    Notes: Objective To study the prevalence and severity of reflux symptoms in pregnancy.Design Self-administered questionnaire detailing age, race, gestational age, parity, weight, height, symptoms and severity of gastro-oesophageal reflux.Setting An antenatal clinic in a teaching hospital.Subjects 607 consecutive women at various stages of pregnancy were recruited during attendance at antenatal clinic.Results The prevalence of heartburn increased with gestational age (22% in the first, 39% in the second, and 72% in the third trimester; P〈0.0001), as did severity of heartburn (P〈0.0001). Pharyngeal regurgitation as a symptom showed little increase during advancing pregnancy. Symptoms of heartburn rapidly increased towards the end of the second trimester. The decrease in heartburn traditionally expected during the last three weeks of pregnancy (fetal head descent) was not observed. Logistic regression analysis showed increased risk of suffering heartburn with increasing gestational age (P〈0.0001), pre-pregnancy heartburn (P〈0.0001), parity (P〈0.0001) and inversely with maternal age (P〈0.05) but not with body mass index before pregnancy, race, or weight gain in pregnancy.Conclusion Heartburn, but less so pharyngeal regurgitation, increases in prevalence and severity with gestational age and parity. We speculate that hormone-related impairment of distal oesophageal clearance mechanisms may have a crucial bearing on whether heartburn develops in those individuals at greatest risk.

    Notes: Background:  Obesity is a risk factor for adenocarcinomas of the oesophagus and gastric cardia. Diabetes mellitus might mediate that association.Aim:  To estimate the risk of diabetes mellitus on the development of adenocarcinoma of distal oesophagus and gastric cardia beyond that of gastro-oesophageal reflux disease.Methods:  A case–control study was performed using a national administrative database of the Veterans Administration.Results:  A total of 311 cases of cancer and 10 154 controls were identified. Gender, age, and race were risks for cancer. Diabetes was diagnosed in 36% of cases, and 32% of controls (P = 0.15). Diabetic complications were diagnosed in 14% of cases and 13% of controls (P = 0.60). Multiple logistic regression confirmed the absence of an association between cancer and diabetes (odds ratio 1.1, 95% confidence interval 0.8–1.5) or diabetic complications (odds ratio 0.8, 95% confidence interval 0.6–1.3), adjusting for age, gender, and race.Conclusions:  Within the limitations of this case–control study, there is no evidence of an association between diabetes and adenocarcinoma of the oesophagus or gastric cardia among US veterans with gastro-oesophageal reflux disease.

    Notes: Abstract In this paper we study almost Hermitian submersions with total space a locally conformal Kähler (l.c.K.) manifold, i.e., l.c.K. submersions. We derive necessary and sufficient conditions for the fibers of a l.c.K. submersion to be minimal and for the horizontal distribution to be completely integrable. We give, under certain conditions, some relations between the Betti numbers of the total space and the base space of a l.c.K. submersion and we obtain all the l.c.K. submersions with totally geodesic fibers and total space a particular class of generalized Hopf manifolds.

    Notes: Abstract The10B/11B isotopic ratio in the H3BO3 presented in the liquid moderator of a nuclear reactor is used as monitor of the performance of the reactor during the first year of operation. Usually this determination is performed by TIMS due to its high precision. An alternate technique to thermal ionization mass spectrometry /TIMS/ for the determination of the10B/11B isotopic ratio by SSMS is presented. The modifications introduced in the general analytical method produced very good results. Furthermore, the relative standard deviation was 5% against the typical value of 10–20%.

    Notes: Abstract An analytical method, SSMS, for the determination of trace impurities in cobalt is described. This element is used as raw material for the production of60Co in Embalse Nuclear Power Station. Tin, aluminum, lead, silicon, molybdenum, calcium, vanadium, boron, chromium, zirconium, tungsten, nickel, magnesium, iron, zinc and copper were determined. Manganese, analyzed by AAS, was used as an internal standard. The measurements were semiquantitative. The accuracy of the results was compared with that of emission spectrography and atomic absorption spectrometry, the agreement was found to be satisfactory.

    Notes: Abstract A procedure for the measurement of the isotopic composition of Fe in rradiated Na2/FeNO/CN/5/.2H2O samples by SSMS is described. Decomposition with K2S2O7, followed by extraction of Fe was necessary to eliminate interferences in the mass spectra. A study of the remaining interferences showed that they were of organic origin from the residual MIBK used for the extraction. The order of magnitude of the55Fe/58Fe and59Fe/58Fe ratios was calculated.
