    Notes: Abstract We investigated the XLRS1 gene in Japanese patients with retinoschisis (RS). All exons of the XLRS1 gene were sequenced in 14 males, including a pair of monozygotic twins, from 11 individual families with RS and five of their mothers who are asymptomatic but diagnosed as carriers. Six kinds of missense mutations and a nonsense mutation, including six novel mutations, were detected in all 14 patients and carriers. Mutations in the XLRS1 gene are also responsible for RS in non-Caucasian patients. Most Japanese RS cases are caused by an XLRS1 gene defect. A novel mutation, Glu72Lys, was found in four families, suggesting a common mutation in the Japanese population. Clinical features of RS patients with both the Glu72Lys and Pro193Leu mutations indicate that a genotype–phenotype correlation is not recognized in RS.

    Notes: Abstract Report on a 63-year-old man with an abdominal aortic aneurysm (AAA) complicated with a left-sided inferior vena cava (It-IVC). A computerized tomographic scan showed that the It-IVC crossed anteriorly over the AAA body from the left to the right side. A superimposed digital subtraction angiogram of the AAA and the It-IVC disclosed a topographic relationship between the two, and also provided useful preoperative imaging data. By dissecting the It-IVC through a transection of several lumbar veins and cranial traction of a transverse portion of the It-IVC, safe proximal aortic control was achieved. The aneurysm was successfully replaced with a bifurcated woven Dacron graft. The development of the It-IVC anomaly and methods for exposing the neck of the AAA when this anomaly is present are also discussed.

    Notes: Abstract The natural history of intermittent claudication (IC) was surveyed in 59 limbs of 44 patients. The mean follow-up period was 3 years (1 to 8.3 years). During the follow-up period, 72.0 per cent of the limbs with a solitary arterial lesion improved or remained unchanged. However, detailed analysis showed that 53.3 per cent of limbs with iliac artery stenosis and 50.0 per cent of those with femoropopliteal artery stenosis worsened, whereas only 18.7 per cent of limbs with iliac artery occlusion and no limbs with femoropopliteal artery occlusion had worsening of symptoms. Of limbs with multiple arterial lesions, 62.5 per cent worsened. Although the natural history of IC is relatively favorable, IC due to a solitary arterial stenosis tends to worsen, whereas IC due to a solitary arterial occlusion will, in more than 80 per cent of cases, improve or remain unchanged. In cases of arterial stenosis, therefore, more careful follow-up, including control of risk factors, is mandatory.

    Notes: Abstract Vibrio vulnificus, a recently described strain of the halophilicVibrio species, was isolated from the blood of a 73-year-old man, who developed rapidly progressive wound infection and fatal septicemia. Twenty-one patients withVibrio vulnificus infection have been reported in the Japanese literature.Vibrio vulnificus most frequently causes primary septicemia and necrotising cellulitis after the eating of raw fish or shellfish or after exposure to seawater. The infection is characterized by its occurring during the warm months of the year, in patients with underlying diseases, especially liver diseases, and the mortality rate is surprisingly high. Clinicians should therefore consider the possibility ofVibrio vulnificus infection in the differential diagnosis of severe wound infections. Early surgical intervention and intensive antibiotic therapy are recommended for preventing the progress of the septicemia.

    Notes: Abstract We conducted a midterm follow-up of 150 claudicants who underwent surgical reconstruction by assessing cumulative patency, survival, and palliation (graft patency in live patients) rates. Eighty-nine claudicants (group I) underwent direct (in situ) proximal revascularization, 33 (group II) had indirect (ex situ) proximal revascularization, while 28 (group III) had distal revascularization. The secondary patency rates at 3 years were 97.5% in group I, 97.0% in group II, and 75.0% in group III, respectively. Only one patient with limb graft thrombosis required below-knee amputation. There were 3 perioperative deaths (2 in group I and 1 in group II). The survival rates at 3 years were 86.0% in group I, 69.5% in group II, and 95.8% in group III, respectively. The palliation rates at 3 years were 84.8% in group I, 70.0% in group II, and 77.9% in group III, respectively. These findings indicate the midterm benefits of supra- and infrainguinal arterial reconstructions, and also suggest that the preoperative assessment of risks in individual patients, the selection of the appropriate operative procedure and graft material, and intensive postoperative follow-up and management of any associated disease are all important aspects in the treatment of claudicants.

    Notes: Abstract The dynamic viscoelasticity of polymeric composites consisting of silicone gel and polymethacrylic acid cobalt(II) salt (PMACo) particles was studied in d.c. electric fields. It was measured by applying sinusoidally varying shear strain or compressive strain to the composites. It was found that the electric fields enhanced the storage and loss moduli of the composites, and changed the loss tangent (electroviscoelastic effect). The amount of the electroviscoelastic effect was influenced by the content of absorbed water in the PMACo particle, the fraction of PMACo particles, and the intensity of the electric field. It also depended on the amplitude and frequency of the applied strain. The increments of the compressive moduli due to the electroviscoelastic effect were much larger than those of the shear moduli.

    Notes: Dual-in-line packages (DIPs) were formed from epoxy molding compounds with various physical properties using a transfer molding machine. The compounds were prepared by changing kinds and amounts of additives and addition methods. The thermal shock test was carried out by the following procedures. The plastic package was soaked alternately in liquid nitrogen (-196°C) and in liquid solder (250°C) in the cycle of 140 s. The packages were intermittently subjected to optical microscopic inspection for crack initiation. The median life to crack initiation is defined to be the number of the cycles when half of the specimens exhibited crack initiation. Glass transition temperature, coefficient of linear expansion, and flexural modulus were measured to calculate thermal stress in plastic packages. According to the linear fracture mechanics, the following expression was obtained among the median life N, thermal stress σt and strength σb. N =/σ b2 We found the linear relation between logarithm of Nσ b2 and logarithm of σb/σ t for various packages, and obtained that the value of C and m are estimated as 3 × 104 MPa2 and 5.5, respectively. Therefore, the median life can be predicted from the glass transition temperature, coefficient of linear expansion, flexural modulus, and strength of plastic materials for packages.

    Notes: Originally incompatible blends of SAN 30% and PC 70% were extruded with extremely high shear rate up to 107 s-1 as a typical example of the blends. The materials were examined with a scanning electron microscope (SEM), a pulsed NMR, etc. The molecular weight of the blends was also measured with gel permeation chromatography. The blends are of binary systems microscopically in the first run of extruding, in which the minor constituent is present as small spherical particles in the major constituent. The apparent volume fraction of the spherical minor constituent estimated from the microscopic photographs decreases with the shear rate. The fraction is decreased also with the repeated runs. SEM observation reveals that dimple fracture of microsize takes place on SAN sphere dispersed in PC matrix. And at the bottom of the dimple, a small particle, which would be composed of PC, is present. From these, SAN in the blend is thought to be partly ductile even at the temperature of liquid nitrogen. At the fifth run, the blend appears uniform or structureless. Dynamic loss tangent gives two peaks corresponding to that of SAN and that possibly attributed to PC. The latter shifts to lower temperatures with the number of extruding run. These show that some of SAN is mixed with PC in a compatible form. The pulsed NMR analysis supports the conclusion. Furthermore, the analysis suggests that some of SAN is mixed in PC. This result shows the compatibility of SAN with PC is enhanced in extremely high shear rate processing.

    Notes: Extremely high shear rate processing was applied to the compound system of acrylonitrilestyrene copolymer/polycarbonate (SAN/PC). The viscosity was measured against the shear rate up to 107 s-1.The first non-Newtonian region, the second Newtonian region and second non-Newtonian region were observed in the compound system. The occurrence of these regions are very similar to those in the parent polymers, SAN and PC. For the calculation of the viscosity-shear rate curve, a concentric multilayer model was proposed. It gives good agreement between the calculated value and the measured one. A new mechanism for the occurrence of the non-Newtonian, second Newtonian, and second non-Newtonian regions was proposed. Nuclear spin-spin relaxation time measured on SAN, T2, seems to be consistent with the consideration that the occurrence of non-Newtonian region, second Newtonian region, and second non-Newtonian region are caused, respectively, by the disentanglement, near saturation of disentanglement associated with snapping of macromolecules, and reentanglement through recoiling of snapped macromolecules, and further snapping of the macromolecules, which is inconsistent with the proposed explanation.
