    Notes: Summary Reports of sinonasal non-Hodgkin's lymphomas, analysed with monoclonal antibodies, are scarce, and differentiation of these lymphomas from Wegener's granulomatosis can be difficult. In this study, we investigated histopathologically and immunohistologically 20 cases of non-Hodgkin's lymphoma, primary in the sinonasal region, and sinonasal biopsies from 11 patients with Wegener's granulomatosis. All T-cell lymphomas (n=7) and plasmacytomas (n=4) were stage I at clinical presentation, while all B-cell lymphomas (n=9) presented at higher stages. T-cell lymphomas tended to be more frequent in the nasal cavity and paranasal sinuses; B-cell lymphomas more often presented in the nasopharynx. Remarkably, 1 B-cell lymphoma expressed MT1, and 1 T-cell lymphoma expressed L26 (CD 20). The follow-up of 2 patients with a clinical diagnosis of Wegener's granulomatosis was suggestive of non-Hodgkin's lymphoma. Retrospective immunohistochemical analysis revealed that the original histological diagnosis of non-specific inflammation had to be changed to T-cell lymphoma, pleomorphic small cell type. We conclude that a biopsy from the sinonasal region with a dense inflammatory infiltrate, consisting predominantly of Tlymphocytes, renders a diagnosis of Wegener's granulomatosis unlikely and is at least suspicious of T-cell lymphoma. Immunohistochemical analysis is warranted for this type of biopsy.

    Notes: Summary Methods for the simultaneous detection of two virus types in cytological preparations or tissue sections by non-radioactive in situ hybridization were investigated. As a model system, CaSki cells, which have human papilloma virus type 16 (HPV 16) DNA integrated in their cellular genome, were in vitro infected with Herpes simplex virus 2 (HSV2). DNA probes for both viruses were labeled with biotin, acetylaminofluorene (AAF), and transaminated-sulfonated cytosine (TS-modified). Best results were obtained when a mixture of biotinated and haptenized DNA probes (AAF-or TS-modified) was used for hybridization. The biotinated hybrid was demonstrated with a streptavidinbiotinated alkaline phosphatase staining reaction, whereas the haptenized hybrid was visualized by an indirect peroxidase method. Visualisation of both viral DNAs in the same cell was possible by a combination of biotinated HPV 16 DNA and haptenized HSV2 DNA.

    Notes: Summary Enriched preparations of E, EA and EAC rosettes formed by human peripheral blood mononuclear cells were freeze-etched and examined electron-microscopically. In E rosettes only lymphocytes were involved, whereas in EA and EAC rosettes lymphocytes and mononuclear phagocytes participated as rosette-forming cells. In EA and EAC rosettes, cytoplasmic extensions of the rosette forming cell were seen to penetrate the sheep red blood cell, whereas E rosettes showed a broad zone of adherence without penetration. None of the three types of rosettes showed an interspace between the membranes. Unlike E rosettes, EA and EAC rosettes showed polarity in the adherence of sheep red blood cells. These observations made by freeze-etch electron microscopy indicate distinct morphological differences between rosettes formed with coated or uncoated erythrocytes.

    Notes: Summary A pulsed discharge helium ionization detector (PDHID) (patent pending) for gas chromatography has been developed. This detector uses a non-radioactive pulsed high voltage discharge source for generation of electrons and pulsed collection of these electrons. We have evaluated this detector for the analysis of a wide range of chemical compounds. In this paper the analytes are passed through the discharge since the permanent gases are difficult to ionize. The initial results for the permanent gases indicate that the PDHID can be used as a universal detector of contaminant traces at detection levels on the order of 1–20 pg. The response in this mode of operation is linear over four orders of magnitude.

    Notes: Summary Although palpation has proved to be an unreliable staging procedure, the indications for and the implications of more reliable radiologic staging methods for the neck in patients with a primary squamous cell carcinoma of the head and neck remain controversial. Only a very accurate imaging technique can replace neck dissection in clinical NO disease. This study compares the value of palpation with computed tomography (CT), magnetic resonance imaging (MRI) and ultrasound (US) with or without guided aspiration cytology for neck node staging. One hundred and thirty-two patients with squamous cell carcinoma of the head and neck were examined radiologically before undergoing a total of 180 neck dissections as part of their treatment. CT, US and MRI proved to be significantly more accurate than palpation for cervical lymph node staging. The accuracy of US-guided aspiration cytology was significantly better than of any other technique used in this study. Modern imaging techniques are essential for appropriate assessment of neck node metastases. In view of advances in the accuracy of contemporary imaging, the need for elective treatment of the neck requires reappraisal.

    Notes: Summary The incidence of micrometastases in cervical lymph nodes from squamous cell carcinomas of the head and neck was studied using routine histopathological examination. Micrometastases were found in 66 lymph nodes in 41 of the 92 tumor-positive neck dissection specimens. The detection of these micrometastases influenced postoperative treatment in 3 of the 77 patients with neck node metastases. The value of additional sectioning for detecting micrometastases was thus assessed. Sectioning at a deeper level in 600 originally histopathologically negative lymph nodes from 64 patients revealed 7 additional micrometastases in 5 patients. Antikeratin staining with a mixture of two monoclonal antibodies (AE1 and AE3) revealed 4 micrometastases in 739 originally histopathologically negative lymph nodes in 3 of 13 patients studied. Because of the unknown prognostic significance of micrometastases and the consequent arbitrary consequences for postoperative treatment, present findings show that the extra workload of immunostaining and deeper sectioning does not warrant their routine use in clinical practise.

    Description / Table of Contents: Summary Whereas most patients with benign lymphoepithelial lesion suffer from the involvement of a major salivary gland a number evolve into a clinical form of Sjögren's syndrome or Mikulicz's disease. In a small number development of malignant lymphomas, especially non-Hodgkin's lymphomas, have been described. Therefore, regular follow-up and appropriate histological evaluation of suspected areas in all patients with a benign lymphoepithelial lesion is indicated. Histologically, diagnosis of a non-Hodgkin's lymphoma may be difficult. Demonstration of a cell pattern, monoclonal for cytoplasmic Ig by means of the immunoperoxidase technique may facilitate the diagnosis. In this report we present the history of two cases out of our series with benign lymphoepithelial lesion who developed a non-Hodgkin's lymphoma.
    Notes: Zusammenfassung Während bei den meisten Patienten eine gutartige lymphoepitheliale Veränderung von einer der großen Speicheldrüsen ausgeht, kann ein Teil im Rahmen eines Sjögren-Syndroms oder einer Mikulicz-Erkrankung entstehen. In einigen Fällen wurde die Entwicklung eines malignen Lymphomes, besonders als Non-Hodgkin-Malignom beschrieben. Deshalb ist eine eingehende histologische Untersuchung jeder benignen lymphoepithelialen Veränderung zwingend. Dabei trägt die Darstellung der zytoplasmatischen Ig mittels der Immunoperoxidase-Technik zur Diagnosestellung bei. Bericht über zwei Fälle mit benignen lymphoepithelialen Veränderungen, die im Sinne eines Non-Hodgkin-Malignoms maligne entarteten.

    Notes: Summary This publication contains our initial development of a pulsed discharge emission detector (PDED) (patent pending). It uses a pulsed high voltage discharge in helium, which provides a stable source for atomic and polyatomic emission spectroscopy. We have evaluated this detector for both quantitative and qualitative analysis of a range of chemical compounds. Emission spectra observed from the pulsed high voltage discharge are valuable for spectral analysis. The results obtained for selected permanent gases in the ultraviolet, visible and infrared regions of the optical spectrum indicate that these spectra can be used for compound identification after a chromatographic separation. The data are unique in our opinion and serve as a basis for the future development and investigation of the analytical significance of this detection method.
