Abstract The case of a comminuted metacarpal head fracture of the index with distinct displacement and 180° longitudinal malrotation as well as irreducibility of head fragments due to entrapment in volar soft tissues is presented. Our case might illustrate the complications possibly encountered in these severe fractures, such as neural compromise and postoperative adhesions. Although the final functional outcome was good and the patient felt minimal pain, radiographic signs of avascular necrosis developed 12 months after injury.
