    Notes: The pathogenesis of most chronic wounds is unknown. In this report, we summarize several areas of investigation which appear ready for further progress and which were discussed at a recent National Institutes of Health workshop on this subject. Of note were presentations of research on the role of fibrosis in wound healing, interaction of cytokines, hypoxia, extracellular matrix formation, keratinocyte migration, and involvement of proteases and neuropeptides in chronic wounds.

    Notes: Antibodies directed against intercellular epithelial structures (ICE) have been important in the diagnosis of pemphigus vulgaris. Our study of thirty-seven patients with maculopapular eruptions following therapy with penicillins, clearly indicate that antibodies morphologically indistinguishable from those found by standard techniques in pemphigus vulgaris are also found here. Indirect staining by fluorescence and horse-radish peroxidase indicated localization against ICE identical to that seen in pemphigus, in about half the patients. Direct staining of skin from two penicillin-allergic patients revealed similar staining in the familiar pattern, which closely resembles the network of lines in giraffe skin. Detection of circulating antibody directed against a component of epithelium introduces new insight into the pathophysiology of maculopapular penicillin eruptions, although further work is required to show what the exact relationship is to pemphigus vulgaris, the potentially fatal skin disease.

    Notes: : A retrospective study of patients with pemphigus vulgaris, drug eruptions, psoriasis, and normal controls was undertaken in an attempt to determine whether a significant difference in drug exposure and/or drug allergy existed between the various groups. A statistically significant difference in drug exposure was found between pemphigus vulgaris and drug eruption patients, as compared with psoriatics and normal patients. Representative case reports and some possible mechanisms for the induction or exacerbation of pemphigus patients is described and discussed.
