    Description / Table of Contents: Zusammenfassung 13 Patienten, die sich eine Subluxationsverletzung des Lisfranc-Gelenks zugezogen hatten, wurden nach einem Zeitraum von durchschnittlich 23 Monaten, klinisch, radiologisch und mittels dynamischer pedographischer Ganganalyse nachuntersucht. Bei allen Patienten fand sich auf der verletzten Seite ein Plattfuß. Radiologisch zeigten sich arthrotische Veränderungen der Mittelfußgelenke sowie eine Restdislokationsstellung des medialen Lisfranc-Gelenks. Im Vergleich zu einem Normalkollektiv fand sich eine vermehrte Beweglichkeit im unteren Sprunggelenk (USG), erhöhte Belastung am Rückfuß kombiniert mit einer verminderten Belastung am Vorfuß, sowie eine verlängerte Bodenkontaktzeit während der gesamten Standphase. Ziel dieser Studie war es, die Ursache der posttraumatischen Plattfußdeformität zu analysieren, um Schlußfolgerungen für die Erstversorgung und die weiteren therapeutischen Maßnahmen zu ziehen.
    Notes: Summary In this retrospective study, 13 patients with subtle Lisfranc joint injuries were examined after a mean period of 23 months using clinical assessment, radiography and dynamic pedographic gait analysis. The aims were to identify the factors leading to a mobile flatfoot deformity, evaluate the functional and clinical outcome of these injuries, and draw practical conclusions for initial management and subsequent intervention. All patients showed a mobile flatfoot deformity, increased motion in the subtalar joint, increased load on the hindfoot, decreased load on the forefoot, and a prolonged contact phase during the stance phase. Radiographs revealed progressive osteoarthrosis in the joint and a residual displacement of the medial Lisfranc joint. An unstable medial Lisfranc joint results in the development of a mobile flatfoot. Initial treatment of a subtly displaced Lisfranc joint should consist of exact anatomical reduction and additional maintenance of the longitudinal arch of the foot. After failed initial treatment, early arthrodesis of the midfoot is recommended as a salvage procedure for the foot.

    Description / Table of Contents: Abstract Congenital pseudarthrosis of the tibia (CPT) is one of the most challenging conditions in paediatric orthopaedics. The history of CPT treatment ischaracterised by repeatedly ineffective surgical interventions with the inevitable outcome of severe disability. Most procedures failed to account for the complexity of the problem. The past two decades have witnessed more experience having been gained with the Ilizarov technique. This method provides a comprehensive approach to all aspects of CPT problem, allowing the surgeon to simultaneously address the problems of union as well as those of deformity, length discrepancy joint function, ankle valgus and weight bearing. It appears that the goal to achieve stable fusion before the child begins school is attainable. In the pre-pseudoarthrotic stage, bracing should be preferred to surgery until the patient is in an older age group. This study reports on the results of 19 patients: 15 have finished treatment, and 14 of them with a stable fusion at follow-up. The results were highly encouraging in reaching the ultimate goal of avoiding amputation and achieving a well-functioning limb.
    Notes: Zusammenfassung Die kongenitale Tibiapseudarthrose gehört zu den schwierigsten Krankheitsbildern in der Kinderorthopädie. Anamenstisch findet sich bei diesen Patienten meist eine Vielzahl ineffektiver, operativer Eingriffe und letztendlich auch ein funktionell unbefriedigendes Ergebnis in Form einer den Patienten schwer behindernden Deformität. Bei den meisten dieser Behandlungsverfahren wird der Komplexität des Problems kongenitale Tibiapseudarthrose nicht entsprechend Beachtung geschenkt. In den letzten Jahren wurden deshalb Verfahren entwickelt, die eine erfolgreiche Behandlung ermöglichen. Es handelt sich dabei um den vaskularisierten Fibulatransfer und die Ilizarov-Technik. Letzterer muss unserer Meinung nach deshalb der Vorzug gegeben werden, da sie dem behandelnden Orthopäden die Möglichkeit gibt simultan sowohl Pseudarthrose, als auch Achsendeformität, Beinlängendifferenz, und Sprunggelenkvalgus zu behandeln. Weiter hat sich gezeigt, dass allzu frühe operative Eingriffe ungünstig sind und dass es sinnvoll ist, primär eine konsequente konservative Therapie durchzuführen, um im Alter von 3–6 Jahren operativ eine stabile Fusion der Pseudarthrose mit Korrektur der Begleitdeformität zu erreichen. Die vorliegende Arbeit berichtet über eigene Ergebnisse bei 19 Patienten, von denen bei 15 die Behandlung abgeschlossen wurde, wobei bei 14 Patienten eine stabile Fusion zum Zeitpunkt der Nachuntersuchung erzielt werden konnte. Die Ergebnisse waren sowohl klinisch als auch funktionell äußerst zufriedenstellend und lassen den Schluss zu, dass eine erfolgreiche Behandlung der Tibiapseudarthrose heute möglich ist und eine Amputation daher als obsolet angesehen werden muss.

Plain radiographs were obtained on event presentation. Later radiographic examination showed the fracture and a periosteal reaction. The fracture healed after immobilising the limb in a cast for 2 months. This case illustrates well an insufficiency fracture to the occurrence of stress risers in a weakened bone when subjected to repetitive loading as a complication following total knee arthroplasty.

    Notes: Abstract This report presents the case of an 8-year-old boy who underwent a second clubfoot operation following early-stage reflex sympathetic dystrophy (RSD). After other conditions had been ruled out, the patient was submitted to physiotherapy supported by antiphlogistic and analgesic drugs as well as a partial immobilisation of the affected extremity. He remained asymptomatic during the following 4 weeks. RSD in children is not a well-recognised entity. This case of early-stage RSD illustrates the need to be aware of this possible complications after operation in the differential diagnosis of local pain and swelling of a limb.
