    Notes: The process of skin aging is a combination of an extrinsic and intrinsic aspect, and knowing the molecular changes underlying both is a prerequisite to being able to effectively counter it. However, despite its importance for a deeper understanding of skin aging as a whole, the process of intrinsic skin aging in particular has barely been investigated. In this study, the molecular changes of intrinsic skin aging were analyzed by applying ‘Serial Analysis of Gene Expression’ (SAGETM) to skin biopsies of young and aged donors. The analysis resulted in several hundred differentially expressed genes with varying statistical significance. Of these, several genes were identified that either have never been described in skin aging before (e.g. APP) or have no identified function, e.g. EST sequences. This is the first time that intrinsic skin aging has been analyzed in such a comprehensive manner, offering a new and partially unexpected set of target genes that have to be analyzed in more detail in terms of their contribution to the skin aging process.

    Description / Table of Contents: Zusammenfassung Ziel der plastisch-chirurgischen Maßnahmen ist es, Kindern mit Down-Syndrom durch Verbesserung der Sprechfähigkeit und Abschwächung des typischen, sogenannten mongoloiden Gesichtsausdrucks die Kontaktaufnahme zu ihrer Umwelt und das Leben in der Gemeinschaft, in der Familie und ihrer Umgebung zu erleichtern. Als plastisch chirurgische Eingriffe kommen in Frage - in unterschiedlicher Notwendigkeit und je nach Alter — u. a. die Korrektur der Makroglossie, der Sattelnase mit Ausgleich der Mongolenfalten, des fliehenden Kinns und des fetten Halses sowie der deformierten Ohren.
    Notes: Summary The aim of plastic surgery in children with Down's syndrome is the improvement of speech as well as improvement of appearance. Altering the typical mongoloid physiognomy facilitates the integration of these children into the community. In our Department plastic surgery was performed on 96 children for the correction of macroglossia, saddle nose deformity, epicanthic folds, receding chin, fatty neck and deformity of the ears during the past 18 months. Macroglossia was present in every case, the extent of further surgery varies according to the clinical deformity.

    Description / Table of Contents: Zusammenfassung An Ratten, Meerschweinchen und Katzen wurden die Effekte verschiedener Narkotica (Pentobarbital, Phenobarbital, Hexobarbital, Propanidid, Äther, Äthylurethan, Chloralhydrat, Methoxyfluorane) auf die energiereichen Phosphatverbindungen in der Gehirnrinde wÄhrend normaler Narkose und nach überdosierung bis zum Verschwinden der elektrischen Potentiale im Elektrocorticogramm untersucht. Zur Verhütung einer kritischen Herz- und Kreislaufinsuffizienz wurde bei den Tieren unter künstlicher Beatmung der arterielle Druck, die Herzfrequenz und der Herzquerdurchmesser fortlaufend registriert. In gewöhnlicher Pentobarbital-Narkose ergab die Analyse der P-Verbindungen in der Hirnrinde von Ratten folgende Werte (in ΜM/g Feuchtgewicht): Kreatinphosphat (CP) 3,1; ATP 1,7; ADP 0,6; Orthophosphat 2,5. Im Gehalt der Hirnrinde an Kreatinphosphat, ATP oder Orthophosphat konnten keine Unterschiede zwischen Tieren ohne Narkose, in gewöhnlicher Narkose und nach extremer überdosierung bis zum Erlöschen der Hirnpotentiale festgestellt werden. Die Ergebnisse zeigen, da\ narkotisch wirksame Substanzen in vivo nicht zu einer Hemmung des oxydativen Stoffwechsels des Gehirns führen. Eine Wiederbelebung des Gehirns sollte daherkünstliche Beatmung vorausgesetzt — in jedem Stadium der Narkotica-überdosierung ohne bleibende SchÄden möglich ssin.
    Notes: Summary The effects of various general anesthetics (pentobarbital, phenobarbital, hexobarbital, propanidid, ether, ethylurethane, chloralhydrate, and methoxyflurane) upon the high-energy phosphate compounds in the cerebral cortex of rats, cats and guinea pigs were studied under conditions of regular anesthesia and at doses leading to complete disappearance of potentials recorded by the electrocorticogram (ECG = 0). During artificial respiration, arterial blood pressure, pulse and heart diameter were monitored in order to prevent cerebral circulatory failure. The following concentrations of phosphorus compounds (in (ΜM/gm) were found in the cerebral cortex of rats under regular pentobarbital anesthesia: phosphocreatine (PC) 3.1, ATP 1.7, ADP 0.6 and orthophosphate 2.5. No differences were found in the concentration of PC, ATP or orthophosphate in the brain of unanesthetized animals, those receiving regular anesthesia and those animals administered an overdose of an anesthetic. The results indicate that general anesthetics do not alter oxidative cell metabolism in vivo. Resuscitation without cerebral damage should be possible at any stage subsequent to an overdose, provided that the supply of oxygen to the brain is sufficient.

    Notes: Abstract Since 1978, 300 children with Down's syndrome have received plastic- and aesthetic-surgical corrections of the face—to the tongue, nose, and less frequently, ears and chin—so that they will be less conspicuous. The parents' satisfaction in 95% of the cases indicates that the children's situation has been improved by the operations. The children are less stigmatized and their social integration became easier.

    Description / Table of Contents: Zusammenfassung Der Einfluß von normalen und extrem hohen Narkoticumdosen sowie von akuter Ischämie auf den Gehalt der Hirnrinde an energiereichen Phosphatverbindungen wurde an Ratten, Meerschweinchen oder Katzen unter künstlicher Beatmung und Kreislaufkontrolle untersucht. Die elektrische Hirntätigkeit wurde mit Hilfe des ECG und des cerebralen Bestandspotentials kontrolliert. In gewöhnlicher Narkose mit Pentobarbital, Phenobarbital, Hexobarbital, Äthylurethan, Chloralhydrat, Chlorbutanol, Propanidid, Penthrane oder Äther wurden bei normal hohen ECG-Potentialen und einem Gleichspannungspotential von 6–10 mV keine Konzentrationsunterschiede im corticalen ATP- und Kreatinphosphatgehalt gegenüber nicht-narkotisierten Gehirnen gefunden. Auch bei Narkotica-Überdosierung bis zur völligen Unterdrückung der ECG-Potentiale (ECG=0) war der ATP- und CP-Gehalt der Hirnrinde gegenüber einfacher Narkose unverändert hoch. Das Bestandspotential des Gehirns war dabei meistens um 1–2 mV angestiegen. Ischämie (oder Anoxie) sowie Cyanidvergiftung bringt die Hirnfunktionen ebenfalls rasch zum Stillstand. Im Gegensatz zur Wirkung der Narkotica war jedoch bereits beim Erlöschen des ECG eine beträchtliche Abnahme der ATP-Vorräte des Gehirns um 33% und der CP-Bestände um 66% zu beobachten. Der anschließende Zusammenbruch des Bestandspotentials war von einem vollständigen Verlust der CP-Vorräte begleitet. Die Ergebnisse zeigen, daß der Stillstand der elektrischen Hirnaktivität-selbst bei extremer Narkoticum-Überdosierung-nach einem anderen Mechanismus als bei Ischämie oder Anoxie erfolgt: Während im letzteren Falle die neuronalen Membranpotentiale infolge Erschöpfung der Vorräte an ATP und CP nicht mehr aufrecht erhalten werden können, beruht die narkotische Hemmung der elektrischen Hirnaktivität nicht auf einem corticalen Energiedefizit. Entscheidend ist hier offenbar eine direkte Blockierung der bioelektrischen Erregungsprozesse durch Membranstabilisierung.

No differences were found in the concentration of PC, ATP, or orthophosphate in the brain of unanesthetized animals, those receiving regular anesthesia and those animals administered an overdose of an anesthetic. Steady potentials were moderately increased in the latter case. If, on the other hand, the ECG-potentials were abolished by ischemia or cyanide poisoning a considerable decrease of ATP and creatine phosphate occurred amounting to 33 and 66% resp. Complete exhaustion of phosphocreatine was accompanied by a steep fall of the cerebral steady potential. The results indicate that the bioelectrical activity of the cerebral cortex can be suppressed in two different ways, (a) by direct inhibition of neuronal membrane excitability due to an overdose of general anesthetics or (b) by an energetic deficiency if the cortical high-energy phosphate stores are exhausted due to ischemia (anoxia) or after administration of metabolic inhibitors such as cyanide.

    Notes: Summary Neurovascular muscle transposition flaps from local muscle groups covered with split skin grafts are simple and effective for covering soft tissue defects in the leg. The healing of osteomyelitis is thereby enhanced. Suitable donor muscles are discussed according to the anatomical level with practical examples from over 20 cases.

    Notes: Abstract  This article presents our experience with an anatomically-shaped implant, which is filled with a special cohesive silicone gel. From 1993 through 1996, 132 women underwent breast augmentation. The indications were simple aplasia/hypoplasia (40), unsatisfatory appearance of previous prosthesis (58), and post-mastectomy deformity of the breast (34). Results and complications are reviewed. The findings suggest that this type of implant is safe and achieves satisfactory results in breast augmentation.

    Notes: Summary In this study of more than 250 cases of retromuscular expander implantations after mammary amputations, we came to the conclusion that primary implantation is now just as successful as secondary implantation. The major problem is infection which occurred mostly in expander cases with a distant valve. As a result of the experience gained in this study, tissue expansion is considered to be the best method for immediate breast reconstruction. The skin-muscle-flap will be studied in a future series.
