    Notes: A series of 73 cases of adenocarcinoma of the cervix has been analyzed. The five-year survival rate for all patients seen was 41 per cent. The survival for 28 Stage I and II growths treated by combined radiotherapy and Wertheim's hysterectomy was 64 per cent.

    Notes: Summary. Colposcopy was performed in 91 women who had cervical cytology suggesting mild dyskaryosis or showing koilocytosis, all previous cytology having been normal. The final histological diagnosis was C1N III in 22%, CINII in 18%, CINI in 31%, koilocytosis alone in 14% and no abnormality in 15%. These results indicate that even with mild cytological atypia, a high proportion of patients will have more advanced disease when colposcopy-directed punch biopsy is performed.

A series of 199 patients with histologically confirmed cervical intraepithelial neoplasia (CIN) grade II or III were allocated by hospital number to receive out-patient treatment by carbon dioxide laser vaporization or large loop excision of the transformation zone (LLETZ). All patients received local anaesthesia. The women in the LLETZ group experienced less post operative haemorrhage, less discomfort, operative time was greatly reduced, and histological material was available for confirmation of the diagnosis. There was no significant difference in recurrence of CIN after treatment between the two groups. At 6 month follow-up, recurrence rates of 8.2% (CIN II) and 7.5% (CIN III) were observed in the laser-group and 5% (CIN II) and 5.3% (CIN III) in the LLETZ group. Further advantages of LLETZ are reduced capital expenditure and no hazard to the eyesight of the surgeon, but laser treatment is preferable in patients with widespread vaginal involvement.

The role of Chlamydia trachomatis, genital mycoplasmas, Campylobacter spp. and other aerobic and anaerobic bacteria in the aetiology of spontaneous abortion was investigated prospectively in 241 pregnant women at a community hospital. Sixteen women who had threatened abortions were a little younger, of lower social class and had had more previous spontaneous abortions than the 76 women who aborted or the 149 women whose pregnancies were not complicated in the early stages by haemorrhage. The demographic characteristics of the latter two groups of women were similar. C.trachomatis was isolated from the cervix of only one woman and she had no genital-tract bleeding at any stage in her pregnancy. Mycoplasma hominis was isolated most often from the women who had threatened abortions but otherwise the prevalence of the other various micro-organisms was similar in women who had spontaneous abortions, threatened abortions, and in those who had pregnancies uncomplicated by vaginal bleeding. It was clear, therefore, that C.trachomatis played no role in the aetiology of spontaneous abortion in the population studied and there was no suggestion that any of the other micro-organisms were involved either.

    Notes: The oxytocic effect of an acridine dye, aminacrine hydrochloride, has been investigated on isolated strips of myometrium from pregnant and non-pregnant human uteri. Contractions were induced when the muscle strips were perfused with an aqueous solution of aminacrine at a concentration of 10–5–10–4M. 50 ml. of an 0.1 per cent solution of aminacrine was introduced into the extra-amniotic space in 23 women in order to induce therapeutic abortion in the mid-trimester. All the patients aborted after a mean induction delivery interval of 59 hours. Most patients required a pitocin infusion to complete the abortion. The incidence of side effects and infection was minimal. The plasma and urinary levels of the dye were measured and found to be very low.It is concluded that the extra-amniotic injection of aminacrine is a safe, easy and effective method of terminating mid-trimester pregnancies.

    Notes: A series of 129 cases of adenocarcinoma of the uterus treated by Wertheim's hysterectomy and lymphadenectomy is reported. Preoperative radium was given together with postoperative irradiation when pelvic lymph nodes were involved in metastatic cancer.The five-year survival was 71·5 per cent. This figure rose to 74 per cent when the growth did not involve the cervix.The incidence of lymph node involvement was 13·2 per cent, or 11·2 per cent when the cervix was free from growth. The incidence of node involvement was related to the degree of differentiation of the tumour and the depth of myometrial invasion. Thirty-six per cent of patients with positive nodes survived 5 years when lymphadenectomy was combined with external radiation. When death occurred the metastatic deposits were usually outside the pelvis.It is concluded that lymphadenectomy need be performed only in patients with poorly differentiated and deeply invasive growth. Postoperative external irradiation is indicated if the nodes contain cancer.

    Notes: The incidence of transplacental haemorrhage has been estimated in 200 women undergoing termination of pregnancy. Vaginal termination by curettage or vacuum aspiration carried a 25 per cent risk of transplacental haemorrhage determined by acid elution.
