    Notes: Simplified Japanese Self-Perception Scale for Young Adults (SJS-PSYA) to identify risk cases Aim. The purpose of this study was to investigate the effectiveness of the newly devised Simplified Japanese Self-Perception Scale for Young Adults (SJS-PSYA) for identifying risk cases. The SJS-PSYA was used to measure self-perception of young adults and its validity was examined. The subjects were 196 Japanese young adults (91 males, 105 females) who had participated in the longitudinal study of Ueda (1996, 1998) from infancy and preschool years to adolescence. Method. Questionnaires for subjects and their parents were mailed separately and then interviews were conducted for possible risk subjects and their parents. Results. (1) The total score of the SJS-PSYA was correlated negatively with the total number of risk variables. (2) Eight out of 10 single risk variables were correlated with the total self-perception score. Better subjective health, higher aspiration for future, employed at present, much satisfaction in life, much support from friends as well as family and higher appraisal of nurturance of own parents, higher was the total self-perception score. These results suggest that the SJS-PSYA could be used in identifying young adults at risk and for intervention.

    Notes: By questionnaire survey, 32 out of 52 workers (61.5%) reported previous or current skin diseases from okra cultivation. The sites of skin lesions were mainly the- arms, lingers and fingertips. Positive patch test millions with preparations of okra leaves or immature pods in 111 workers, compared to 63 control subjects, wore significantly higher in okra workers than in controls (p〈0.01), ranging from 9.8 to 30.0%, 37 out of 111 workers (33.3%) I were diagnosed as having allergic contact dermatitis (n= 17; 15.3%) and irritant contact dermatitis (n= 18; 16.2%) from okra cultivation.

    Notes: Abstract We report herein a case of successfully treated advanced, nonfunctioning islet cell carcinoma associated with left-sided portal hypertension. The splenic vein was obstructed by a huge intravenous tumor thrombus developing from the main pancreatic tumor. Direct invasion to adjacent organs such as the spleen, colon, left kidney and stomach was also observed, although liver metastasis was not present. Radical resection was carried out with removal of these five involved organs and the patient is alive without recurrence more than 5 years after surgery.

    Notes: Abstract In this study, we analyzed 149 surgical cases of colorectal cancer between January 1983 and August 1989. Thirteen cases (8.7 per cent) of colorectal primary cancer associated with extracolonic primary malignancy of 14 lesions and 10 cases (6.7 per cent) of multiple primary colorectal cancers were included. Among the 14 lesions of extracolonic primary malignancy, there were 6 gastric carcinomas, 2 endometrial carcinomas, 2 urinary bladder carcinomas, and one each in the esophagus, liver, bile duct and jejunum. The second tumor was not detected preoperatively in 3 of 4 cases of synchronous multiple primary colorectal carcinoma. A curative resection was done in 10 (77 per cent) out of 13 cases of colorectal cancer associated with extracolonic malignancy, while 7 (88 per cent) out of 8 cases of multiple colorectal cancers had a curative resection. Nine patients (69 per cent) with colorectal cancer associated with extracolonic malignancy were disease-free for 2 months to 14 years. Seven patients (88 per cent) with multiple colorectal cancers were disease-free for one to 22 years. We recommend, therefore, that in any patient with colorectal cancer, the entire large bowel should be thoroughly searched for any other primary tumors, by taking the existence of extracolonic tumors into account. A curative resection should be performed, and the follow-up period should be life-long.

While the preoperative serum bilirubin levels is one of the criteria in determining the indication for radical hepatic resection, it does not play a decisive role in the case of combined Dubin-Johnson syndrome.

    Notes: Background. Increased numbers of mast cells are found in various solid tumors. To investigate the role of mast cells in the vicinity of gastric cancer cells, we used special staining and an immunohistochemical technique. Methods. Specimens were surgically obtained from 102 patients with gastric cancer. Mast cells around the tumor edge of gastric cancer nests were counted by staining with 0.05% toluidine blue solution. Blood vessels in these areas were also counted, by immunohistochemical staining of endothelial cells for factor VIII. Results. The average number of mast cells and blood vessels in gastric cancer specimens was significantly higher than that in normal gastric tissue. Specimens from patients with advanced disease with metastases to lymph nodes had more mast cells than specimens from patients with early-stage disease. Mast cells in specimens from patients with metastatic lymph nodes were significantly increased in comparison with numbers in specimens from those without nodal metastases. Mast cell numbers in the specimens of patients with lymphatic or blood vessel invasion were significantly higher than numbers in specimens from patients without such invasion. Mast cells were localized near the new vessels around gastric cancer cells. Mast cell numbers increased as the number of blood vessels increased (correlation coefficient, 0.783). Postoperative survival curves revealed that patients with increased numbers of mast cells had a poor prognosis. Conclusions. All these results suggest that mast cell accumulation at the tumor site may lead to increased rates of tumor vascularization and, consequently, increased rates of tumor growth and metastasis.

    Notes: Abstract Histopathology of cirrhosis was compared to the hepatic functional reserve and to the prognosis. One hundred and thirteen patients including 51 surgically treated for hepatocellular carcinoma (HCC) and 62 subjected to wedge biopsy of the liver during surgery for esophageal varices were studied. The type of cirrhosis associated with hepatocellular carcinoma was classified into 4 groups according to the degree of inflammation and the piecemeal necrosis. Of the 16 without cirrhosis, 13 (81 per cent) are living. Nine had an inactive cirrhosis and 5 (56 per cent) are living. Ten had a slightly active cirrhosis and 4 (40 per cent) are living. Sixteen had a fairly active cirrhosis and 7 (44 per cent) are alive. Immediate postoperative death due to acute hepatic failure occurred in 6, 4 of whom had a fairly active cirrhosis. In patients with active alcoholic hepatitis with numerous Mallory bodies and ballooning degeneration of liver cells, the prognosis was the poorest, if the liver was resected, even though the functional reserve seemed to be adequate. To prevent acute hepatic failure, liver histology during the surgery is predictive and wedge biopsy of the liver is recommended.

    Description / Table of Contents: Résumé L'embolisation artérielle associée avec la perfusion artérielle d'un agent chimique a été employée chez 58 malades qui présentaient des tumeurs hépatiques impossible à réséquer. La survie actuarielle à un an a ét de 56%. Chez 10 des malades ainsi traités il fut possible de pratiquer ultérieurement une résection hépatique. Ils sont tous en bonne santé. Chez 5 de ces 10 opérés on a pu constater de 1 à 11 mois après l'intervention que la tumeur était totalement nécrosée. Il s'agissait de tumeurs solitaires, de diamètre inférieur à 4 cm, encapsulées, situées profondément dans le foie et vascularisée par une seule artère. Chez les 5 autres malades des cellules néoplasiques persistaient au sein de la tumeur et la lésion principale s'accompagnaient de nodules néoplasiques satellites. L'embolisation artérielle associée avec la perfusion artérielle d'un agent chimique se trouve ainsi justifiée. Elle s'applique à certains cas de tumeurs malignes du foie bien sélectionnées.
    Notes: Abstract Transcatheter arterial embolization (TAE) combined with transcatheter arterial infusion (TAI) was done in 58 patients with unresectable hepatic malignancies. The actuarial 1-year survival rate of the patients was 56.2%. Elective hepatic resection was performed on 10 patients following these procedures. These patients are all doing well. Complete tumor necrosis was demonstrated in 5 of these 10 patients by pathohistological studies on resected specimens from 1 to 11 months after surgery. These tumors were solitary, small (less than 4 cm in diameter), encapsulated, and located deeply inside the liver, being supplied by a single artery. In the other 5 patients, tumor cells in the main tumor and/or in daughter nodules were viable. We conclude that TAE combined with TAI was justified for treatment of hepatic malignancies in selected cases.

    Notes: Summary In the present study, we compared the survival of patients with multi tumor hepatocellular carcinoma (HCC) following their treatment with liver resection versus TAE. A total of 336 HCC patients were treated at Osaka University Hospital between 1980 and 1989. Of these, 140 patients underwent liver resection in the presence or absence of TAE and 173 subjects were treated with TAE alone. Our TAE protocol consisted of 50 mg Adriamycin, 3–5 ml lipiodol, and Gelfoam. The 1-, 3-, and 5-year survival values found for the liver resection group were 87.4%, 66.0%, and 47.4%, respectively, whereas the values calculated for the TAE group were 64.6%, 29.9%, and 15.8%, respectively. The survival of patients in the resection group was clearly better than that of subjects in the TAE group. Of the 140 patients who underwent resection, 36 cases were proven to have multiple lesions by histopathological examination. The 1- and 3-year survival values determined for this special group were 67.9% and 33.3%, respectively. Of the TAE cases, 113 were diagnosed as having multiple lesions by imaging examination, and their 1- and 3-year survival values were 59.7% and 24.9%, respectively. No significant difference in survival was found between these two different treatment modalities for these multiple-lesion cases. The results of this study indicate that it is unlikely that surgical resection is superior to TAE alone for the treatment of HCC patients with multiple lesions.
