    Notizen: Iotasul®, a new water-soluble contrast medium, is very suitable for subepidermal infusion (indirect lymphography) for the demonstration of small (initial) dermal lymphatics in the macro-range (precollectors). By this method normal lymph vessels can be demonstrated in the healthy skin of hands and feet which are so fine that they cannot be punctured for direct lymphography.We studied thirty-eight patients with various forms of lymphoedema by indirect lymphography. In patients with congenital lymphoedema (particularly Milroy's disease) the initial dermal lymphatics in the lymphoedematous region were absent but large lymph collectors were present. Hyperplasia of dermal lymphatics in the form of an extremely fine net was seen in most patients with lymphoedema praecox and tardum.In some forms of primary lymphoedema the defect may be in the initial dermal lymphatics. In secondary lymphoedema, the changes in the peripheral lymphatics (mainly distension and congestion) are apparently a consequence of the primary lesion in the lymph nodes and large collectors. Indirect lymphography might be useful for the early diagnosis of mild lymphoedema and for excluding patients with hypoplasia of initial lymph vessels from surgery.

    Notizen: Summary Background  Most leg ulcers occur in patients with venous insufficiency. However, not all patients with venous insufficiency develop leg ulcers. Recent studies have found that factors causing clotting abnormalities, e.g. anticardiolipin antibody (ACA), are associated with leg ulcers. Although lupus anticoagulant, like ACA, belongs to the group of antiphospholipid antibodies, its presence in patients with venous leg ulceration has not been previously reported.Objectives  To determine the presence of lupus anticoagulant in patients with venous leg ulceration.Methods  We investigated the presence of lupus anticoagulant in 27 patients with venous leg ulcers and compared these data with controls. Lupus anticoagulant was evaluated in all subjects by the Russell's viper venom test.Results  Of 27 patients with venous leg ulceration, 16 (59%) were shown to have lupus anticoagulant, while only one of 32 controls (3%) was found to have lupus anticoagulant. Thus, lupus anticoagulant was significantly more frequent in patients with venous leg ulcers than in controls (P 〈 0·001).Conclusions  We suggest that lupus anticoagulant could be a hitherto unknown factor contributing to the development of venous leg ulcers.

    Notizen: Aim of the study An observational study with valaciclovir was conducted to assess clinical outcome in herpes zoster, especially pain and associated neurological signs and symptoms in relation to a series of demographic and disease characteristics discernible at presentation. The safety and acceptability of valaciclovir for treatment of zoster was assessed in a wide variety of primary care and clinic referral settings. Methods In total, 1897 immunocompetent adults with clinically diagnosed, localized acute herpes zoster were enrolled in this international, open-label study of valaciclovir. All subjects received treatment with oral valaciclovir (1000 mg three times daily) for 7 days from entry to the study and were asked to record the presence of zoster-associated pain and abnormal sensations throughout treatment and 6 months’ follow-up. They were seen frequently in clinic to verify subjective assessments and for evaluation of rash healing. Safety and tolerability were assessed by adverse event monitoring. Results Overall, 1191 subjects (63%) were aged ≥ 50 years, and 203 (11%) had ophthalmic zoster. Cessation of zoster-associated pain was significantly faster in the younger age group; median times to loss of zoster-associated pain were 23 days and 9 days in the ≥ 50 and 〈 50 years age groups, respectively. Similarly, abnormal sensations resolved significantly more rapidly in the younger subjects; the median duration of abnormal sensations was 31 days in the ≥ 50 year olds and 16 days in those aged 〈 50 years. In cases of ophthalmic zoster, the rate of pain resolution was not different from those with zoster in other dermatomes (median duration of pain 18 vs. 16 days). However, abnormal sensations persisted significantly longer in subjects with ophthalmic zoster than in those with zoster at other sites (47 vs. 22 days). In addition to advancing age, subjects suffering moderate to severe prodromal pain or acute pain during the rash phase were at significantly greater risk of zoster-associated pain and abnormal sensations persisting for longer. Subjects with concomitant neurological disorders were also more likely to develop prolonged abnormal sensations. Valaciclovir treatment was well tolerated, and adverse events were rare and generally mild. Conclusion This study confirmed the prognostic importance of advancing age and the intensity of prodromal or acute pain as risk factors for prolonged zoster-associated pain and persisting abnormal sensations in the affected dermatome. Ophthalmic zoster and pre-existing neurological disorders are also identified as highly significant risk factors for prolonged abnormal sensations in herpes zoster.

    Notizen: Abstract Highly optimised algorithms are, in general, hard to understand. This is a consequence of the designer's sacrifice of clarity and modularity in favour of efficiency. In this paper we present a formal derivation of a rather ingenious algorithm, viz., the fast pattern matching algorithm of Boyer and Moore. The development illustrates that transformational programming combined with assertional reasoning provides an appropriate approach for developing and understanding highly optimised algorithms.

    Beschreibung / Inhaltsverzeichnis: Zusammenfassung Es werden die Ergebnisse neurohistologischer Untersuchungen am gemischten Nerven, Muskel, Knochen und Haut bei einem Patienten mit hereditärer sensorischer Neuropathie vorgelegt. Der Patient gehört der ersten in Österreich erfaßten, mit diesem Leiden behafteten Familie mit bisher 93 Probanden an. Er erkrankte mit 14 Jahren mit Druckstellen an den Zehen. Später rezidivierenden Paronychien, trophischen Geschwüren und Akroosteolyse. Absetzung beider Füße im Bereich der Unterschenkel mit 45 Jahren. Die histologische Untersuchung ergab: Im Nerven eine fortgeschrittene Entmarkung vom segmentalen Typ, ohne Abbauprodukte, Proliferation der Schwannschen Zellen, vermehrte Bildung von Mastzellen im Endo- und Perineurium und ausgeprägte Veränderungen an der Endstrombahn (Intimaproliferation in den Arterien, Quellung der Capillarwandzellen ohne perivasale Ödembildung). Gleiche Gefäßveränderungen in dem sonst unveränderten Knochen. Inzipiente neurogene Atrophie der Muskulatur. Chronisch entzündliche Veränderungen in der Haut (plasmacelluläre und lymphocytäre perivasculäre Infiltrate), thrombembolischer Verschluß der größeren Arterien mit Rekanalisation und Bildung von Capillarangiomen in den äußeren Muskellagen sowie Myohyperplasie der Muskulatur. Epithelhyperplasie und vermehrte Verhornung. Proliferation der Schwannschen Zellen der Hautnerven bei vollkommener, alle Anteile des Nervensystems betreffender Denervierung der untersuchten Hautabschnitte. Neubildung von Capillarangiomen an der Grenze von Cutis-Subcutis mit reichlicher Ablagerung von eisenhaltigem Pigment im Bereich der Gefäßneubildungen. Starke Vermehrung der Mastzellen in der Haut.
    Notizen: Summary Results of the neurohistological examination of mixed nerve, muscle, bone, and skin in a patient with hereditary sensory neuropathy are reported. The patient belongs to the first family found to be affected by this disease in Austria, the whole sibship consisting of 93 persons so far. His illness began at the age of 14 when he developed pressure sores on his toes. Later on, recurrent paronychia, trophic ulcers, and osteoacrolysis occurred. At the age of 45 both feet had to be amputated. Histological findings included: progressive demyelination of the segmental type of nerves without degradation products; proliferation of Schwann cells, increased formation of mast cells in the endo- and perineurim, and pronounced alterations of the terminal vascular bed (proliferation of the intima of arteries, swelling of the cells of the capillary wall in the absence of perivascular edema). Similar vascular lesions in the otherwise unaffected bone. Incipient neurogenic atrophy of muscles. Chronic inflammatory lesions in the skin (plasmacellular and lymphocytic perivascular infiltrations, thrombembolic occlusion of the larger arteries with recanalization and formation of capillary angiomas in the exterior muscle layers, as well as myohyperplasia of the muscular system. Epithelial hyperplasia and increased hornification). Proliferation of the Schwann cells of cutaneous nerves with complete denervation involving all nervous structures of the skin areas examined. De novo formation of capillary angiomas at the borderline between cutis and subcutis associated with large deposits of iron pigment around the vascular overgrowth. Marked increase of mast cells in the skin.
