    Notes: Summary This study was designed to investigate the effect of topical ketanserin, a selective S2-serotonin antagonist, on wound healing and contraction in surgical wounds allowed to heal by second intention. Thirty-five patients applied placebo or 2% ketanserin gel, double-blind, to head and neck wounds resulting from skin tumour excision, and allowed to heal by second intention. Healing rate was assessed from the change in the square root of wound area, and in 11 patients wound contraction was assessed by the change in the area delineated by wound margin tattoo marks. There were no significant differences in any of the measurements made between the ketanserin- and placebo gel-treated patients. Wound healing was significantly faster in large wounds. Wound contraction was greatest in the early stages of healing, and accounted for 53% of healing.We conclude that, on normally vascularized skin, ketanserin gel has no significant effect on wound healing rate.

    Notes: Summary In an attempt to improve the prediction of PUVA erythemal sensitivity, we have examined, in 2 51 patients, the relationship between the minimal phototoxic dose (MPD) and a number of variables, including skin type, ingested dose of 8-methoxypsoralen (8-MOP) and history of previous PUVA treatment. The MPD was determined by phototesting 2 h after ingestion of crystalline 8-MOP. given at a standard dose of 0–6 mg/kg (calculated to the nearest 10 mg).No reaction to the highest dose of UVA used for phototesting occurred in 16% of cases; the dose of 8- MOP was significantly associated with non-response. In the patients in whom an erythemal response was obtained, a significant association was found between the MPD and the variables of skin type, ingested dose of 8-MOP. and history of previous PUVA treatment. Thus, by taking these factors into account, a more accurate prediction can be made of an individual patient's erythemal sensitivity to PUVA than relying on skin type alone. That erythemal sensitivity is affected by the ingested dose of 8-MOP (even when all patients received 0–6 mg/kg) suggests that conventional psoralen dosing according to body weight is not ideal.

    Notes: The effect of terfenadine on the response to irradiation with blue light was measured in seven patients with erythropoietic protoporphyria. Terfenadine caused significant inhibition of the immediate flare reaction, but did not alter the erythemal response localized to the irradiation sites. Treatment with β-carotene had no effect on the flare and erythemal responses. However, the flare reaction was still inhibited by terfenadine during treatment with β-carotene.These results show that histamine release is involved in the mechanism of porphyrin photosensitivity. H1 receptor antagonists may be of use in the treatment of patients with erythropoietic protoporphyria.

    Notes: We have compared the effectiveness of oral 8-methoxypsoralen photochemotherapy (PUVA) using ultraviolet fluorescent lamps with peak emission at either 325, 352 or 370 nm in the treatment of 24 Patients with psoriasis. The forearms of each patient were treated three times weekly with two of the three lamps. The erythmal sensitivity of each patient was tested before the first treatment to ensure that equally erythemal doses of radiation were given from each of the lamps. A side-to-side comparison was used to assess response to treatment at weekly intervals for the 6 weeks of the trial. The lamp with peak emission at 325 nm was shown to be significantly superior to either of the other lamps in terms of response assessed at weekly intervals, and time to clearance of psoriasis. An action spectrum, constructed from the median doses required for clearance of psoriasis using each of the lamps showed that the effectiveness of the radiation decreased exponentially with increasing wavelength throughout the UVA waveband, such that radiation at 320 nm was an order of magnitude more effective than at 360 nm. This suggests that lamps with peak emission around 325 nm will be more dfective than those conventionally used in PUVA units with a peak emission at 352 nm. Lamps with peak emission around 325 nm are also highly effective in the treatment of psoriasis with phototherapy alone. Thus a single treatment unit containing these lamps could be used either for PUVA or ultraviolet phototherapy of psoriasis, avoiding duplication or irradiation equipment.

    Notes: We have investigated the effect on sensitization of altering the area of application of 2,4,dinitrochlorobenzene (DNCB) at a constant dose per unit area. We showed that, when an area of less than 1 cm2 is used, this area is critical in determining the degree of sensitization. This contrasts with previous work that showed, for larger areas, an alteration in the area of application had little effect on sensitization, whereas keeping the area constant and increasing the concentration of DNCB increased the degree of sensitization. We suggest that not only is the amount of antigen important in determining response, but also the distribution of the antigen as presented to the afferent limb of the immune system.

    Notes: We have investigated the differences between the sexes in the development of contact sensitivity induced by dinitrochlorobenzene (DNCB). Ten male and 12 female subjects were sensitized with DNCB (30 μg applied on a 1 cm patch test disc) and challenged 1 month later with doses of 8.8, 12.5, 17.7 and 25 μg. The responses were measured after 48 h as increase in skinfold thickness with Harpenden callipers. Females showed a larger response at all challenge doses studied, and the slope of the log-dose response curve was significantly steeper in females. We conclude that there are significant differences in delayed type hypersensitivity between males and females.
