    Notes: We propose a model for assessing the function of 5HT receptors in migraine by evaluating their expression on monocytes by means of double-labeling fluorocytometry (CD14-positive cells FITC-labeled). This model demonstrates that during headache induced in, migraine without aura sufferers given isosorbide dinitrate followed by sumatriptan treatment 5HT expression on monocytes progressively increases. This increase may be due to the activation of 5HT turnover and to the increased availability of 5HT displaced by sumatriptan from ‘cerebrovascular receptors during head pain.

    Notes: The collisional relaxation of highly vibrationally excited CF2O* molecules prepared by infrared laser multiphoton absorption is compared with the results obtained when CF2O* is generated as a product of the reactions of CF3 and CF2Cl radicals with NO2. The three methods produce molecules with probably different energy distributions and also different average excitation energies 〈E〉. Thus, IR laser excitation results in a bimodal distribution, with average excitation energies in the range 3 000–20 000 cm−1, while the chemical reactions of CF3 and CF2Cl radicals produce CF2O* with a undetermined level of vibrational excitation that depends on the specific energy change of the process. Irrespective of the method of preparation, the same exponential decays are obtained for the each of various colliders studied (Ar, N2, NO2, and CF2O). It is shown that under these conditions, the observed bulk average energy transferred per collision, 〈〈ΔE〉〉, is equal to the microscopic value 〈ΔE〉. However, a single exponential energy decay is not sufficient condition to assure that equality. © 1999 American Institute of Physics.

    Notes: The CCR5 gene encodes a cell-surface chemokine receptor molecule that serves as a co-receptor for macrophage-tropic strains of human immunodeficiency virus type 1 (HIV-1). A mutation in this gene may alter the expression or the function of the protein product, thereby altering chemokine binding and/or signalling or HIV-1 infection of cells that normally express CCR5 protein. Individuals homozygous for a 32-bp deletion allele of CCR5 (CCR5 Δ32), heritable as a Mendelian trait, are relatively resistant to HIV-1 infection. The CCR5 Δ32 mutation is present in the Caucasian population at different frequencies. The aim of this study was to investigate the frequency of truncated alleles of the CCR5 Δ32 gene in a Sicilian population, as the interpopulation variation in CCR5 Δ32 frequency may be a significant factor in the prediction of AIDS endemicity in future studies. We examined 901 healthy individuals from several Sicilian provinces. We found a mean (± standard deviation) Δ32 allele frequency (fr) of 0.04 ± 0.012. The highest value was observed in the province of Messina, with a mean Δ32 allele frequency of 0.06 ± 0.024, where we collected samples from a cohort of 114 HIV-1-infected individuals. The observed frequency amongst these patients was quite low (fr = 0.03 ± 0.031) compared to the healthy population, although the difference was not statistically significant.

    Notes: Abstract— Creep Crack Growth (CCG) tests were performed at 405°C on specimens cut out of the cold bent extrados of five tubes of a C-Mn-Mo steel. Intergranular fracture and grain boundary cavitation was less in the C-Mn-Mo than in the C-Mn steels, in accordance with better CCG resistance of the former material. The dimensions and hardness variation across the crack tip process zone were measured by microhardness profiles performed on metallographic sections of the broken samples. TEM analysis of the dislocation patterns close to the fracture surface confirmed the presence of temperature- and stress-induced plasticity phenomena. A significant enrichment of N at grain boundaries (GB) inside the process zone was detected by Auger spectroscopy; N not only inhibits dislocation motion and stress field relaxation at the crack tip but also causes a decrease in GB cohesion ahead of the crack tip. These results help in understanding the micromechanisms which reduce the creep ductility of C-Mn-Mo and C-Mn cold bent tubes and the role of chemical composition in improving CCG resistance.

    Notes: The ablation of arrhythmias progresses towards an approach based upon application of linear lesions between nonconducting anatomic/electrical areas. Hence the identification of detailed anatomy together with electrical behavior becomes increasingly important. This study aims to achieve true electroanatomic mapping by the use of three-dimensional intracardiac imaging of the right atrium combined with use of a right atrial basket to obtain detailed electrical information. We studied nine patients, seven requiring atrial flutter ablation. A 9 Fr, 9 MHZ intracardiac echo catheter was pulled back from SVC to IVC using respiratory and ECG gating. The images, recorded on a Clearview ultrasound machine, were reconstructed using commercially available software. The intracardiac basket was placed into the atrium using the markers and fluoroscopy to allow orientation. Isochronal maps were obtained from the basket in sinus rhythm, pacing from different sites within the atrium and in atrial flutter. Isochronal maps were constructed and superimposed on the ICE image. The maps with pacing were consistent with that which was expected, confirming the validity of this approach. We were able to visualize changes in activation sequence following the placement of bidirectional isthmus block. True electroanatomic mapping is possible by the use of three-dimensional ICE reconstruction of the right atrium with electrical activation obtained from an intracardiac basket. This has significance for anatomically based arrhythmia ablations such as the ablation of atrial flutter, atrial fibrillation, with transcatheter MAZE procedures and pulmonary vein isolation. Further developments in software will allow such maps to be produced simultaneously with greater rapidity. (PACE 2004; 27:318–326)

    Notes: ICD implantation is standard therapy for malignant ventricular arrhythmias. The advantage of dual and single coil defibrillator leads in the successful conversion of arrhythmias is unclear. This study compared the effectiveness of dual versus single coil defibrillation leads. The study was a prospective, multicenter, randomized study comparing a dual with a single coil defibrillation system as part of an ICD using an active pectoral electrode. Seventy-six patients (64 men, 12 women; age 61 ± 11 years ) were implanted with a dual (group 1, n = 38) or single coil lead system (group 2, n = 38 ). The patients represented a typical ICD cohort: 60% presented with ischemic cardiomyopathy as their primary cardiac disease, the mean left ventricular ejection fraction was 0.406 ± 0.158 . The primary tachyarrhythmia was monomorphic ventricular tachyarrhythmia in 52.6% patients and ventricular fibrillation in 38.4%. There was no significant difference in terms of P and R wave amplitudes, pacing thresholds, and lead impedance at implantation and follow-up in the two groups. There was similarly no difference in terms of defibrillation thresholds (DFT) at implantation. Patients in group 1 had an average DFT of 10.2 ± 5.2 J compared to 10.3 ± 4.1 J in Group 2, P = NS. This study demonstrates no significant advantage of a dual coil lead system over a single coil system in terms of lead values and defibrillation thresholds. This may have important bearing on the choice of lead systems when implanting ICDs. (PACE 2003; 26:1684–1690)
