    Notes: The active vitamin D metabolite, 1,25-dihydroxycholecal ciferol induces differentiation of monocytes into macrophages. The pharmacological induction of differentiation of primitive, rapidly proliferating cell lines into more mature cells with lower proliferative potential is a new dimension in the treatment of myeloproliferative disorders, which may prove to be an important alternative to more traditional regimens, Furthermore, the cell primarily engaged in bone resorption-the osteoclast-represents another differentiated form of mononuclear phagocytes, and 1,25-dihydroxycholecalciferol increases the number of osteoclasts. Since the cellular action of 1,25-dihydroxycholecalciferol is exerted mainly through its binding to nuclear receptors, a detailed knowledge of ligand-receptor interactions is mandatory for future work in this area. In order to investigate the interaction between 1,25-dihydroxycholecalciferol and its receptor in mononuclear cells, the nuclear uptake of the hormone was studied using a whole cell assay. The nuclear uptake of l,25-dihydroxy[3H]cholecalciferol in human monocytes at physiological temperature and pH was saturable, specific, and fully reversible. When eight normal individuals were investigated, the maximal binding capacity (Bmax) was 0.4–8.4 fmol/106 cells and the dissociation constants (Kd) were 0.12–0.45 nmol/l. The characterization of the nuclear uptake of 1,25-dihydroxy [3H]cholecalciferol in intact human monocytes shows that it is mediated by binding of the ligand to a specific nuclear receptor. The binding to the nuclear receptor is the result of the passage of ligand across the cytoplasmic membrane and of the cytoplasmic transport of ligand. In contrast to conventional receptor assays in hypertonic cellular extracts, this system provides information on the role of the cytoplasmic membrane in relation to the nuclear uptake of 1,25-dihydroxycholecalciferol, which may be closer to in vivo cellular conditions.

    Notes: Abstract Molybdenum single crystals are irradiated at 20 K with 6 MeV protons. The radiation damage and lattice defect annealing is studied by positron lifetime spectroscopy in the temperature range from 15 to 720 K. Loss of vacancies due to recombination with mobile interstitials is observed at 40 K (Stage I) in agreement with resistivity measurements. This is the first time Stage I is observed by positrons below 77 K. The implanted hydrogen decorates the vacancies around 100 K, which is consistent with a hydrogen migration energy in molybdenum:E M H = 0.3–0.4 eV. Clustering of spatially correlated vacancies takes place in a wide temperature region below the usual vacancy clustering stage (Stage III). Stage III is observed at rather low temperatures (400–480 K) due to the very high vacancy concentration. Hydrogen bound to vacancies and vacancy clusters is released above 540 K, which puts an upper limit to the hydrogen binding energy:E B H ≦1.4 eV. The present work emphasizes the advantage of employing a vacancy sensitive technique to study hydrogen in metals, where its intrinsic solubility is low. In such metals (as molybdenum) both the effective solubility and the effective mobility of hydrogen are strongly influenced by the presence of vacancies.

    Notes: Abstract The fast irradiation facility Mach-1 installed at the Danish DR 3 reactor has been used in boron determinations by means of Instrumental Neutron Activation Analysis using12B with 20-ms half-life. The performance characteristics of the system are presented and boron determinations of NBS standard reference materials as well as fertilizer materials are compared by literature value and spectrophotometric measurements, respectively. In both cases good agreement is obtained.

    Notes: Summary Pigmented villonodular synovitis is a benign disease of the synovial membrane of joints, tendon sheaths, or bursae, which nevertheless can cause marked local destruction. Its diagnosis is often delayed because complaints and symptoms are nonspecific. Familiarity with the disease may ensure an earlier diagnosis and consequently early onset of therapy, which may prevent serious damage. This paper describes 18 patients suffering from localized or diffuse pigmented villonodular synovitis. Findings possibly suggestive of pigmented villonodular synovitis include hemarthrosis, soft tissue swelling, radiological evidence of cyst formation at a distance from the weight-bearing area of a joint, an increased triglyceride concentration, and a positive bone scan. A normal appearance on arthroscopy does not rule out the disease. Therapeutic results are better in the localized than in the diffuse form of the disease.

    Notes: Summary Between 1978 and 1983, 14 patients suffering from rheumatoid arthritis were treated by bilateral total hip and knee replacement. At the time of this follow-up study (32–92 months postoperatively) one patient had died of concomitant disease and was excluded. The postoperative results regarding pain relief, improvement in performing daily activities, and function were studied in the remaining 13 patients. A 200-point rating scale was used, which compared the pre- and postoperative findings at follow-up. All patients showed marked relief from pain, good function and a good functional score. Although walking ability did not always improve, all patients were enthusiastic.

    Description / Table of Contents: Zusammenfassung Es wird die seltene Komplikation der Dislokation einer Knieprothese beschrieben, die ohne Fraktur einhergeht and nicht durch ein Trauma verursacht ist. Es handelt sich um einen Patienten mit einem Osteosarkom des Femur, das Veranlassung für den künstlichen Gelenkersatz war. Die 2 Jahre nach der Operation eingetretene Luxation wird in diesem Falle als Folge einer überdurch-schnittlichen Beweglichkeit der Prothese angesehen. Die Analyse des Einzelfalles führte zu einer Veränderung der Prothese, die eine Luxation vermeiden soll.
    Notes: Summary A rare complication, namely dislocation of a knee component without fracture of the prosthesis and not caused by a trauma, is described in a patient who was treated for osteosarcoma of the femur. Probably the dislocation, which occurred 2 years postoperatively, was caused by an extraordinary function of this prosthesis. A partial revision of the prosthesis was performed. The newly designed knee component, which prevents dislocation, is described.

Ein Patient hatte eine große Zyste des lateralen Meniskus, während bei dem anderen ein Synovial-Sarkom vorlag. Ätiologie, Symptomatologie, Untersuchungsmethoden, Therapie, Prognose und DifferentialDiagnose werden diskutiert. Es wird unterstrichen, daß eine Meniskuszyste nicht angemessen behandelt ist, solange der Meniskus nicht vollständig entfernt wurde. Die rätselhaften Befunde und Symptome eines Synovial-Sarkoms des Kniegelenkes werden betont. Bei ungewohnlichen Schwellungen des Kniegelenkes ist es unverzichtbar, eine Biopsie vorzunehmen, um einen bösartigen Prozeß, insbesondere das heimtuckische Synovial-Sarkom, auszuschließen.
    Notes: Summary The authors describe two patients with knee symptoms. In both cases the correct diagnosis was established late. One patient had a large cyst of the lateral meniscus, while the other had a synovial sarcoma. Aetiology, symptomatology, methods of investigation, therapy and prognosis are discussed, as well as the differential diagnosis. It is stressed, that a meniscal cyst is not adequately treated until the entire meniscus is removed. The puzzling signs and symptoms of a synovial sarcoma of the knee are mentioned. It is obligatory to perform a biopsy in all patients with an unfamiliar swelling of the knee, in order to rule out a malignancy, especially the treacherous synovial sarcoma.

    Description / Table of Contents: Zusammenfassung Anomalien des proximalen tibiofibularen Gelenkes werden selten beobachtet. Eine Instabilität tritt meist im Gefolge einer Verletzung auf. Vier Typen der Instabilität werden unterschieden: Subluxation, anterolaterale, posteromediale und craniale Dislokation. Vier röntgenologische Methoden, die für die Darstellung der Anomalien des proximalen tibio-fibularen Gelenkes entwickelt wurden, werden erörtert. Eine besondere Aufmerksamkeit wird der klinischen Bedeutung einer neuen Methode gewidmet. Eine Instabilität wurde bei 19 Patienten gezeigt, von denen 16 operativ behandelt wurden. Die Vorgeschichte, die Untersuchungsbefunde und die Behandlungsmaßnahmen werden diskutiert.
    Notes: Summary Abnormalities of the proximal tibiofibular joint are infrequently encountered. Mostly instability occurs as a result of trauma. Four types of instability are distinguished: subluxation, anterolateral, posteromedial and superior dislocation. Four radiological methods designed to visualize abnormalities of the proximal tibiofibular joint are discussed. Special notice is given to the clinical relevance of a new method. Instability was demonstrated in 19 patients; 16 of these were treated by an operation. History, data obtained by physical examination and ways of treatment are discussed.
