Zusammenfassung Ein eigenes Krankengut von 142 Brückenwinkeltumoren aus einer neurochirurgischen Gesamtserie von 2600 Hirntumoren und anderen raumbeengenden Prozessen wurde unter klinischen Gesichtspunkten analysiert. Folgende 4 große Gruppen wurden gesondert betrachtet: Akustikustumoren, Felsenbeinmeningiome, Epidermoide, Brückentumoren. Obwohl der Brückenwinkel vom neurologischen Blickpunkt aus zu den besonders symptomreichen Gebieten des Gehirns gehört, kann die Differentialdiagnose zwischen den verschiedenen Tumorarten schwierig sein. Als wesentliche Daten müssen berücksichtigt werden: Lebensalter, Häufigkeit der Erstsymptome, Entwicklung und Dauer der Vorgeschichte, verschiedene klinische Syndrome usw. Die Bedeutung einiger zusätzlicher radiologischer und Liquor-Untersuchungen wird kurz besprochen. Es ist schließlich außerordentlich wichtig zu betonen, daß ein großer Brückenwinkeltumor auch mit nur geringer lokaler Symptomatologie einhergehen kann. Einige atypische Akustikustumoren können ein Spätstadium mit großem Hydrocephalus und psychischen Störungen erreichen, bei Fehlen von Lokalsymptomen, allenfalls verbunden mit einer gelegentlich übersehenen Schwerhörigkeit. Andere verursachen lediglich eine Trigeminusneuralgie, ohne andere Brückenwinkelsymptome, beispielsweise einige Epidermoide und einzelne Meningiome. Von klinischer Bedeutung ist auch, daß manche intrapontin wachsenden Gliome ein voll ausgeprägtes Brückenwinkelsyndrom mit gelegentlich nur langsamer Progredienz hervorzurufen vermögen. Alle diese verschiedenen Tatsachen unterstreichen das Interesse von Neurologen und Neurochirurgen an der Kenntnis der typischen und atypischen Syndrome der Brückenwinkeltumoren. In dieser Region ist eine sehr sorgfältige Wertung der klinischen Symptome notwendig, um im Einzelfall zu einer zutreffenden Beurteilung zu kommen. Der hohe Anteil gutartiger Tumoren im Brückenwinkel erhöht die praktische Bedeutung der Frühdiagnose.

Résumé Un ensemble de 142 tumeurs de l'angle cérébellopontin, tiré d'une série totale neurochirurgicale de 2600 tumeurs du cerveau et d'autres vastes lésions, a été analysé du point de vue clinique et en prenant en considération les quatre principaux groupes: — F-tumeurs acoustiques, méningiomes, cholestéatomes et gliomes intrapontins. Bien que l'angle cérébellopontin soit l'une des zones les plus expressives du cerveau du point de vue neurologique, le diagnostic différentiel peut être très difficile entre les différents types de tumeurs. Les principaux aspects s'accentuent en prenant en considération l'âge, la fréquence des premiers symptômes, l'évolution et la longueur des faits cliniques, les différents syndromes neurologiques etc. La valeur de certains examens complémentaires radiographiques et du liquide cérebrospinal est brièvement discutée. Enfin il est extrêmement important de souligner qu'une vaste tumeur dans l'angle peut se présenter avec une symptomatologie focale plutôt légère. Certaines tumeurs acoustiques atypiques peuvent atteindre un stade avancé avec grande détérioration hydrocéphale et mentale avec une absence de symptômes focaux visible ou une certaine surdité qui était quelque peu négligée. D'autres peuvent présenter seulement une névralgie du trijumeau sans autres symptômes ou signes de l'angle évidents comme certains épidermoïdes ou quelques méningiomes. Un autre fait clinique intéressant est que certains gliomes se développent à l'intérieur de la protubérance et peuvent se manifester par des syndromes d'angle à très lente évolution. Tous ces faits différents montrent le grand intérêt neurologique et neurochirurgical que présentent les tumeurs de types différents occasionnant des syndromes typiques et atypiques de l'angle cérébellopontin, A ce point de vue une appréciation clinique convenable des cas individuels est nécessaire.

Summary A personal series of 142 tumours of the cerebellopontine angle, out of a total neurosurgical series of 2600 brain tumours and other expansive lesions, has been analysed from the clinical standpoint concentrating upon the four main groups: accoustic tumours, petrous bone meningiomas, epidermoids and intrapontine gliomas. Although the cerebellopontine angle is one of the more expressive areas of the brain from a neurological point of view, differentiation between the various types of tumours may be very difficult. The main features are stressed taking into consideration age, frequency of first symptoms, evolution and length of clinical histories, different neurological syndromes, etc. The value of some complementary radiographical and cerebro-spinal fluid studies is briefly discussed. Finally, it is extremely important to emphasize that a large tumour in the angle may be present with only slight focal symptomatology. Some atypical acoustic tumours may reach a late stage with advanced hydrocephalus and mental deterioration and with an absence of obvious focal signs, or with no more than some deafness that had been overlooked. Other tumours may cause trigeminal neuralgia without other obvious angle symptoms or signs; this occurred with some epidermoids and with a few meningiomas. Another interesting clinical feature is that some gliomas growing within the pons may give rise to well developed angle syndromes that may occasionally have a very slow evolution. All these different facts show the great neurological and neurosurgical interest occasioned by the tumours of different types which produce typical or atypical syndromes of the cerebellopontine angle. In this region a very sound clinical judgement for the proper evaluation of the individual case is necessary. The high proportion of benign tumours in the angle increases the practical importance of an early diagnosis.
