    Notes: This paper reports the results of the first stage of an evaluation study of a health visitor caseload weighting system. In Bristol, health visitor caseloads are compiled annually to complete caseload weighting summaries based on 28 health and social factors known to reflect long-term health outcome. The main study aims to determine the validity and reliability of the caseload weighting system by assessing health visitor variation in interpretation of the health and social factors, the accuracy of caseload factor recording and methods of analysis used in interpreting their significance. This paper describes the exploratory phase of the study involving the distribution of a questionnaire to 252 health visitors in five Trust areas where no caseload weighting system operates. The purpose of the questionnaire and follow-up interviews with 28 health visitors was to develop a multiple-choice questionnaire to assess how health and social factors were being interpreted by health visitors using them in their everyday work. The study indicated that the 28 health and social factors were perceived to significantly influence the need for health visiting services. Some differences in perception of a small number of factors were identified according to the level of deprivation on the caseload. Health visitors' prioritization of child protection, and child protection related health and social factors, raised the possibility that caseload weighting might be used as a ‘child protection check list’ rather than as an overall measure of health visitor need. Interviews showed that a range of thresholds governs the health visitors' application of a particular factor to a family. Health visitor workload is also affected by other factors relating to practice location, which would need to be taken into account if these health and social factors were to be used for estimating workload. Health visitors not currently using the factors view them positively for the assessment of health needs in an area, and for planning work on an individual or management level, but the interviews identified a need for training in their use and suggested more work is needed to tighten and standardize definitions for use in the field.

    Notes: Medium density (3×109 cm−3) and high density (3×1012 cm−3) plasmas, generated by low and medium power rf techniques, have been used for the implantation of 10–20 keV nitrogen ions into mild steel targets which were immersed in the plasma and biased to −20 kV. Use of the high density plasma resulted in significant damage to the surface by arcing. At medium densities the nitrogen was implanted to a depth and dose consistent with expectations, there was no arcing damage, and tests showed improved wear and hardness compatible with the level of implantation.

    Notes: Abstract A statistical study was made of reported night-waking in a national cohort of British 5-year-olds in order to explore long-term associations with physical, behavioural and mental development. After adjusting for the most likely confounders, reported night-waking al five was found to be linked at 10 years of age to certain chronic medical conditions and some symptoms of emotional distress. No convincing relationships existed between night-waking at five and selected measures of later physical development or intellectual ability.

    Notes: MPB70 is a soluble secreted protein highly expressed in Mycobacterium bovis and strains of bacille Calmette–Guérin (BCG); as such, it is a candidate for subunit and DNA vaccines against tuberculosis. MPB70 was screened for T-cell epitopes in four different inbred mouse strains. Major histocompatibility complex (MHC) H-2b-expressing mice (C57BL/6) secreted interferon-γ (IFN-γ) after stimulation with peptides from the regions 1–20, 41–50, 81–110, 121–150 and 161–193 of the MPB70 sequence. H-2db mouse (B6D2) splenocytes secreted IFN-γ after stimulation with some of the same peptides, whereas H-2d mice (BALB/c and DBA/2) did not secrete IFN-γ upon stimulation with the peptides. Sera from H-2db mice immunized with native MPB70 in incomplete Freund's adjuvant (IFA), mpb70 DNA or live BCG Moreau were found to contain antibodies against the native MPB70 antigen. H-2db mice immunized with native MPB70 in IFA exhibited high titres of peptide-reactive immunoglobulin G1 (IgG1) antibodies, whereas DNA-immunized mice reacted with IgG2a antibodies against some of the same peptides. As some of the epitopes recognized by mouse T and B cells have previously been found to stimulate immune responses in humans, cattle and rabbits, we conclude that these epitopes may be good general epitopes for the stimulation of T- and B-cell responses and candidates for a DNA vaccine with a broad applicability.

    Notes: The mpt83 gene (Rv2873) encodes the exported MPT83 lipoprotein of Mycobacterium tuberculosis. The corresponding identical mpb83 gene of Mycobacterium bovis is expressed to varying extents in different substrains of M. bovis Bacille Calmette Guerin (BCG), BCG Tokyo and BCG Moreau being high producers and BCG Danish 1331, a low producer of the MPB83 protein. Immunization with the 13-mer N-terminal part of the signal peptide of MPT83, MINVQAKPAAAASC, coupled to keyhole limpet haemocyanin (KLH) through the added C-terminal cysteine resulted in rapid antibody formation monitored by enzyme-linked immunosorbent assay (ELISA) with free immunizing peptide on the solid phase. In ELISA, with four 20-mer overlapping peptides covering the N-terminal part of the MPT83 sequence, three polyclonal rabbit antisera reacted only with the N-terminal peptide. Antigenic signal peptide could not be detected in sonicates of BCG Tokyo and BCG Moreau. After SDS-PAGE and blotting, the antibodies reacted with sonicates of recombinant Escherichia coli containing the entire mpt83 gene including the signal sequence, but not with the 22 kDa form of native MPB83 purified from BCG culture filtrate. In partition chromatography the recMPT83 partitioned in the water phase while 26 kDa MPB83 in BCG culture filtrate partitioned in the lipid phase confirming that lipidation at the N-terminal cysteine residue occurs after the splitting of the polypeptide chain by signal peptidase II.

    Notes: A 26-kDa antigen has been shown to be a dominant antibody target in Mycobacterium bovis-infected cattle. In this study, that antigen was used as an immunogen to raise a panel of mouse monoclonal antibodies. The majority of those bound to native protein with a molecular mass of 26 kDa and to recombinant MPB83, strongly suggesting that MPB83 is an important B-cell antigenic target in bovine tuberculosis. In order to provide assessment of the potential of measuring antibody responses to the native protein, one monoclonal antibody, 1F11, was incorporated into an enzyme-linked immunosorbant assay format to trap antigen from a crude bacterial extract. Despite some disadvantages of this format, serum samples from cattle which had been infected experimentally with M. bovis, and from tuberculin skin-test-negative and -positive field cattle were tested for the presence of antibodies. Data from the skin-test-negative cattle allowed an arbitrary cut-off value to be established and, under these conditions, test sensitivity and specificity were estimated at 37.5 and 89%, respectively. These results indicate potential for MPB83 in the development of assays for serological diagnosis of bovine tuberculosis.
