Notes: Abstract Human papilloma virus (HPV; 16 and 18) is known to play an important etiologic role in cervical dysplasia, but its relationship with anal carcinoma is still unclear. Surgical samples from 80 female patients treated for anal epidermoid carcinoma in the Policlinic of Surgery in Geneva between 1976 and 1989 were retrospectively studied. Of these, HPV detection was performed in 41 whose DNA was preserved well enough to allow such an analysis. Seventeen (42%) samples contained HPV, with a high percentage of high risk HPV (15/41, 36%). Thirty-eight of the 80 patients had a cervical smear, of which 18% revealed cervical carcinoma. When compared with epidemiological data, the results of this study suggest that genital HPV infection predisposes not only to cervical cancer, but also to anal carcinoma, possibly by means of contiguous contamination.

Notes: Abstract. The aim of this study was to evaluate the clinical efficacy, mechanical stability, and safety of the Simon nitinol inferior vena cava filter (SNF). The SNF was inserted in 114 consecutive patients at two institutions for prophylaxis of pulmonary embolism (PE). Clinical follow-up data were obtained retrospectively on all patients, and 38 patients underwent a dedicated radiologic follow-up protocol consisting of abdominal radiography, Doppler sonography, and CT. There was no immediate complication following filter insertion. Fifty patients died, on average, 5.6 (1–23) months after filter insertion, and 64 patients were alive, on average, 27 (3–62) months after filter insertion. Recurrent pulmonary embolism was documented in 5 patients (4.4 %) but originated distal to the filter in 1 patient. Deep venous thrombosis (DVT) was documented in 5.3 %, thrombosis at the access site in 3.5 %, and thrombosis of the inferior vena cava in 3.5 %. The rate of thromboembolic complications was similar in patients who did receive long-term anticoagulation and in those who did not. Radiologic follow-up showed no filter migration after, on average, 32 (5–62) months. A CT examination showed that struts of the SNF had penetrated the vena cava in 95 %, and were in contact with adjacent organs in 76 %; however, there were no clinical symptoms attributable to the filter. Filters were in an eccentric position in 63 % and partial filter disruption was found in 16 %; however, this did not affect filter function. The rate of recurrent pulmonary embolism after insertion of the SNF is 2.4 % per patient per year. Regardless of long-term anticoagulation, the rate of caval thrombosis is acceptably low. Except for occasional access-site thrombosis, no other filter-related morbidity was observed.

Notes: Abstract The purpose of this retrospective study was to determine the CT findings diagnostic of cardiac and pericardial injury, including signs of pericardial tamponade, in patients suffering from blunt and penetrating trauma. A search of the CT radiology database at a level I trauma center was performed to identify cases in which injury to the heart or pericardium was diagnosed, as well as to identify cases of pericardial tamponade. All cases were reviewed to ascertain the specific CT findings, and medical records were reviewed to assess the influence of CT findings on management and to assess for clinical evidence of pericardial tamponade. Eighteen patients had direct CT evidence of cardiac or pericardial injury, including nine cases of pneumopericardium, eight cases of hemopericardium, and one case of intrapericardial gastric herniation. Four of these patients were found to have direct cardiac injuries. Three additional cases with CT evidence of pericardial tamponade were identified, two secondary to cardiac compression by an anterior mediastinal hematoma and one following repair of left ventricular rupture. Of 11 patients with CT evidence of tamponade, only three were suspected clinically. Cardiac and pericardial injuries are usually diagnosed surgically and are often clinically unsuspected, particularly in blunt trauma. As CT is increasingly utilized as a general screening test for thoracic/abdominal trauma, these injuries may be first suspected on the basis of CT findings, and knowledge of the CT findings of cardiac injury or tamponade is crucial.

Notes: Thirteen antihistaminic drugs are separated on a CP-Sil 5 capillary column and are evaluated by a NP detector after extraction of spiked plasma samples, alkalinized, in a solvent mixture of chloroform, dichloromethane, and methanol. Intra- and inter-assay analyses showed high reliability, therefore this method can be usefully applied in experimental or clinical pharmacokinetic investigations.

Notes: Calcium blocker drugs are of increasing therapeutic relevance. Verapamil, nifedipine, diltiazem, and flunarizine are the most widely applied drugs of this type. New dihydropyridines (felodipine, nitrendipine) and related agents are presently undergoing intense investigation. HRGC and HPLC were used to determine eight of these drugs in spiked plasma samples. Both methods allow separation of most of the compounds in a single run and are specific, sensitive, and reproducible both for therapeutic monitoring and for pharmacokinetic studies in man and animals.
