    Notes: The effect of topical retinoic acid on the clinical and histological features and mitotic counts of lesions in 12 patients with psoriasis have been studied for a 2-week period.Serial biopsies were performed on lesions treated with retinoic acid in all 12 patients; control lesions treated with unmedicated ointment base were similarly biopsied in 7.There was a significant clinical improvement in 11 of the 12 patients treated with retinoic acid, and in all these patients it was greater than that achieved in the control lesions. Reformation of the granular layer, which was previously absent or only partially formed, occurred in 6 of the 12 psoriatic lesions treated with retinoic acid, but in none of the controls. Mitotic counts for the treated group fell after the 2-week period, but not in the controls.Topical retinoic acid would seem to be of therapeutic value in the treatment of psoriasis. The possible mechanisms of action are discussed.

    Notes: SUMMARY.— Mitotic counts in psoriatic epidermis removed from (a) the centre and edge of a single plaque in 17 patients, and (b) lesions at 3 different sites—the arm, leg and back—in 16 patients, have been compared.Mitotic counts at the centre and edge of a plaque were similar for an individual patient, although there was variation in the group as a whole. There was no significant difference between the average centre and edge counts. The counts from the arm, leg and back were similar for an individual patient despite variation between patients. There was no evidence of any consistent variation between the 3 sites.These results suggest that the factors influencing mitosis in psoriasis at any given time are similar for all lesions and each part of a lesion in an individual patient, although there are variations between patients.

    Notes: Summary.— Clinical and histological features and mitotic counts have been studied in the lesions of 11 patients with psoriasis who were treated with topically applied retinoic acid ointment for a period of 3 months.Serial biopsies were taken at monthly intervals from treated lesions in all 11 patients and from lesions treated with the unmedicated ointment base in 9.There was no significant difference between treated and placebo groups regarding any of the parameters studied. Irritant effects of the retinoic acid were prominent in 7 patients.The significance of these results is discussed in the light of the known mechanism of action of retinoic acid.

    Notes: SUMMARY.— Serial biopsies have been performed on psoriatic lesions from the forearm in 13 patients in whom the lesions were occluded with plastic occlusive dressings for 2 weeks, and in 13 control psoriatic subjects.A complete granular layer was not present in the occluded or control lesions prior td the study. In the occluded lesions a complete layer developed in 25 of the subsequent 65 biopsies but in only 2 of the 65 biopsies in the control group. In 10 of the 13 subjects the occluded lesions showed a complete granular layer at some stage during occlusion, although the time when it first appeared was variable. In 4 of these 10 patients the granular layer became incomplete after reformation during the period of study.Mitotic counts of the occluded psoriatic lesions showed a slight but significant fall at the end of the 2-week period but this did not occur in the control lesions. There was no significant depression of the mitotic count at the time when the granular layer first reformed.It is suggested that plastic occlusive dressings have some effect on psoriasis, and possible mechanisms of action are discussed.

    Notes: SUMMARY. —Mitotic counts and histological features have been studied in 35 psoriatic patients prior to treatment, and during treatment with topically applied fluocinolone acetonide, methotrexate, dithranol and coal tar.Prior to treatment there was considerable variation in the mitotic counts in the 35 biopsies. The granular layer was absent in 10 biopsy specimens, partially formed in small areas in 21, well formed in most areas in 2, and normal in 2. There was no correlation between the mitotic counts and state of the granular layer.With methotrexate and dithranol treatment the granular layer showed improvement prior to a significant fall in the mitotic counts. With fluocinolone acetonide the improvement in the granular layer appeared at the same time as a significant fall in the mean mitotic count, but the granular layer had completely reformed in 7 of the 9 patients while the mitotic count was still considerably raised compared to uninvolved psoriatic skin. With coal tar the results were not uniform; some patients showed a significant fall in mitotic counts prior to improvement in the granular layer and others first showed improvement in the granular layer.It is suggested that in clearing psoriasis these drugs have an action other than, or in addition to, inhibiting mitosis.

    Notes: A study of thirty-three patients with a clinical diagnosis of dermatitis herpetiformis (DH) referred to our DH clinic over the last 11 years is reported. Twenty-six were referred by other consultant dermatologists. The diagnosis had been made by the clinical features and response of the rash to dapsone. Seventeen patients were found to have IgA in the uninvolved skin (IgA positive) and in sixteen no IgA was found (IgA negative). The duration of the rash prior to referral to the DH clinic was 3 months to 19 years (mean 5.0 years) for the IgA negative patients and 2 months to 22 years (mean 5.2 years) for the IgA positive group. The length of follow-up was 3 months to 11 years (mean 5.0 years) for the IgA negative, and 2–11 years (mean 5.6 years) for the IgA positive group. During follow-up the rash cleared completely and required no treatment in seven of the sixteen IgA negative patients. Thirteen of these sixteen patients no longer required dapsone, but six patients were receiving alternative treatment. In the three patients still taking dapsone IgA has not been found on subsequent biopsy. Of the seventeen IgA positive patients only three were able to stop dapsone during follow-up and in these three the IgA was still detected in the skin. Small intestinal mucosa was abnormal in eight of eleven IgA positive patients, but was normal in all thirteen IgA negative patients in whom jejunal biopsies were performed.An alternative diagnosis to DH has subsequently been made in thirteen of the sixteen IgA negative patients. Although the significance of IgA in the skin in DH is not known it appears to be part of the disease process. Patients who have a rash suggestive of DH and which is dapsone responsive, but in whom IgA is not found in the uninvolved skin, usually turn out to have a dermatosis other than dermatitis herpetiformis.Referral to a unit with expertise in immunofluorescence techniques of skin biopsies would appear to be helpful.

    Notes: Nine out of thirty-four patients with linear IgA disease (LAD) had oral ulceration. Four out of seven of these patients who were examined by an ophthalmologist had changes of a cicatrising conjunctivitis indistinguishable from those of benign mucous membrane pemphigoid (BMMP). Three of these patients gave no history of ocular symptoms up to the time of examination. These findings indicate a need for oral and ophthalmological assessment in all patients with LAD.Twenty-seven patients with a diagnosis of BMMP were also studied. Nine presented with oral symptoms alone, nine with ocular symptoms alone, seven with oral and ocular symptoms, and two with cutaneous lesions in additon to oral and ocular symptoms. All the patients were examined by the same ophthalmologist. Six of the nine patients who presented with oral symptoms alone had signs of a cicatrising conjunctivitis. Four of these six patients had the clinical pattern of erosive gingivitis which was not previously thought to be associated with a cicatrising conjunctivitis. There is a similar need, therefore, for an ophthalmological assessment in all patients presenting with oral BMMP.Three of the twenty-seven patients with BMMP had homogeneous-linear deposits of IgA in uninvolved skin. The finding of linear IgA deposits in the skin of these patients with only mucous membrance lesions, and the finding that patients with LAD have a high incidence of oral and conjunctival lesions, raise the possibility of a common pathogenic pathway but with varying clinical expressions in these two groups.
