Résumé Le propylenglycole par voie buccale explique un effet hypotensif intraoculaire au moyen d'un méchanisme d'action osmotique, soit chez des yeux atteints de différentes formes de glaucome soit chez des yeux normaux. La dose employée pour obtenir une efficace réduction de la tension intraoculaire est de 1.5 g/Kg de poids. La durée de l'effet est environ de 5 heures dans la plus part des cas traités. La baisse de la pression intraoculaire variait avec le niveau tensionnel au moment du début de la thérapie; plus haute était la pression initielle, plus grande la réduction. La réponse variait avec la quantité du dosage employé. Le propylenglycole par voie buccale s'est révélé particulièrement utile dans le traitement prolongé des sujets glaucomateux avec diabète, car il ne modifie pas significativement le niveaux glycémiques. La substance est très bien tolérée et n'a jamais provoqué des effets collatéraux, tels que céphalée, nausée, diarrhée, soif, vertige. Seulement chez quelques sujets on a observé un modeste état d'ivresse. L'application locale d'une pommade de propylenglycole s'est démonstrée efficace pour éclaircir la cornée chez 7 sujets atteints d'oedème cornéen de différente origine parfois accompagné d'une kératopathie bulleuse.
Abstract:
Zusammenfassung Die perorale Verabreichung von Propylenglykol sowohl in Augen mit verschiedenen Formen von Glaukom als auch in normalen Augen erwirkt durch einen osmotischen Mechanismus eine Senkung des Augendruckes. Die angewandte Dosis zur Erhaltung einer Augendrucksenkung ist 1.5 g/Kg Körpergewicht. Die Dauer der Wirkung beträgt etwa 5 Stunden in den meisten behandelten Fällen von Glaucoma simplex. Die Senkung des Augendruckes ging mit dem Druckniveau am Anfang der Behandlung ein; je höher der Anfangsdruck war, desto ausgeprägter war die Senkung. Die Senkung war nach der Menge der angewandten Dosis verschieden. Die perorale Anwendung dieser Substanz hat sich bei der protrahierten Behandlung von Glaukompatienten mit Diabetes als besonders wirksam erwiesen; tatsächlich ruft sie keine bedeutsamen Änderungen der Glykaemie hervor. Das Propylenglykol wurde sehr gut vertragen und hat nie Nebenwirkungen, wie zum Beispiel Kopfschmerzen, Ubelkeit, Durchfall, Durst und Schwindel verursacht. Nur bei einigen Patienten wurde eine leichte Erregung beobachtet. Die lokale Anwendung der Propylenglykol-Salbe ermöglichte eine Erhellung der Hornhaut bei 7 Patienten mit Hornhautoedem verschiedenen Ursprungs, manchmal kompliziert durch ‘Bullous keratopathy’.

Abstract Oral propylenglycol is able to lower intraocular pressure through an osmotic mechanism of action both in eyes suffering from different forms of glaucoma and in normal eyes. The dosage employed to obtain an effective reduction in intraocular pressure is 1.5 g./Kg of body weight. Duration of the tension-lowering effect is about 5 hours in most cases treated. The drop in pressure varied with the height of the pressure at the time therapy began - the higher the initial pressure, the greater the fall. The response varied with the strength of the dosages employed. Oral propylenglycol has proven particularly useful in longterm treatment of glaucomatous patients with diabetes, as it does not alter significantly blood sugar levels. The drug was fairly well tolerated and side-effects, such as headache, nausea, diarrhea and thirst were never reported. Only some patients complained of slight dizziness. The topical application of an ointment composed of propylenglycol has proven effective in clearing the cornea in 8 patients suffering from corneal edema of various origin, sometimes accompanied by bullous keratopathy.
