    Notes: The prevalence of skin and internal malignancies was estimated from the general practitioners’notes of 2247 patients with psoriasis and 4494 age- and sex-matched controls. The prevalence of skin cancer in the psoriatics was 155% that of controls, but this was not significant at the 5% level. Subgroup analysis showed an increase in skin cancers in women (P 〉 0.05). There was no difference in the age of onset of skin cancers between psoriatics and controls and there was no evidence of a cumulative therapeutic risk. There was no difference in the prevalence of non-skin cancers between psoriatics and controls.

    Notes: Forty-two patients with chronic resistant varicose ulcers were observed for 6 weeks on standard out-patient therapy, and then randomly allocated to daily treatment with either cadexomer iodine (CI), a new topical agent, or dextranomer for a further 6 weeks, at which time ulcers judged clinically not to be responding could be changed to the other treatment for the remaining 20 weeks of the trial. There was no significant reduction in ulcer area during the first 6 weeks on CI or dextranomer. A significant preference for CI was shown by the optional crossover. Retrospective analysis of ulcer size shows this was not related to severity and preference was probably due to bias towards the new treatment, the trial not being blind. Neither therapy significantly reduced colonization by β-haemolytic Streptococcus, Staphytocoaus aureus, Pseudomonas or Proteus. The discrepancy between these and previous findings with CI is largely due to different methods of analysis rather than different results. We conclude that, in chronic resistant venous ulcers, topical therapy is of little importance compared with measures which improve tissue oxygenation and repair.

    Notes: Summary A simplified method of patch testing was designed, in which several totally unrelated allergens were combined in each patch. The method was evaluated by double blind comparison of responses to 17 standard allergens applied individually as 17 conventional patch tests, and as two different sets of five patches each containing combinations of three or four allergens per patch, in 137 patients under investigation for contact allergic dermatitis. There were 89 positive responses to conventional patch testing with separate allergens and 94 and 86 positives to the two different combinations of the same allergens. Concordance of positive reactions to the two combinations was 80% and there were no irritant reactions. Conventional testing detected 70 and 74% of reactions to combination patches 1 and 2 and combination patches 1 and 2 detected 80 and 79% of the reactions to conventional testing. The combinations detected clinically relevant sensitivities not found by conventional testing. Thus, combination patch testing appears to give consistent and reliable results; its use would reduce the number of patches and increase the diagnostic yield for the specialist and permit preliminary screening by the general practitioner.

    Notes: Plasma β-MSH levels and sebum excretion rates (SER) were measured in thirty male and twelve female patients with acne vulgaris. The mean SER in both male and female patients was significantly increased as compared to normal control levels.Plasma β-MSH levels were normal in male and female patients with acne and showed no correlation with the SER. The SER that occurs in acne cannot therefore be explained by an increased secretion of β-MSH.

    Notes: SUMMARY.— The application of a topical anticholinergic agent (poldine methyl -methosulphate) to the forehead skin of 10 patients with mild acne vulgaris twice daily for 4 weeks produced a small but significant reduction in sebum excretion. This is compatible with the suggestion that the control of sebum secretion may have a cholinergic component, but the effect is small.

    Notes: Application of 1% potassium hydroxide (KOH) reduced subsequent development of anthralin inflammation without loss of its therapeutic effect on psoriasis. Teepol had a similar but smaller effect on subsequent development of inflammation. The action of KOH appears to have resulted from enhanced oxidation of anthralin to inactive products and the action of Teepol to have increased anthralin solubility and removal. The effect of KOH and Teepol decreased with time after anthralin application and both were ineffective by 24 h, indicating that anthralin persists on the skin in an active form for up to 24 h after a single application. The reduction of anthralin inflammation without loss of therapeutic effect is potentially useful in short contact anthralin therapy.

    Notes: A randomized, double-blind, placebo-controlled study was made of 2% ketoconazole cream and shampoo in 20 patients with seborrhoeic dermatitis of the face. Sixteen also had seborrhoeic dermatitis of the scalp and five had seborrhoeic dermatitis of the chest or back. Responses were measured by clinicians and patients independently using a grading system and linear analogue scales, respectively. Face and scalp lesions, assessed by both patient and clinician, showed a significant improvement or complete clearance in the group treated with ketoconazole. The patients who had seborrhoeic dermatitis of the chest or back and were treated with ketoconazole also improved. There was no improvement with placebo. This study provides further evidence for the aetiological role of pityrosporon yeasts in seborrhoeic dermatitis and of the efficacy of topical ketoconazole in its treatment.

    Notes: The distribution and sub-cellular localization of cytochrome P-450-dependent mono-oxygenase activity in the skin of adult hairless mice has been investigated using ethoxycoumarin, ethoxyresorufin and benzpyrene as substrates. Highest levels of mono-oxygenase activity per unit area of skin were found in the dermis, both before and after induction with benzanthracene. Subcellularly, mono-oxygenase activity was predominantly microsomal and, prior to induction, located entirely in the smooth microsomal fraction.
