    Notes: Background  5-aminolaevulinic acid (ALA) is used as a protoporphyrin IX-precursor for the photodynamic therapy of superficial skin cancer and cutaneous metastases of internal malignancies. However, the permeability of hydrophilic ALA across the skin is very low.Objectives and methods  The objective of this study was to optimize and enhance the in vitro skin permeation of ALA by two resurfacing techniques: erbium:yttrium-aluminium-garnet (Erb:YAG) laser and microdermabrasion. Light microscopic changes in pig skin caused by these techniques were also compared. The electrically assisted methods, iontophoresis and electroporation, were also used to facilitate ALA permeation across laser- or microdermabrasion-treated skin.Results  Among the modalities tested in this study the Erb:YAG laser showed the greatest enhancement of ALA permeation. The laser fluence was found to play an important role in controlling the drug flux, producing enhancement ratios from 4-fold to 246-fold relative to the control. The skin permeation of ALA across microdermabrasion-treated skin was approximately 5–15-fold higher than that across intact skin. Both the ablated effect of the stratum corneum (SC) and ALA flux were proportional to the treatment duration of microdermabrasion. The application of iontophoresis or electroporation alone also increased the ALA permeation by approximately 15-fold and 2-fold, respectively. The incorporation of iontophoresis or electroporation with the resurfacing techniques caused a profound synergistic effect on ALA permeation.Conclusions  This basic study has encouraged the further investigation of ALA permeation by laser or microdermabrasion.

    Notes: Summary Rosai–Dorfman disease (RDD; sinus histiocytosis with massive lymphadenopathy) and Langerhans cell histiocytosis (LCH) are two different yet pathogenetically related histiocytic disorders. While systemic and localized forms have been identified in both diseases, each has its own characteristic histological, immunohistochemical and ultrastructural profile. Rarely, either RDD or LCH can also occur in the context of certain malignant neoplasms. However, the coexistence of RDD and LCH has never been described. We report a case of cutaneous RDD in which a focus of LCH was found. Clinical and laboratory examinations revealed no evidence of extracutaneous involvement of RDD or LCH. We believe that this is the first report of such a coexistence, and the possible pathogenesis is discussed.

    Notes: The clinicopathological features of 37 patients with invasive melanoma of the conjunctiva have been studied. Prognosis was closely related to the subsite and size of the primary tumour. Twenty of 21 patients with small localized bulbar neoplasms (95%) and four of six patients with diffuse bulbar melanomas (67%) have survived with no evidence of secondary spread. By contrast, only one of six patients with neoplasms involving the fornix (17%) and two of four (50%) with caruncular melanomas have survived. Metastatic spread was very uncommon in patients with melanomas less than 1.5 mm in maximum thickness, but the outcome of the disease in patients with tumours 〉1.5mm was not always bad. Treatment by local excision biopsy was followed by a high rate of conjuctival recurrence (59%). Exenteration of the affected eye guarded against the development of further orbital disease, but was not infrequently followed by the appearance of metastases. Many (62%) of the tumours appeared to have arisen in a pre-existing melanotic lesion or pigmented naevus of long-standing. Histologically, the tumours could be divided into those with an adjacent intra-epithelial component, manifest as atypical melanocytic hyperplasia in the conjunctival epithelium adjacent to the invasive melanoma, and those without (nodular melanoma). However, clear separation of the former group into the subtypes described for cutaneous melanomas proved impossible. Prognosis was not related to the type of melanoma, mitotic rate, cell type or degree of pigmentation.

    Notes: Summary Three malignant melanomas of the iris, classified as spindle-B type by paraffin histology, have been studied by electron-microscopy. The cells from each tumour were similar at the level of fine structure, but there was some variation in appearances between the tumours and the pattern suggested stages of dedifferentiation. The fine structural features of the tumours differed in no significant way from the previously published descriptions of posterior uveal melanomata and indicated an origin from stromal melanocytes.

    Description / Table of Contents: Zusammenfassung In einer elektronenmikroskopischen Studie der Intercellularverbindungen des Trabekelwerkes bei drei Primatenarten und beim Kaninchen wurden zwei Typen von Verbindungen identifiziert. Diese fanden sich gleichermaßen bei allen vier Species. Die eine Intercellularverbindung wurde als Macula adhaerens klassifiziert, während die andere nach en-bloc-Behandlung mit Uranylacetat für eine maculäre Gap junction gehalten wurde. Obwohl das Vorkommen dieser Verbindungen in Einzelschnitten relativ selten war, fanden sie sich doch überall dort, wo im Trabekelwerk Zellen aneinander stießen. Ihre mögliche Rolle im Sinne eines Adhäsionsmechanismus wird diskutiert.
    Notes: Summary In an electron-microscopic study of cell-to-cell attachments in the trabecular meshwork of three primate species and the rabbit, two types of junctions were identified. These junctions were common to all four species. One was classified as a macula adhaerens while the other, after uranyl acetate “en bloc” treatment of the tissue, was thought to be a macular gap junction. These junctions were common to all regions of apposition in the meshwork although their incidence in isolated sections was low; their possible role as an adhesive mechanism was discussed.

    Description / Table of Contents: Zusammenfassung Mit Hilfe des Rasterelektronenmikroskopes wurde an Rhesusaffen das Verhalten des Endothels der trabekulären Seite des Schlemmschen Kanals bei Vorderkammerdrucken von 8, 15 und 22 mm Hg untersucht. Die Expositionszeit für diese Drucke betrug jeweils 1 Std. Proportional zur Höhe des Druckes wurden die Vorwölbungen der spindelförmigen Strukturen im Endothel runder. Die Zahl der Öffnungen in der Oberfläche war bei 22 mm eindeutig größer als bei 15 und 8 mm Hg. Ein quantitativer Vergleich des Gewebes bei 8 und 15 mm Hg zeigte bei 15 mm Hg eine statistisch signifikante Vermehrung der Zahl der Öffnungen.
    Notes: Summary The scanning electron microscope was used to compare the appearances of the endothelial monolayer of the trabecular wall of Schlemm's canal in the rhesus monkey after exposure to intracameral pressures of 8, 15 and 22 mm Hg for one hour. Over this pressure range, the bulges in the spindle-shaped structures in the monolayer became rounder in shape and the number of openings on the surface was apparently greater at 22 mm Hg than at 15 and 8 mm Hg. A quantitative comparison between the tissue at 8 and 15 mm Hg showed a statistically significant increase in the number of openings on the bulges at 15 mm Hg.
