Zusammenfassung Eine Gruppe von 137 Hauern in Erzminen, im Durchschnittsalter von 44,7 Jahren und mit einer Durchschnittsexposition von 13,9 Jahren, die wegen Verdacht auf LÄsion des Nervensystems durch Vibrationen untersucht wurden, hatte außer der FingerlividitÄt folgende subjektive Beschwerden: verschiedene Dysaesthesien, Schmerzen im Bereich der oberen Gliedmaßen und der HalswirbelsÄule, Hypodynamie und KrÄmpfe der Muskulatur. Im objektiven neurologischen Befund sind es Hypaesthesie (am hÄufigsten handschuhförmig), Hypo- oder Areflexie mit acraler PrÄvalenz, gesteigerte idiomuskulÄre Reizbarkeit und PositivitÄt der Karpaltunnelteste. Die Untersuchung der Leitungsgeschwindigkeit der motorischen und sensiblen Fasern des N. ulnaris und N. medianus bewies fast bei der HÄlfte der Untersuchten (47,4%) pathologische Werte in einem oder mehreren Nerven. Die klinischen und EMG-Ergebnisse wurden mit Röntgenaufnahmen der HalswirbelsÄule und der Ellbogengelenke und mit Abkühlungsproben verglichen. 1. Klinische und EMG-Korrelation zeugt am hÄufigsten von SchÄdigung der motorischen Fasern des N. medianus und Ähnelt dem Karpaltunnel-Syndrom. 2. SchÄdigungen des N. ulnaris werden in der Regel gleichzeitig mit Ellbogengelenkprozessen beobachtet, wobei Erschütterungen und Vibrationen eine komplexe Wirkung haben. 3. Kombinierte LÄsion des N. ulnaris und N. medianus korreliert überwiegend mit den positiven Abkühlungstesten oder den verlÄngerten Lewis-Prusik-Zeiten. 4. Bei kombinierter SchÄdigung des peripheren und zentralen Neurons, die verhÄltnismÄßig selten vorkommt, besteht in der Regel ein grob-pathologischer Befund an der HalswirbelsÄule und ein dementsprechendes Bild einer cervicalen Myelopathie. 5. Außer den spinalen FÄllen haben die festgestellten pathologischen Befunde eine acrale PrÄvalenz. 6. Die beschriebenen VerÄnderungen erfordern eine lange Exposition (in der Regel mehr als 10 Jahre). Auch eine relativ lange Anamnese der Beschwerden spricht für verhÄltnismÄßig geringe ProgressivitÄt. Die LÄsion des peripheren Nervensystems bei der Arbeit mit vibrierenden Instrumenten ist das Ergebnis von komplexer SchÄdigung der oberen Gliedmaßen und der gegenseitigen Beeinflussung anderer geschÄdigter Systeme (GefÄßsystem, Gelenk- und Muskelsystem). Es handelt sich also um multifaktoriale Wirkung, an der auch eine Reihe PrÄdisponierender Momente teilhat.

Summary A group of 137 workers from the ore mines, mean age 44.7 years, mean exposure 13.9 years, examined on suspicion of lesions of the peripheral nervous system due to vibration, manifested besides finger lividity the following subjective complaints: acrodysesthesia, pains in the upper extremities and cervical vertebral column, hypodynamia and muscle spasms. Among the objective neurological findings were hypaesthesia (mostly in glove shape), hypo- and arreflexia with acrale prevalence, increased idiomuscular irritation and positive carpal-tunnel tests. Estimation of the conduction velocity of the motor and sensitive fibres in N. ulnaris and N. medianus proved pathological values in one or several nerves in almost half the examined persons (47.4%). The clinical and EMG results were compared with the x-ray of the cervical vertebral column and the elbow joints, and with the cooling test. 1. Clinical and EMG correlation indicates mostly a damage of the motor fibres of N. medianus and resembles the carpal-tunnel syndrome. 2. Damage of the N. ulnaris is usually observed simultaneously with the elbowjoint processes, in which shocks and vibrations have a complex effect. 3. Combined lesion of N. ulnaris and N. medianus correlates predominantly with the positive cooling tests or with the prolonged Lewis-Prusik times. 4. With combined damage of the peripheral and central neuron-though unusual-there is often a roughly pathological finding on the cervical vertebral column and an according picture of cervical myelopathy. 5. Besides the cases of myelopathy the ascertained pathological findings are of acral prevalence. 6. The reported changes require long exposure (usually more than 10 years). Even a relatively long case history of the complaints tends to prove relatively small progressivity. Lesions of the peripheral nervous system in work with vibrating instruments is the result of complex damage of the upper extemities and of the mutual influencing of the other damaged systems (vascular system, joints- and muscle system). Thus it is a case of multifactorial effect in which a number of predisposed moments participate.
