    Description / Table of Contents: Zusammenfassung In meinem Krankengut von 6800 Eingriffen unter peroperativer manometrischer und röntgenographischer Kontrollen seit 1942 waren nur 382 Fälle von benigner Stenose der Papilla versammelt. Fehlerquellen bestehen in Anaesthesica und Präanaesthesica, oder Iodkontrastmittel. So benutzt man hingegen nur Kochsalzlösung für die Manometrie und iodhaltige öllösung für die Cholangiographie. Es gibt zwei verschiedene Formen, wahrscheinlich verschiedene Stufen derselben Krankheit: labile Stenose mit persistierendem Spasmus, die Diagnose ist mittels der Lokalanaesthesie der Nervi Vagi gemacht; fixierte Stenose mit Schleimhaut- und Muskelveränderungen. Die Operationstypen sind: partielle Sphincterotomie, distale Vagotomie, Choledocoduodenostomie.
    Notes: Summary In my practice of 6,800 operations under peroperative manometric and radiographic control there were only 382 cases of benign stenosis of the papilla since 1942. Errors of diagnosis may be caused by the use of anesthesic or preanesthesic drugs or iodocontrast products. Only the use of a saline solution for manometry and iodo-oil for cholangiography gives no physiological trouble. There are two different forms of this disease, probably various steps of the same disease: labile stenosis with a persistant spasm. Diagnosis is made by the test of a local anesthesia of the vagus nerve: stabilized stenosis with lesions of the muscular and mucosa layers. Different operations may be performed: partial sphincterotomy, distal vagotomy, or choledocoduodenostomy.

    Description / Table of Contents: Zusammenfassung 406 Operationen wegen Postcholecystektomiesyndrom wurden mit Radiomanometrie aus 6000 Gallenwegeoperationen herausgegriffen; jedesmal wurde eine genaue Ursache festgestellt, und in 149 Fällen (37%) endete der Eingriff an den Gallenwegen mit einer Choledochoduodenostomie: vergessene Steine in den Hauptgallenwegen: 27 von 78 Fällen sekundäre Steinbildung: 50 von 71 Fällen Carcinom : 7 Pankreatitis: 3 relative Verengung des Konfluenz: 26 Sklerose des Sphincter Oddi: 12 Hypotonie der Hauptgallenwege, Erweiterung mit Stase und Infektion: 22 Fälle. Sterblichkeit: 8%. Funktionelle Spätergebnisse: ausgezeichnet in 94%, gut bei 2%, Rezidive in 4% der Fälle. Die radiographische Kontrolle der Anastomose zeigte sehr häufig eine bemerkenswerte Verringerung des Kalibers des Ductus choledochus.
    Notes: Summary 406 operations for the post-cholecystectomy syndrome were selected by radiomanometry from 6,000 operations of the bile duct. The exact cause was determined, and in 149 cases (37%) the operative intervention at the bile ducts terminated with choledochoduodenostomy: forgotten stone in the major bile ducts: 27 out of 78 cases secondary calculi: 50 out of 71 cases carcinoma: 7 pancreatitis : 3 narrowing of the confluence: 26 sclerosis of Oddi's sphincter: 12 hypotension of the major bile ducts, dilatation with stasis and infection: 22 cases. Mortality: 8%. Late functional results: excellent in 94%, good in 2%, recurrence in 4% of cases. Radiographic control of the anastomosis frequently showed great reduction of the calibre of the common bile duct.
