    Description / Table of Contents: Résumé Il est admis par presque tout le monde que l'omphalocèle et le gastroschisis sont deux entités différentes. Leurs étiologies, et leur pathogénies, cependant, restent controversées. Quel que soit l'agent étiologique ou les lésions qui sont à l'origine des ces malformations, le pronostic du gastroschisis est nettement meilleur que celui de l'omphalocèle. Ceci est dû principalement à l'incidence plus élevée de l'association d'autres malformations congéntiales avec ce dernier. La mortalité globale est entre 30 et 45%. Avec les techniques modernes d'échographie, le diagnostic prénatal peut être fait dès la 10è ou la 12è semaine de la gestation. La présence de viscères abdominaux à la base du cordon ombilical est un charactéristic diagnostique de l'omphalocèle. Cependant, même si des études n'ont pas démontré l'avantage de la césarienne sur l'accouchement par les voies naturelles, la plupart des obstétriciens semblent préférer la première solution dès lors que le diagnostic d'omphalocèle ou de gastroschisis est fait. Les détails de la technique chirurgicale dans chacune de ces entités sont données. Il faut y associer, en période postopératoire, une alimentation parentérale avec un excédent protéique.
    Abstract: Resumen Hoy es generalmente reconocido que el onfalocele y la gastrosquisis son dos entidades clínicas diferentes. Sinembargo, persiste controversia en cuanto a su etiología y patogénesis. En este artículo se revisan las hipótesis sobre etiología y patogenesis, se describen las técnicas de diagnóstico prenatal de éstas y en otras malformaciones congénitas que pueden estar asociadas. También se discuten las diferentes opciones en el tratamiento quirúrgico del onfalocele y de la gastrosquisis, así como el manejo postoperatorio, el cual incluye cuidado intensivo con soporte nutricional y ventilatorio. Los pacientes con gastrosquisis con frecuencia exhiben dismotilidad gastrointestinal, por lo cual requieren nutrición parenteral prolongada. La nutrición enteral en las etapas precoces del período postoperatorio puede dar lugar a episodios de enterocolitis necrotizante que demanda tratamiento médico agresivo y, en algunas ocasiones, intervención operatoria.
    Notes: Abstract It has been widely acknowledged that exomphalos and gastroschisis are two different clinical entities. Their etiology and pathogenesis, however, remain controversial. Several techniques are available for making a prenatal diagnosis of these as well as many other malformations. Some prenatal treatment is possible, but operative management is the more usual course. In most cases, of omphalocele and gastroschisis, treated either conservatively or by any kind of surgery, intensive care is mandatory to support nutrition and often ventilation as well. Enterai nutrition at an early stage during the postoperative period might lead to bouts of necrotizing enterocolitis requiring aggressive medical treatment and sometimes even operative treatment.

    Notes: Summary The nephrotoxic interation between cis-diamminedichloroplatinum (CDDP) and amikacin (AMI) was studied in rats. Following a single dose of CDDP (5 mg/kg i. v.), AMI (60 mg/kg s. c.) was given for 14 days. When given alone CDDP caused a 40% fall in the glomerular filtration rate (GFR), whereas AMI alone had no effect on GFR. This nonnephrotoxic course of AMI potentiated the CDDP-induced fall in GFR. Only a limited recovery of renal function was observed during a 15-week follow-up period.

    Notes: Abstract We report the results of a parental questionnaire concerning possible etiological and teratological factors, such as exposure to herbicides, in the development of congenital diaphragmatic hernia (CDH). The herbicide Nitrofen interferes with lung development in rats, can induce diaphragmatic hernia and greatly resembles thyroid hormone. No association with the studied teratogens nor with maternal thyroid dysfunction was found. The questionnaire was completed by 33 parents whose baby had CDH, and by 43 couples whose baby had oesophageal atresia. The resemblance of Nitrofen to thyroid hormone, a well-known growth factor for the developing lung, is of particular interest from a pathogenetic point of view in the development of CDH.

    Notes: Abstract Changes in the glomerular filtration rate (GFR) were studied in rats, 4 h to 4 weeks after unilateral nephrectomy (NX). The GFR was determined with a technique using51Cr-EDTA and a single timed blood sample. The GFR determined in this way corresponded with the GFR calculated by two compartment analysis and with the plasma levels of creatinine and urea. Increases in the GFR, compared with half the GFR of sham operated rats, were observed as early as 4 h after NX. This increase was entirely due to an increase in the GFR per gram of kidney, since no increase in kidney weight was observed at that time. After the initial increase, the GFR remained at that level during the first 48 h after NX. At 48 h a significant increase in kidney weight per 100 g body weight had taken place. The longterm changes in the GFR amounted to an increase of about 80% of that of sham operated rats after 3–4 weeks. After 4 weeks, the increase in the GFR of the remaining kidney was due to an increase in kidney weight of 35% as well as an increase in the GFR per gram of kidney of 20%. These data indicate that the increase in the GFR of the remaining kidney after unilateral NX occurs rapidly and is independent of an increase in kidney weight. Compensatory hypertrophy develops at a later stage and helps to maintain the increased function of the single remaining kidney.

    Notes: Abstract We compared the nephrotoxic interaction between cisplatin (CP) and amikacin (AM) in young and adult rats, using different dosage combinations. Following a single i.v. dose of CP, AM was administered s. c. for 14 days. The dose of CP was chosen to cause a 20%–50% fall in the glomerular filtration rate (GFR), while a dose of AM was chosen that had only a minimal effect on GFR. In adult rats, a decrease in GFR to 60% of the control value after CP alone was seriously aggravated by a non-toxic AM course given during 2 weeks after CP. In this combination, the GFR per 100 g body weight was reduced to 30% of control at week 2, which rose to 40% of control at week 15. In young rats, a non-toxic AM course did not aggravate the CP-induced impairment in GFR. However, when the dose of AM was increased to cause a 20% reduction in the GFR, the nephrotoxicity was potentiated. When measured at week 2, the GFR per 100 g body weight was 40% of control after the combined treatment compared with 80% of control after CP alone. As in adult rats, there was only a partial recovery of the GFR. In conclusion, in both adult and young rats, a course of AM following a single injection of CP potentiated CP-induced nephrotoxicity.
