    Notizen: Summary 96 patients with lumbar spinal stenosis were operated on after two to sixteen years of disabling symptoms. There were 50 women and 46 men with a mean age of 59 years. 33 of the patients had been previously operated on for spinal complaints. A laminectomy was performed in 61 patients; at one level in 31 patients, at two levels in 23 patients, and at three levels in seven patients. A hemilaminectomy was performed in 35 patients; at one level in 28 patients, at two levels in five patients, and at three levels in two patients. There were neither internal fixation devices used, nor spondylodesis performed in these operations. Special attention was focused on the effect of age, sex, body mass index and smoking, as well as previous surgery and extent of surgical intervention on the outcome of operative treatment. The follow-up time was 3–11 years (5.5 mean). Laminectomy at one level resulted in significantly most acceptable results of operative treatment methods. Younger patients and women were more prone to inferior results of operative treatment. Also the extent of surgical intervention, overweight and smoking seemed to have a tendency to worsen the result of operative treatment.

    Notizen: Abstract Eighty-two patients (50 women, 32 men) underwent isokinetic muscle testing on average 13 years after a conservatively treated unilateral primary patellar dislocation. Three study groups were formed according to the natural history of recovery: group A (n = 32), patients with only primary conservative treatment; group B (n = 34) patients with conservative (group B1; ¶n = 24) or surgical (group B2; n = 10) treatment of redislocations; group C (n = 16) patients with other residual complaints (anterior knee, pain subluxations) requiring surgery. The Cybex 6000 dynamometer system was used as the testing machine for quadriceps and hamstrings muscles, with proportional deficits of peak torque as the test parameter. Isokinetic testing revealed both quadriceps and hamstring muscle atrophy even after long-term recovery from injury. There were statistically significant differences between the three study groups at both tested speeds of quadriceps muscles ¶(60 rad/s, P 〈 0.002; 180 rad/s, ¶P 〈 0.009). Groups B1 and B2 presented similar results. The muscle performance findings are probably due to more than one factor: primary immobilization, poor outcome, patellofemoral degeneration, redislocations, and residual knee complaints followed by surgery and deficiency in motor control of thigh muscle had – together or separately – an effect on muscle performance.

    Notizen: Abstract Forty-nine patients with Achilles peritendinitis (APT) (11 bilateral) and 31 patients with retrocalcanear bursitis (RCB) (5 bilateral) were treated surgically (altogether 96 heels). There were 37 men and 12 women in the APT group, with a mean age of 38.4 years, and 26 men and 5 women in the RCB group, with a mean age of 32.3 years. Forty-five patients in the APT group and 30 patients in the RCB group were active in sports. All patients had been treated conservatively for at least 6 months (range 6 months to 13 years) without relief of symptoms. The operative method was bilateral longitudinal incision of fascia cruris and trimming of the adhesions to fascia and base of Kager's triangle in APT group, and ablation of the posterior upper corner of os calcaneus in RCB group. In order to assess the ability to return to sports, the healing results were evaluated by questionnaire in 42 patients (47 operations) in the APT group and 25 patients (28 operations) in the RCB group 2–11 years postoperatively. The results were excellent in 27, good in 11, fair in 7 and poor in 2 in the APT group, and excellent in 13, good in 10, fair in 2 and poor in 3 in the RCB group, respecitvely. Operative treatment of APT and RCB in patients whose symptoms persist after conservative treatment seems to give favourable results in the majority of cases.

    Beschreibung / Inhaltsverzeichnis: Résumé  Nous avons mesuré l’adaptation et le remplissage radiographique postopératoire de deux modèles différents de tiges fémorales, une tige droite avec la porosité proximale (BI-Metric, n=50) et une tige proximalement anatomique revêtue d’hydroxyapatite avec un surfaçage distal (ABG, n=26). Nous avons comparé les renseignements cliniques et radiographiques de ces deux séries de données résultant d’un suivi de cinq ans. Le remodelage osseux autour des tiges a été pris en compte. Les différences observées dans les paramètres de l’adaptation et de remplissage dans la région métaphysaire étaient minimales. Par contre dans la région diaphysaire la tige droite faisait montre de plus de contact cortical et ceci d’une manière significative. Les résultats cliniques sur ces deux groupes furent excellents. L’enfoncement (〉2 mm.) de tiges anatomiques ABG était plus fréquent. Les résultats présentés indiquent que le modèle anatomique pourrait améliorer l’adaptation et le remplissage de la tige fémorale dans la région de l’os métaphysaire.
    Notizen: Abstract  We measured the post-operative radiological ”fit and fill” of two different femoral stem designs, one with a straight proximally porous-coated (Bi-Metric, n=50) and the other with a proximally anatomic, hydroxyapatite-coated and distally over-reamed stem (ABG, n=26). A comparison was then made between the clinical and radiological 5-year follow-up data of these two series and also of the bone remodeling changes around the stems. The observed differences in ”fit and fill” parameters in the metaphyseal region were minute. However, in the diaphyseal bone the straight stem had substantially more cortical contact. The clinical results were excellent for both groups. Subsidence (〉2 mm) was more frequent with the anatomical ABG stems, although the ABG stems had better bone ingrowth in the lower metaphyseal area. The present results indicate that the anatomical design may improve the fit and fill of a femoral stem in the metaphysis. On the other hand, a looser fill of the diaphyseal bone made the stems of this type more susceptible to subsidence. The straight stem with tight diaphyseal fit showed excellent stability, but the good bone ingrowth and remodeling around the distal part of the stem indicates stress transfer through this region and increased stress shielding of the proximal metaphyseal femur.

    Notizen: Abstract The aim of our study was to assess the incidence of subluxation of the first carpometacarpal joint (CMC I) and to evaluate which degree of subluxation produces swan-neck deformity of the thumb in rheumatoid arthritis (RA) occurring over 20 years. The hands of 83 rheumatoid factor (RF)-positive RA patients with recent (≤6 months) arthritis were evaluated radiographically at onset and at 1, 3, 8 and 15 years; 68 patients were evaluated at 20 years from entry. Subluxation was assessed in millimetres and compared with the MCP-I angle measurement to evaluate the thumb deformity. A statistical end-point analysis was performed between two different grades of subluxation. Subluxation of 2–3 mm was non-specific and only one third of these thumbs showed swan-neck deformity. At the end-point, subluxation of ≥4 mm was present in 17% of the thumbs, 81% of which had the swan-neck deformity; only five thumbs did not show this deformity, but presented deformed and unstable MCP I and interphalangeal joints. The frequency of swan-neck deformity was highly significantly (p〈0.0001) increased in the thumbs with severe CMC I subluxation (≥4 mm) compared with lesser subluxation (〈4 mm). When subluxation of the CMC I exceeds 4 mm, the swanneck deformity of the thumb is a common consequence. This deformity is often progressive, and the hand function of such patients should be followed up carefully, both clinically and radiographically.
