    Notes: Zusammenfassung Bericht über die Ergebnisse von Untersuchungen, die dem Nachweis von fetalem Hämoglobin im retroplacentaren Hämatom dienten. Dabei fand sich in 29 von 93 Fällen ein HbF-Anteil, der mehr als 5% des Gesamthämoglobins betrug, während in den gleichen Fällen der Gehalt an fetalem roten Blutfarbstoff im mütterlichen Venenblut erheblich unter den ermittelten Werten lag. Diese Feststellung läßt den Schluß zu, daß es sich um einen diaplacentar erfolgten kindlichen Blutverlust handeln muß, dessen Bedeutung für die Entstehung eines posthämorrhagischen Schocks und den Sensibilisierungsmechanismus bei dem Morbus haemolyticus neonatorum anschließend diskutiert wird.

    Description / Table of Contents: Summary The alarming rise in the number and the levels of medical liability claims and the fact that the number of legal actions brought against doctors of medicine remains high demand countermeasures, and doctors must see to it that their responses do not stop with new insurance contracts at higher premiums or repeated complaints about the “unworldliness” and/or “unreasonableness” of jurists. Rather, doctors and hospitals must seek out the reasons behind the claims and work to prevent them, or in other words address judicial weak spots in such sectors as organization, documentation, patient information, and safety of appliances, which are subject to strict standardization, and look for solutions instead of waiting until the latent source of error has precipiated concrete harm to a partient or patients. This concept of risk analysis and risk prevention is called risk management and is presented in more detail in this article.
    Notes: Zusammenfassung Der besorgniserregende quantitative und qualitative Anstieg der Arzthaftpflichtansprüche und die nach wie vor erhebliche Zahl von Strafverfahren gegen Ärzte erfordern Gegenmaßnahmen, die von Seiten der Ärzteschaft nicht nur im Abschluß neuer Versicherungsverträge mit höheren Prämien oder Klagen über die „Weltfremdheit“ oder „Unvernunft“ der Juristen bestehen darf. Krankenhäuser und Ärzte müssen vielmehr den Ursachen der Schäden auf den Grund gehen und Schadensprävention betreiben, anders formuliert, juristische Schwachstellen in den stark normativ geprägten Bereichen Organisation, Dokumentation, Aufklärung, Gerätesicherheit u. a. aktiv angehen und nach Lösungen suchen, anstatt abzuwarten, bis die latente Fehlerquelle sich in einem konkreten Patientenschaden niedergeschlagen hat. Dieses Konzept der Risikoanalyse und Risikoprävention heißt „Risk management“ und wird in den folgenden Ausführungen näher dargestellt.

    Notes: Abstract This paper summarizes the results of four statistical approaches for the estimation of long-term trends (1983–92) in sulfate concentration data from 90 monitoring sites across the United States. Least squares regression models and nonparametric techniques were applied to these data. Sulfate concentrations were found to be generally decreasing on the order of 0–4% at most sites. There was general agreement that trends were significant in the Great Lakes, Pacific northwest, and southwest regions. Although strengths and weaknesses are described for each approach, all of these approaches are useful for long-term trend estimation. Visualization techniques are recommended for displaying trend patterns and associated levels of statistical significance.
