    Notes: Summary In order to investigate whether the ability of peripheral blood monocytes to bind bacteria is impaired in diabetes, we studied carbohydrate-binding (“lectin-like”) receptors and the receptor for the Fc portion of immunoglobulin on monocytes from 25 male Type 1 (insulin-dependent) diabetic patients and 10 age-matched healthy control subjects. Peripheral blood monocytes from the diabetic patients expressed lower levels of “lectin-like” receptors compared to the control subjects, whereas the expression of the receptor for the Fc portion of immunoglobulin was similar in both populations. There was no correlation between the degree of “lectin-like” binding activity and plasma glucose concentration or glycaemic control. Recognition of unopsonized bacteria by the “lectin-like” receptor is impaired in Type 1 diabetes; this may affect the efficient elimination of potential pathogens.

    Notes: Abstract This review discusses recent knowledge of the many causes of cobalamin deficiency in infancy and childhood. These causes are classified as (1) inadequate intake, (2) inadequate absorption and (3) inadequate utilisation. It is emphasized that the ‘serum B12’ may be normal in cases of the grossest derangement of B12 metabolism, with extreme functional deficiency of this group of vitamins (the cobalamins). For completeness, causes of folate deficiency and other conditions causing megaloblastosis in children are also briefly outlined.

    Description / Table of Contents: Summary The field covered by this review is expanding with great rapidity. It now presents a remarkable and in some ways confusing mélange of observations, derived from experiments using a wide range of techniques, on various species of animal, and from investigations in man in health and disease. Out of this fertile muddle, intensely stimulating to research, useful hypotheses are beginning to emerge, and provide a tentative physiological framework to which clinical observations may be related. It is essential to appreciate, however, that in a growing subject of such complexity, valid generalisations are not easy to make, and the temptation to impose order prematurely must be resisted.
    Notes: Zusammenfassung Das in vorliegender Übersicht zu den genannten Themen ausgebreitete Wissen ist in ständigem und raschem Zunehmen begriffen. Bislang stellt es eine bemerkenswerte und in mancher Weise verwirrende Mischung von Beobachtungen dar, die z.T. aus experimentellen Studien unter Verwendung mannigfaltiger Techniken und verschiedener Tiergattungen, aber auch aus Untersuchungen am Menschen im gesunden und kranken Zustand gewonnen wurden. Aus dieser zunehmenden Verwirrung, die andererseits einen intensiven Stimulus für weitere Forschungsarbeit abgibt, beginnen sich inzwischen doch brauchbare Arbeitshypothesen abzuzeichnen, die für die experimentelle Physiologie ein nützliches Gerüst abgeben, an die sich auch klinische Beobachtungen halten und orientieren können. Es bedarf jedoch auch wesentlich der Einsicht, daß bei einem ständig wachsenden Gegenstand solcher Komplexität allgemeingültige Verallgemeinerungen nicht leicht zu erstellen sind und den Versuchungen einer voreiligen Täuschung widerstanden werden muß.
