    Notes: Ileocolonoscopy and biopsies of patients with spondylarthropathy reveal gut inflammation in 62% of cases. In order to better understand the pathogenetic mechanisms of spondylarthropathy-related gut inflammation, the follicle-associated epithelium was examined. Biopsies from nine controls and 18 patients with spondylarthropathy were studied by electronmicroscopy. Membranous (M) cells were investigated in normal and inflamed ileum. In normal mucosa, M-cells were scarce whereas in inflamed mucosa their number was increased (up to 24% of follicle-associated epithelial cells). They showed a thin rim of cytoplasm covering groups of lymphocytes. In chronic ileitis, necrotic M-cells, rupture of M-cells and lymphocytes entering the gut lumen were observed. The bursting of M-cells at the top of the lymphoid follicles leads to interruption of the gut epithelial lining and gives the luminal content access to the lymphoid tissue. This pathogenetic mechanism may cause aphthoid ulcers.

    Notes: One of the advantages of flexible colonoscopy is that the terminal ileum can easily be reached and biopsied by an experienced operator. Thus, the pathologist will, with increasing frequency, receive ileal biopsies from patients who have or may not have ileal disease and it is therefore useful to have a good knowledge of the spectrum of ileal mucosal histology and pathology, of its normal function, and of the changes seen in different conditions. It is also desirable to be aware of diseases that occur less frequently.The ileal microscopic features are related to its absorptive function on the one hand, and to non-specific and specific defence mechanisms of the organism against potential hazardous components on the other. As a consequence the mucosa features a constant physiological and controlled inflammatory process. Part of the ileal mucosal structure is determined by the presence of the gut-associated lymphoid tissue (GALT) which plays a key role in discriminating harmless nutrients and harmful pathogens.It will be important to distinguish this normal situation from alterations seen in infectious or inflammatory pathology. Further, because of the therapeutic implications, it is necessary that acute and chronic (idiopathic) inflammation are recognized. This will in general be possible by evaluating the composition of the inflammatory infiltrate and the occurrence of epithelial and mucosal architectural changes.

    Notes: Abstract. The spondyloarthropathies (SpA) are a related group of disorders, characterized primarily by spondylitis, pauci-articular arthritis and enthesitis. The presence of subclinical gut inflammation in patients with SpA ranges from 25 to 75%, depending upon the type of SpA. Several data suggest that the association between gut inflammation and synovitis reflects an etiopathogenetic relationship, and that strategies which interfere with the gut inflammation may also modulate the synovitis. Here we review some standard as well as experimental drugs used in the treatment of patients with inflammatory bowel disease and discuss what is known about their effect on SpA-related locomotor manifestations. For the more experimental drugs, such as cytokines, anti-cytokines and anti-adhesion compounds, clinical trials in patients with SpA are still very scarce.

    Notes: Abstract Patients with active rheumatoid arthritis are characterized by a decrease in the number of circulating T-suppressor lymphocytes (identified by OKT8), resulting in an imbalance between helper (identified by OKT4) and suppressor cells. Piroxicam is a non-steroidal anti-inflammatory agent which modulates lymphocytic functions, especially by reducing the concentration of the rheumatoid factor in vitro and in vivo. A double-blind placebo-controlled study was performed in 20 patients suffering from active RA to investigate the acute effect of a single administration of piroxicam 40 mg on the number of circulating OKT3, T4, T8 and Ia1 positive cells. Blood samples were obtained 16 hours before and 0,2,6, 8, 24, 48, and 72 h after administration of piroxicam or placebo. There was a significant decreaseP〈 0.05) in T4/T8 cell ratio 48 and 72 h after piroxicam, whereas placebo had no effect. There were no significant changes in absolute numbers of total T-lymphocyte (OKT3 positive cells), T-helper-inducer (OKT4 positive cells) and T-suppressor cytotoxic lymphocytes (OKT8 positive cells). The number of IA1 positive cells (B-cells and activated T-lymphocytes) was significantly higher in the afternoon samples (at 14.00 and 16.00 hours) than in the morning samples (at 08.00 and 10.00 hours) after both placebo and piroxicam administrationP〈0.05). These data show that piroxicam decreases the T4/T8 cell ratio in active RA, but only 48 h after the first administration. These results contrast with those previously obtained after low doses of prednisolone(5- and 10mg) which induced a decrease inthe T4/T8 ratio as early as 6 h after intake, but which was no longer apparent after 24 h. Furthermore, these results demonstrate a diurnal rhythm for the number of IA1 positive lymphocytes.

    Notes: Abstract: We report a case of polyarteritis nodosa with a clinical presentation mimicking polymyalgia rheumatica, as well as pathological findings of non-giant-cell arteritis on temporal artery biopsy with symptoms of jaw claudication. Although certain clinical syndromes have been attributed to specific types of systemic vasculitis, considerable overlap occurs. Obtaining tissue biopsy in cases of vasculitis is mandatory for diagnosis and classification.

    Notes: Abstract: We report a case of arthritis due to Candida (Torulopsis) glabrata in two different joints at different times in the same patient. The first episode of arthritis was situated in the right ankle and lasted more than 1 year before the patient agreed to the proposed treatment. Therapy with intravenous amphotericin B and oral fluconazole failed. A cure was achieved with weekly intra-articular administration of amphotericin B, which was continued for more than 20 weeks and combined with oral itraconazole. Several weeks later the patient developed Candida glabrata arthritis of the left knee while still taking itraconazole. Immediately, intravenous amphotericin B therapy was started and was successful. Because there were no previous invasive point manipulations or trauma, the infections were considered to be haematogenously disseminated. Chronic corticosteroid and repeated antibiotic therapy for infectious exacerbations of chronic obstructive pulmonary disease and alcohol abuse are the presumed risk factors in this otherwise immunocompetent patient.

    Notes: Summary Sulphasalazine was administered to 48 patients with reactive synovitis (RS) and ankylosing spondylitis (AS) with peripheral arthritis, resistent to nonsteroidal antiinflammatory drugs. Thirty-seven patients underwent an ileocolonoscopy and in 33 patients signs of chronic or active inflammation of the ileum and/or ileocecal valve were found. Forty-two patients improved after 3 to 12 months of treatment; 50 per cent of them went into remission. In most of the patients improvement failed to occur in the first 2 months of treatment. There was no recurrent disease activity in patients responding to the treatment. Adverse reactions were rare and did not necessitate interruption of treatment. The beneficial effect of sulphasalazine in the treatment of RS and AS with peripheral arthritis needs to be confirmed in double-blind controlled studies.

    Notes: Summary In order to study gastrointestinal irritation caused by the intake of Benoxaprofen, blood loss in stools before and after intake of the drug was measured by labelling red blood cells with51Cr. The results were compared with previous studies in which the same method was used to determine fecal blood loss after intake of other non-steroidal anti-inflammatory drugs. Anti-inflammatory drugs inhibiting prostaglandin synthetase caused abnormal fecal blood loss in 20 to 60 % of the patients. Benoxaprofen which is a weak prostaglandin synthetase inhibitor and acts on the leukotrines, entailed no abnormal fecal blood loss. It was concluded that with respect to microbleeding, Benoxaprofen had no harmful effect on the gastrointestinal tract in this population.

    Notes: Summary A double-blind trial was carried out to weigh the effectiveness and tolerance of intraarticular injections of bufexamac (20 mg) against methylprednisolone (40 mg) in 40 patients with an acute bout of inflammatory arthritis. Leucocyte and polymorphonuclear counts and viscosity measurement of the synovial fluid were performed before and 14 days after injection, which showed methylprednisolone to be significantly superior to bufexamac in suppressing inflammation. In both groups a statistically significant improvement of pain on mobilisation and pain index was noted, but the number of clinical remissions was significantly higher in the methylprednisolone group. The blind investigator found a statistically significantly greater improvement with methylprednisolone than with bufexamac, and patients' opinion on effectiveness did not differ significantly. Important local or general side-effects were not observed. It was concluded that, although both drugs appeared to exert a considerable anti-inflammatory effect in inflammatory arthritis, methylprednisolone was more effective.
