    Notes: Summary Cephamandol 6.0 g, cephazolin 6.0 g or cephacetrile or cephalothin 8.0 g were administered as short-term infusions on 3 consecutive days to informed volunteers, who had no history or evidence of impairment of renal funktion. There were 15 subjects in the cephamandol, cephacetrile and cephalothin groups and 14 subjects in the cephazolin group. Alanine-aminopeptidase, a characteristic tubule enzyme, was determined in a 24-hour urine 2 days before administration, during the 3 day administration and on the 4 subsequent days. In addition, alanine-aminopeptidase was also estimated immunologically in concentrated urine with the aid of an anti-brush border antibody. Cephamandol, cephazolin and cephalothin were completely without effect on the proximal tubule. Cephacetrile, on the other hand, showed clear reactions in 9 out of 15 subjects, in the form of elevated AAP activity in urine and in 6 of the cases membrane elimination was demonstrable immunologically. After withdrawal of the medication, the values of the responder group returned spontaneously to normal, i. e. no cumulative effect was detected. These investigations show that elimination of alanine-aminopeptidase in the urine is a very sensitive index of the action of cephalosporins on renal tubules.

    Description / Table of Contents: Summary Supplementary to previous investigations with six other cephalosporins, the effect of the cephalosporin cefoxitin on the proximal tubule was examined in volunteers. The excretion in the urine of alanine aminopeptidase, a membrane enzyme of the brush border membrane of the proximal tubule, was used as parameter. It was seen that the cephalosporins have almost no toxic effect on the tubule. Therapy of more than three days' duration and the possible effects on the impaired kidney were not considered in this study.
    Notes: Zusammenfassung In Probandenstudien wurden die Wirkungen des Cephalosporins Cefoxitin, ergänzend zu früheren Untersuchungen mit sechs weiteren Cephalosporinen, auf den proximalen Tubulus untersucht. Als Parameter diente die Ausscheidung der Alanin-Aminopeptidase im Harn, ein Membranenzym der Bürstensaummembran des proximalen Tubulus. Dabei zeigte sich, daß die Cephalosporine nahezu keine tubulotoxische Potenz besitzen. In diesen Untersuchungen blieben die möglichen Wirkungen auf die vorgeschädigte Niere und eine längere Therapiedauer über drei Tage hinaus unberücksichtigt.

    Description / Table of Contents: Summary The effect of cefamandole, tobramycin and the combination of both drugs on the proximal tubule was investigated in a study with normal subjects. The elimination of the membraneous enzyme of the brush border, alanine aminopeptidase (AAP), was determined in the 24 h urine. Cefamandole exhibited no effect on the proximal tubule, whereas tobramycin produced a cumulative increase of the AAP in the urine, and the combination of both drugs resulted in an AAP increase exceeding that produced by tobramycin alone, without being statistically significant.
    Notes: Zusammenfassung In einer Probandenstudie wurde die Wirkung von Cefamandol, Tobramycin und der Kombination beider Medikamente auf den proximalen Tubulus untersucht. Gemessen wurde die Ausscheidung des Membranenzyms des Bürstensaums, die Alaninaminopeptidase (AAP) im 24 h-Harn. Cefamandol zeigte keine Wirkung auf den proximalen Tubulus, dagegen Tobramycin einen kumulativen Anstieg der AAP im Urin und die Kombination beider Medikamente gegenüber dem Tobramycin-Effekt hinaus, ohne daß er statistisch gesichert werden konnte.
