Zusammenfassung Elektronenmikroskopische Biopsiebefunde einer Myopathie, die durch ein ACTH-aktives Lebercarcinom mit Nebennierenrindenhypertrophie entstanden ist, werden dargestellt. Die Frühveränderungen bestehen aus subsarkolemmalen und intermyofibrillären Anhäufungen von Mitochondrien und Grana. In Muskelregeneraten zeigen sich abnorme Filamentaggregate, verbreiterte Z-Streifen mit beiderseitigen Filamentbüscheln und gehäufte T-Systeme mit tubulären Strukturkomplexen. Bei Myopathien sollen Gewichtsverlust, abnorme Pigmentierung, hypokalämische Alkalose, Diabetes mellitus und verminderte Infektresistenz Anlaß zu einer Hormonanalyse sein, die einen „paraneoplastischen Hypercorticismus“ aufdecken kann.

Summary Electron microscopic findings in a myopathy caused by an ACTH-producing liver carcinoma and hypertrophy of the adrenal glands are described. Early changes include the accumulation of mitochondria and grana in the subsarcolemmic and intermyofibrillar space. Many regenerating muscle fibers showed unusual aggregations of filaments, dense Z disks with attached filaments, and T-systems with tubular networks. If myopathy presents with loss of weight, hyperpigmentation, hypokalemic alkalosis, diabetes mellitus, and a diminished resistance to infections steroid hormones should be assayed in order to trace a possible “paraneoplastic hypercorticism”.
