    Description / Table of Contents: Résumé Le terme d'ulcère réfractaire s'applique pour l'auteur aux ulcères qui ne cicatrisent pas malgré 3 mois de traitement actif, qui récidivent dans l'année en dépit d'un traitement par les anti-H2 ou qui persistent sans rémission à l'endoscopie. Environ 20–30% des ulcères répondent à cette définition d'ulcère réfractaire. Les facteurs qui vont de pair avec l'ulcère réfractaire sont les suivants: ulcère de grande taille, obstruction pylorique, déformation en sablier, pénétration, infection à candida, résistance à la cimétidine. La cicatrisation incomplète de l'ulcère lorsqu'il est traité par les anti-H2 peut prédisposer à la persistance à long terme de l'ulcère. Le succès du traitement de l'ulcère réfractaire dépend davantage de la durée du traitement que du médicament employé. L'agent anti-secrétoire le plus efficace: l'omeprazole représente le meilleur médicament. Le rôle du tabac et des autres facteurs d'environnement reste imprécis.
    Abstract: Resumen El término refractaria es aplicado a todas aquellas Úlceras que fallan en cicatrizar a pesar de 3 meses de tratamiento, presentan recurrencia en el curso de un año a pesar de terapia sostenida con antagonistas de receptores H2, o cuando, en ausencia de tratamiento, la Úlcera persiste sin remisión. Aproximadamente 20–30% de las Úlceras cumplen uno de estos 3 criterios. Factores asociados con Úlcera refractaria incluyen gran tamaño del cráter ulceroso, obstrucción de la desembocadura gástrica, deformación del estómago en reloj de arena, penetratión, infección con candida, y resistencia a la cimetidina. La cicatrización incompleta de la Úlcera y/o el tratamiento con antagonistas de receptores H2 pueden predisponer a una prolongada evolución refractaria. El éxito en el tratamiento de la Úlcera refractaria se correlaciona mejor con la duración del tratamiento que con la droga utilizada. Los agentes antisecretorios de mayor potencia tales como el omeprazol pueden ser las drogas de escogencia. El rol del cigarrillo y de otros factores ambientales no aparece claro.
    Notes: Abstract The term refractory is applied to those ulcers that fail to heal despite 3 months of active treatment, recur within 1 year despite maintenance therapy with H 2-receptor antagonists, or, in the untreated state, persist without remission. Approximately 20–30% of ulcers fulfill 1 of these 3 criteria. Factors often associated with a refractory ulcer include large ulcer size, gastric outlet obstruction, hour-glass deformity, penetration, Candida infection, and cimetidine resistance. Incomplete healing of the ulcer and/or initial treatment with H 2-receptor antagonists may predispose to a refractory long-term course. Successful treatment of the refractory ulcer correlates more with duration of treatment than with the drug used. The more potent antisecretory agents, such as omeprazole, may be the drug of choice. The role of smoking and other environmental factors is not clear.

    Notes: Summary 1. The most powerful adsorbents of pepsin are aluminum hydroxide gel and charcoal. 2. The adsorbent action of pepsin is uninfluenced by the substrate concentration and is less active at lowpH levels. When used to inactivate pepsin, the adsorbent effect of aluminum hydroxide gel is more important than thepH effect. 3. The capacity of aluminum hydroxide gel to adsorb other substances such as anticholinergic agents and antibiotics should be remembered when it is used in the treatment of peptic ulcer. 4. The antipeptic capacity of aluminum hydroxide gel and the secretory responses found in ulcer patients have been discussed in relationship to the dosage used in ulcer therapy.

    Notes: Summary 1. By maintaining the pH of the gastric contents above 7, a series of nonproteolytic enzymes have been demonstrated to be present in gastric juice. These include lactic dehydrogenase, isocitric dehydrogenase, leucine aminopeptidase, alkaline phosphatase, glutamic oxalocetic transaminase, glutamic pyruvic transaminase, ribonuclease, beta-glucuronidase. Enzymes sought and not found include 6-phosphogluconic dehydrogenase, alpha-ketoglutaric dehydrogenase, and fructose 1–6 diphosphatase. 2. The activities of the above mentioned enzymes in normal subjects and patients with peptic ulcer, gastric carcinoma, and pernicious anemia are described, and the enzyme activities found in homogenates of gastric mucosa and gastric carcinoma are compared with those found in gastric juice.     Notes: Abstract A subgroup of patients with nonulcer dyspepsia (NUD) have no definite cause for their dyspepsia, termed essential dyspepsia.The aim of the present study was to determine if environmental factors are associated with essential dyspepsia. The patterns of ingestion of analgesic drugs (aspirin, acetaminophen, dextropropoxyphene), nonaspirin, nonsteroidal antiinflammatory drugs, alcohol, coffee, tea, and smoking in 113 essential dyspepsia patients were compared with 113 randomly selected community controls matched for age, sex, and social grade. Associations were studied in two six-month periods, before diagnosis in all patients and before the onset of NUD in those patients with a short history of dyspepsia. It was found that acetaminophen ingestion was associated with essential dyspepsia, and this association was present both before the onset of the dyspepsia (OR 3.1, 95% CI 1.3– 7.1) and before diagnosis (OR 1.8, 95% CI 1.2– 2.6). None of the other environmental factors were associated with essential dyspepsia.

    Notes: Summary 1. Histamine stimulates pepsin secretion in man, both the output and concentration rising. In the dog, histamine produces a fall in pepsin concentration and output; some investigators have observed a transient rise in pepsin output preceding a fall. This could be accounted for by a washing out effect. 2. This difference in the effect of histamine on pepsin output in man and dogs is probably a true species difference. If pepsin is secreted continuously, the low or absent basal secretion in the dog makes a washing out effect theoretically possible. Also, there is evidence that histamine may inhibit the action of the peptic cells of the dog.

    Notes: Summary The hepatic and portal venous systems were studied by postmortem contrast radiography in 12 cirrhosis patients. The hepatic venous system was abnormal in 11 cases, the most common abnormalities being distortion of the smaller veins and deficient filling of the hepatic venous system. In some cases the caliber of the veins appeared irregular. In no case was there a definite abnormality of the portal vein. There did not appear to be any correlation between the extent of the vascular changes and the etiology of the cirrhosis, the presence of splenomegaly, varices, ascites, a low serum albumin, or the pathological findings.

    Notes: Summary Ribonuclease in human gastric juice is described. This enzyme has two pH optima, one at pH 4.5–5.0, and one at pH 7.0–7.5. It is heat labile and Cu++ has an inhibitory effect. The properties of this enzyme are compared with those of ribonucleases derived from other animal tissues.

    Notes: Summary The mucus secretion of a series of patients with gastric ulcer, gastric carcinoma, and pernicious anemia, studied in regard to concentration, output and constituent sugars in basal gastric secretion, showed few differences between the groups of patients. Pernicious anemia patients showed high mucus concentrations due to their low secretory volume. Gastric ulcer patients had a mucus output and concentration similar to control subjects. The only obvious difference in mucus secretion appeared to be that in gastric carcinoma patients; in this group, the amount of glucose in proportion to other sugars was high.
