    Notes: The morphological, enzymatical, immunocytochemical and functional properties of Langerhans' cells are briefly reviewed. Langerhans' cells are located mainly in the squamous stratified epithelia, but are also present in the thymus and in superficial lymphnodes. At the ultrastructural level, they are characterized by unique cytoplasmic organelles, the Birbeck granules, whose function is still unknown. Langerhans' cells possess strong ATPase activity and are weakly positive for alpha-naphtyl acetate esterase and for acid phosphatase; they are immunoreactive for CD1a (T6), class II MHG antigens and S-100 protein. In some pathological conditions, like dermatopathic lymphadenopathy and Langerhans' cell histocytosis, Langerhans' cells also are characterized by the expression of monocyte-macrophage antigens. Langerhans' cells act as antigen-presenting cells to T lymphocytes; their functional capacity is strictly dependent on the levels of expression of class II MHG antigens. Langerhans' cells are of bone marrow origin and are derived from a circulating precursor which is probably related to the monocyte.

    Notes: Abstract Background: CH-40 is a suspension of activated carbon particles that was developed in Japan to carry anticancer drugs to regional nodes and peritoneal seedings of gastric cancer. Methods: Forty-five consecutive patients who had surgical resection and D2 lymph node dissection for gastric cancer over a 2-year period were randomly assigned to preoperative endoscopic submucosal injection of CH-40 (group A) or no staining (group B). A total of 21 patients in group A and 24 in group B were available for analysis. Results: The number of resected nodes per patient was significantly higher (t=6.06; 40df; P〈.0001) in group A (mean±S.E.=35.3±1.24) than in group B (mean±S.E.=25.5±1.02). The rate of metastatic nodes resected was significantly higher (χ2=6.903 ; 1df; P=.009) in stained (22.5%) than in non-stained (14.7%) nodes of group A and also (χ2=6.906 ; 1df; P=.009) in stained nodes of group A than in group B (15.8%). Conclusions: Preoperative endoscopic vital staining with CH-40 proved to be rapid, safe, and effective in all cases in this series. Its use allowed surgeons to resect a higher number of lymph nodes, and to identify and examine more metastatic nodes. It also permitted identification of nodal micrometastases on routine histopathologic examination.

    Notes: Summary A 16-yr-old white female was affected by continuous fever, pancytopenia with relative increase of T-8 lymphocytes, severe bone marrow hypoplasia, generalized lymphadenomegaly and splenomegaly. A first lymph node biopsy, obtained at the onset of the disease, was involved by a paracortical tumor with some S-100+ “lymphocyte-like” cells in the neoplastic areas; in the cell suspension, 70–80% of cells were E4 + /E37 + lymphocytes with prevalent expression of the T-8 phenotype (52%). A second lymph node biopsy, obtained five months later, was involved by a diffuse proliferation of S-100 + cells with high mitotic activity; in the cell suspension, the majority of cells were E-/T-11+/ T-3+/T-8+. At the TEM level, the neoplastic cells were characterized by regular or indented nuclei with finely dispersed chromatin and by regular or indented nuclei with finely dispersed chromatin and by irregular cytoplasmic profiles with thick pseudopodia-like projections. The possibility is discussed that this neoplasm may share some similarities with the T-γ lymphoma being part of a poorly described group of tumors with intermediate features between T cell lymphoma and malignant histiocytosis.
