    Notes: Abstract A new method is described to allow interactive selection of the reconstruction filter at the time of interpretation of images from single-photon tomography. In the filtered back projection algorithm, the only part of the reconstruction process requiring user interaction is the selection of the window function. Since the ramp and window filters have different purposes, they can be separated, placing the window at the end of the reconstruction process as a three-dimensional filter. All stages of reconstruction except the window filtering are performed before the physician begins to interpret the study. The three-dimensional filtering is performed very rapidly with use of the Chebyshev convolution algorithm. A 64×64×64 pixel cube of data is filtered in 13–33 s using filters of 3–11 lengths. Smaller volumes of image data can be filtered in less than 1 s; thus, the user can interactively choose any desired filter for a given tomographic study at the time of interpretation of the images.

    Notes: Abstract Exercise-induced increases in pulmonary uptake of thallium-201 (201Tl) have been associated with exercise-induced myocardial dysfunction. To evaluate this phenomenon more replicably, a quantitative semi-automated computer program was used to generate, from anterior exercise and delayed views, lung-myocardial ratios (LMR) of201Tl uptake in 78 patients [40 normal, 38 with coronary artery disease (CAD)]. Patients with CAD had a significantly higher mean exercise lung myocardial ratio (EXLMR) than normals (30.8 vs. 27.3;P 〈 0.003). In patients with adequate exercise (≥85% of an age-adjusted maximal heart rate), the EXLMRs of CAD patients were significantly higher than those of normals (29.7 vs. 25.5;P=0.003). However, this difference between CAD and normal patients was not apparent in a patient subgroup with submaximal exercise levels (〈 85% of an age-adjusted maximal heart rate). In both normal and CAD patients, EXLMR decreased with increasing exercise levels (r=-0.555;P=0.007). In patients with201Tl scans lacking visually defined perfusion defects (visually normal), an elevated LMR detected 60% of CAD cases with 81% specificity. A considerably elevated EXLMR in patients achieving adequate exercise should suggest the presence of CAD, even if there are no visually apparent cardiac perfusion defects. With submaximal exercise, however, the EXLMR is not a useful discriminator between CAD patients and normals.

The general combination algorithm, a particular implementation of this algorithm, and an interactive approach to combining functional images are described. Combined phase and amplitude images derived from gated blood-pool studies are presented to illustrate the method, and other applications of the technique in nuclear medicine are suggested.

    Notes: Abstract An image network that is in use in a large nuclear medicine department is described. This network was designed to efficiently handle a large volume of clinical data at reasonable cost. Small, limited function computers are attached to each scintillation camera for data acquisition. The images are transferred by cable network or floppy disc to a large, powerful central computer for processing and display. Cost is minimized by use of small acquisition computers not equipped with expensive video display systems or elaborate analysis software. Thus, financial expenditure can be concentrated in a powerful central computer providing a centralized data base, rapid processing, and an efficient environment for program development. Clinical work is greatly facilitated because the physicians can process and display all studies without leaving the main reading area.

    Notes: Abstract The use of the Wiener filter is proposed for temporal filtering of nuclear medicine dynamic studies. This filter adapts to the signal and noise levels of each pixel activity curve in a dynamic study to produce an “optimal” suppression of noise, while maintaining the signal content of the curve. The filter is derived to be a simple function of the power spectrum of the time-activity curve. Examples of its use for temporaly filtering gated blood-pool studies for cine viewing and functional image formation are shown.

    Notes: Abstract Sixty-five patients with suspected pulmonary embolism were studied prospectively with both Kr-81 m and Xe-133 ventilation imaging and Tc-99m MAA perfusion imaging. The krypton images, perfusion scintigrams and chest radiographs were read independently of the xenon images, perfusion scintigrams and chest radiographs by three observers. The studies of 53 patients were interpreted as normal or as indicative of a low or intermediate probability for pulmonary embolism with both gases. One study indicated intermediate probability with Xe-133 due to diffuse, severe xenon retention but low probability with Kr-81 m because of close ventilation-perfusion correspondence. The studies of 9 patients indicated a high probability of embolism with both gases, while those of two additional patients (one with emboli at angiography) indicated a high probability only with Kr-81m. While essential agreement between Xe-133 and Kr-81m ventilation imaging was found in most patients, the significant difference in interpretation in 2 of 11 patients with probable pulmonary embolism suggests that a controlled, prospective trial with pulmonary angiography is warranted before Kr-81m is employed for routine clinical use.

    Notes: Abstract The finite impulse response (FIR) digital filter is a spatial domain filter with a frequency domain representation. The theory of the FIR filter is presented and techniques are described for designing FIR filters with known frequency response characteristics. Rational design principles are emphasized based on characterization of the imaging system using the modulation transfer function and physical properties of the imaged objects. Bandpass, Wiener, and low-pass filters were designed and applied to 201T1 myocardial images. The bandpass filter eliminates low-frequency image components that represent background activity and high-frequency components due to noise. The Wiener, or minimum mean square error filter ‘sharpens’ the image while also reducing noise. The Wiener filter illustrates the power of the FIR technique to design filters with any desired frequency reponse. The lowpass filter, while of relative limited use, is presented to compare it with a popular elementary ‘smoothing’ filter.
