. This is a contradiction to the knowledge of engineers. Its disadvantages are: tilting and rolling-out, steps and gaps in the cartilage surface and distraction. Rigid cerclage wire is not a tensile instrument. Corresponding to this, the results are only mediocre. Here we describe a static tension band principle, consisting of axial power input by 2 lateral cables. The very good and good X-ray and functional results amount to 78 %, far exceeding the rate with the conventional wire tension band.

    Description / Table of Contents: Zusammenfassung Bei der Implantation von Hüftgelenk-Totalendoprothesen unter Verwendung des Knochenzements Palocos wird während der Polymerisation der Palacosflüssigkeit Wärmeenergie freigesetzt. Nach am Menschen in vivo durchgeführten Temperaturmessungen tritt an der Grenzfläche zwischen Palacos und Knochen im Bereich der Pfanne eine mittlere Maximaltemperatur von 50,4°C, im Bereich des Prothesenschaftes eine mittlere Maximaltemperatur von 45,2°C auf. Wegen der hohen Coagulationstemperatur des Knochenkollagens von 72°C bei hoher spezifischer Wärme und geringer Wärmeleitfähigkeit des Knochens ist mit thermischen Schädigungen in der Hüftgelenkprothetik nicht zu rechnen. Spezielle Maßnahmen zur Kühlung sind nicht notwendig; auf möglichst sparsames Auskürettieren der Knochen sollte jedoch Wert gelegt werden.
    Notes: Summary By the implantation of hip-joint endoprothesis under application of Palacos bone cement, thermal energy is released during the polymerisation of the palacos fluid. After measuring the temperature in human hip-surgery, an average maximum temperature of 50.4°C showed on the interface between palacos and bone in the socket area, and at the femur a temperature of 45.2°C was registered. Because of the high coagulation temperature of the bone collagen (72°C) by a high specific heat and low thermal conductivity of the bone, thermal damage is improbable. Special measures for cooling are unnecessary, care should, however, be paid to economical curettage of the bones.

    Notes: Summary By means of compression plate osteosynthesis, of the lateral tension band and of the treatment with external chucks the main demand for stability of osteosynthesises is derivated: by adjustified, in the resultant axial tension of the metal implantats is to produce only pressure in the entire fracture zone without detrimental osteosynthesis induced excentric powers. The joint biomechanic aspects of the three methods are worked out.

    Notes: Summary This is the analysis of external forces acting on the elbow joint and their influence on the appearance of bending forces, shearing forces and tensions along the ulna and their alterations with fractures of the olecranon. The influence of flexor muscles on the fracture site is being evaluated too. The fracture of the olecranon is being defined by means of statics. All disadvantages of a dorsal tension band i.e. uncertainty to neutralize all forces acting at the fracture site adequately, irregular interfragmentary compression, creation of unwanted stearing forces and moments, rest on its excentric localisation. The optimal distribution of hard ware is being evaluated.

Ohne Wirkungsverlust ist sie auch bei den proximalen Ellenschaftbrüchen anwendbar, die bisher zu den Olecranonfrakturen gezählt wurden, weil sie auch die beugerabhängigen Kräfte im Bruch aufnimmt. Patellafrakturen stellen eine zweite wichtige Indikation dar. Alle Nachteile der „klassischen“ dorsalen Zuggurtung werden durch zentrischen Kraftansatz vermieden.
    Notes: Summary The lateral tension band generates continuous interfragmentary compression through two wires alongside the ulna. Without loss of compressive forces this procedure can be applied to proximal ulna shaft fractures which until now have been considered among fractures of the olecranon because the flexion forces at the fracture site are neutralized. Fractures of the patella are the second main indication for this procedure. All disadvantages of the “classical” dorsal tension band are being avoided by centric compression.
