    Notes: Abstract Purpose: The necrotic effect of chemotherapy on primary osteosarcoma has been shown to be predictive of the final outcome. Little attention has been paid to the local response of the host (LHR), which reflects the tumour–host relationship. Design: A four-step grading system was developed based on distinct histological patterns of the LHR around the lesion. These responses were correlated with the chemotherapy-induced necrosis or chemosensitivity and analysed in an attempt to ascertain their influence on the patient prognosis. The ability of conventional radiographs and computed tomography to measure LHR was studied. Methods: The grading system was applied to macroslides of specimens obtained from 72 patients with stage II B primary osteosarcoma in various limbs after wide resection and complete courses of pre- and postoperative chemotherapy who were treated between 1985 and 1991 with a median follow-up of 5 years and 9 months. The histological specimens were blindly reviewed by two pathologists and two experienced musculoskeletal oncologists to assign a grade of response. The results were correlated with tumour necrosis, patient survival and response features on conventional radiographs and CT images. Results: Significant correlation was found between LHR and tumour necrosis or chemosensitivity (r = 0.55) and between LHR and CT response (r = 0.56). There was no correlation between LHR and the findings on conventional radiographs. A grade 4 LHR was predictive of long-term survival. Conclusions: The LHR to preoperative chemotherapy has a prognostic influence on patient survival and can be predicted by CT.

    Notes: Abstract This study was undertaken to analyse metastases of patients with intermediate- or high-grade chondrosarcomas. Out of 24 intermediate-grade tumours, 5 (21%) developed metastases, as did 6 of 10 high-grade tumours (60%) (P=0.04). Four patients developed pulmonary metastasis only, 5 developed both pulmonary metastases and metastases of the other sites. Two patients showed a rare metastatic pattern: bone metastases only. The metastasis rate in the primary chondrosarcoma (42%) was higher than that in the secondary chondrosarcomas (0%) (P=0.03). The metastasis rate was higher in patients with local recurrence (86%) than in those without local recurrence (19%) (P=0.01). In 5 of 6 patients who had a local relapse and metastasis, the interval between the two relapses was a few months.

    Notes: Abstract We analysed local recurrences in young children with aneurysmal bone cysts. Nine were boys and five were girls, aged 10 years or less. The sites of the lesions were distributed as follows: five femurs, four humerus, two tibia, one sacrum, one thoracic spine and one pubis. After intensive curettage, five patients underwent cementation and five patients bone grafting. Four patients underwent resection of the lesion. No patients received irradiation. One of the five patients who underwent cementation had a relapse and the lesion was controlled after the second curettage and cementation. One of the five patients who underwent bone grafting had a relapse, and he relapsed again after the second curettage and bone graft; finally his disease was controlled by resection of the lesion and bone grafting. After 55 months (median) of follow-up, all patients were disease-free. The local recurrence rate of aneurysmal bone cysts in children was not high in this hospital.

    Notes: Abstract  Objective. To evaluate the conventional X-ray and MR imaging features of malignant fibrous histiocytoma (MFH) of bone. Design. MRI examinations and conventional radiographs were reviewed in 39 patients with biopsy-proven MFH. Imaging characteristics were analyzed and the differential diagnoses assessed in a masked fashion by two experienced radiologists. Results. Typical X-ray features included aggressive, destructive tumor growth centrally located in the metaphysis of long bones. Periosteal reactions and expansive growth were rarely seen. On MR images extraosseous tumor spread was frequently noted. On T2-weighted images and contrast-enhanced T1-weighted images most of the tumors displayed an inhomogeneous, nodular signal pattern with peripheral Gd-DTPA enhancement. Conclusions. Although several MR imaging criteria were typical for MFH none of them was specific. X-ray diagnosis of MFH may also prove difficult, with the main differential diagnosis being metastasis in the older and osteosarcoma in the younger population.
