Zusammenfassung Die akuten eitrigen Infektionen der Finger und der Hand unter dem Sammelbegriff Panaritium erfordern unsere frühestmögliche Aufmerksamkeit, exakte Kenntnisse der Anatomie und große Erfahrung in der Beurteilung des Entzündungsstadiums und bei der Wahl des Zeitpunktes zur operativen Behandlung. Die empfohlene Technik des operativen Vorgehens bei den verschiedenen Formen des Panaritium ist orientiert nach folgenden Prinzipien: Ausreichende Anaesthesie, gewebs- und funktionsschonende Schnittführung in Blutsperre, vollständige Eröffnung des Eiterherdes und Eliminierung der Gewebsnekrose. Verhinderung des Fortschreitens der Entzündung durch Ruhigstellung, lokale und allgemeine Antibioticatherapie, wenn dies zweckmäßig erscheint. Die fortgeschrittene Infektion gehört in die Hände eines handchirurgisch versierten Arztes, am besten in klinische Behandlung. An einer Fingerruine sollte jedoch nicht zu lange „alles versucht” werden, da hierdurch der Gebrauchswert der ganzen Hand in Gefahr geraten kann.

Summary The acute suppurating infections of the fingers and hands summarized under the heading Panaritium demand prompt attention, exact knowledge of anatomy, and experience in evaluation of the progression of the infection as well as in the timing of surgical intervention. The technique recommended for the surgical intervention with the different types of Panaritium is determined by the following principles: adequate anaesthesia, direction of the incision to prevent damage to tissue and function during the interruption of circulation, complete opening of the focus of infection and elimination of necrotic tissue. Prevention of progression of the infection is achieved by immobilization, and local and general antibiotic therapy if this seems necessary. Advanced infection should be treated by a surgeon with experience in surgery of the hand, if possible with hospilization. Too much time should not be spent on one badly affected finger, as this may endanger the use of the whole hand.
