    Notes: 371 patients with histologically confirmed adenocarcinomas of the digestive system were postoperatively monitored by analysis of the time course of the CEA concentration in the serum. The tendencies of the time courses of the computerized CEA surveillance diagrams allowed an early prognosis of local tumor recurrence or of generalized metastasizing. The diagnosis of a relapse on the basis of a rise in CEA concentration preceded a positive clinical diagnosis up to 10 months. The diagnosis was confirmed by second-look operation or by other diagnostic means. The analysis of the CEA time courses made it possible to differentiate local tumor recurrence and generalized metastasizing.

    Notes: Background: Rectal foam enemas provide for drug delivery to the distal colon for treatment of left sided ulcerative colitis. However, currently available formulations contain chlorofluorocarbons which are due to be phased out in the near future. The objective of this study was therefore to determine the degree of dispersion of a newly developed nonchlorofluorocarbon rectal foam preparation in ulcerative colitis patients. Methods: This was an open label non-controlled study of a single administration of a mesalazine foam enema (two actuations containing 2 g of mesalazine in approximately 120 mL foam) in 10 patients with quiescent ulcerative colitis. Spreading of the 99mTc-labelled foam enema was assessed over a 4-h period by the non-invasive technique of gamma scintigraphy. Results: All patients retained the enema for the full 4-h imaging period. In nine out of the 10 patients, the enema was observed to spread as far as the descending colon and on average 23% of the dose was present in the descending colon at 4 h post-dose. Conclusions: The extent of spreading observed in the study supports the use of the formulation in the treatment of left sided ulcerative colitis.

    Notes: Summary Induction of neoplastic growth of murine stroma cells within the human tumor xenograft was observed after serial passage of CEA and β2-microglobulin producing human colonic SLu tumor xenografts in nu/nu BALB/c mice. Mouse tumors within the human tumor xenografts wre identified using specific immunohistologic staining techniques for mouse histocompatibility marker or human CEA. These mixed tumors could be distinguished from normal human tumor xenografts by a different relationship between development of the tumor marker in the serum and tumor size. We were able to establish transformed murine cells from human xenografts, either induced by SC injection of 1×106 tumor cells of the SLu cell line or by human SLu or mammary carcinoma tissue serially passaged in athymic animals. The established human and murine cell lines were characterized by cytogenetic methods. Transformed murine cells were then continuously passaged in tissue culture. The transformed mouse fibroblasts proved to possess tumorigenicity in nude mice. In the case of SLu-derived mouse tumor cells, tumors also developed in the immunocompetent BALB/c mice using 1×106 to 5×106 tumor cells for SC transplantation.

    Description / Table of Contents: Zusammenfassung CEA-Immunkomplexe wurden neben freiem CEA im Serum von 350 Patienten mit Adenocarcinomen des Gastrointestinaltraktes präoperativ und während einer zweijährigen Überwachung routinemäßig bestimmt. Bei 25% der Patienten (n=86) konnten schon präoperativ zirkulierende CEA-Immunkomplexe nachgewiesen werden. Das Auftreten von CEA-Immunkomplexen erwies sich als geeigneter prognostischer Marker hinsichtlich der Tumorausdehnung, da bei 72/86 Patienten mit CEA-Immunkomplexen Metastasierung bei der Erstoperation vorlag. Das postoperative Auftreten von CEA-Immunkomplexen konnte als zusätzlicher Parameter für die Rezidivdiagnose herangezogen werden. 32/60 Patienten mit Rezidivierung entwickelten im Verlauf der Überwachung CEA-Immunkomplexe. Alle Patienten mit lokalisierten Rezidiven waren frei von CEA-Immunkomplexen. Wenn dagegen CEA-Immunkomplexe nachgewiesen werden konnten, war klinisch immer eine Fernmetastasierung feststellbar. Dieser Diagnose ging immer ein Anstieg des freien CEA und/oder der CEA-Immunkomplexe voraus. Bei 5/60 Patienten konnte die Rezidivierung nur klinisch gesichert werden, da diese Patienten im Überwachungszeitraum immer CEA-negativ blieben.
    Notes: Summary CEA immune complexes and free CEA were routinely determined in sera of 350 patients with adenocarcinoma of the gastrointestinal tract preoperatively and during a 2-year surveillance period. We could detect circulating CEA immune complexes preoperatively in 25% of our patients. The appearance of CEA immune complexes proved to be a useful prognostic marker with respect to tumor extension since 72/86 patients with CEA immune complexes showed metastasis at the primary resection. The postoperative appearance of CEA immune complexes could be used as an additional parameter for the diagnosis of the relapse; 32/60 patients with a relapse developed CEA immune complexes during the period of surveillance. All patients with localized disease recurrence were found to be free of CEA immune complexes. Detection of CEA immune complexes, however, coincided always with the clinical diagnosis of distant spread of disease. This diagnosis was always preceded by an increase of free CEA and/or CEA immune complexes. In 5/60 patients the relapse could only be demonstrated by clinical methods since these patients stayed CEA-negative throughout the surveillance period.

    Notes: Summary In a prospective study of 928 patients with gastrointestinal cancer registered for primary resection, the postoperative carcinoembryonic antigen (CEA) time courses were analysed in connection with disease recurrence. Only patients with established diagnosis of disease recurrence and complete follow-up to death entered the evaluation of prognostic criteria of the CEA time course. In this group the CEA time courses of 103/201 patients with recurrent disease exhibited an exponential increase of the serum CEA concentration, i.e. a linear relationship of log CEA and time, which allowed the calculation of the CEA doubling time. All 103 patients developed metastatic spread and generally exhibited CEA doubling times ranging between 10 and 158 days, in patients developing peritoneal carcinosis up to 343 days. The individual CEA doubling times of patients with recurrent disease who received no treatment (n=71) correlated well with the times of individual survival after the initial CEA increase of the log CEA phase (rs=0.812;P〈0.001) thus confirming the results of a previous retrospective study. When the survival time is expressed in multiples of the individual CEA doubling time (IDT), no patient survived longer than 10.8 IDT. The median value of survival was 5.4 IDT. Patients with metastatic spread who underwent various treatments of recurrent disease (n=32) survived distinctly longer showing survival times up to 32.6 IDT. This could be confirmed by comparing the observed survival after the initial CEA increase of treated and untreated patients (life table method) exhibiting highly significant differences (P〈0.001).

    Notes: Summary The clinical validity of monitoring the tumor markers carcinoembryonic antigen (CEA) and CA 19-9 were investigated in 602 patients with colorectal, gastric, and pancreatic carcinomas. Sensitivity and specificity of the tests were evaluated preoperatively as well as in the postoperative follow-up for early detection of disease progression and recurrence. At a 95% level of specificity as calculated from a group of 150 patients with benign diseases, the CEA test with monoclonal antibody had a preoperative sensitivity of 39% in colorectal cancer and 21% in gastric cancer. On the other hand, CA 19-9 had a sensitivity of 19% in colorectal cancer, 21% in gastric cancer, and 89% in pancreatic cancer. In the postoperative follow-up it was found that a combination of both tumor marker tests was most profitable in gastric carcinomas, yielding an increase of sensitivity from 59%–94%, showing a high degree of complementarity. The gain in sensitivity provided by the CA 19-9 test over the CEA-test in colorectal cancer was very low. The gain in sensitivity, however, provided by the CEA test over the CA 19-9 test in pancreatic carcinoma was also very low. On the basis of these results it has to be recommended that cases with pancreatic carcinoma are to be monitored most efficiently with the CA 19-9 test, whereas in cases with colorectal cancer the CEA test should be used primarily. However, in gastric cancer the combined use of CEA and CA 19-9 represents a highly valuable basis for monitoring the course of disease.

    Notes: Summary K12.148 (INN: lifibrol), a new cholesterol synthesis inhibitor, was studied in healthy volunteers to evaluate tolerance/safety, the effects on lipids, and pharmacokinetics. In a sequential block design the doses of 150, 300, 600, or 900 mg, given once daily in the morning for 14 consecutive days, were examined in 40 healthy young males (8 active drug and 2 placebo per group, randomized) under well-controlled conditions. Total and LDL cholesterol serum levels decreased significantly in the 300, 600, and 900 mg groups ( -13.4% , - 23.8%, −25.6%, and −14.7%, −33.3%, −34.8% respectively), whereas no significant change was seen with placebo and 150 mg. The antiatherogenic index Apo A-I/B increased in a dose-dependent manner between 300 and 900 mg. Changes in HDL cholesterol and triglycerides were not statistically significant. The study compound was tolerated well, and safety laboratory parameters did not show any relevant alterations. K 12.148 might be a very effective drug for the treatment of hypercholesterolemia.

    Notes: Summary The effect of droloxifene, a new anti-oestrogenic drug, onN-nitrosomethylurea-induced mammary tumours of Sprague-Dawley rats was investigated and compared with that of tamoxifen. The response of tumour growth to ovariectomy or to treatment with aminoglutethimide or high doses of oestradiol was also studied. Ovariectomy was by far the most effective treatment for mammary-tumour-bearing animals. More than 75% of the tumours in ovariectomized rats did not grow progressively but remained in remission for up to 12 weeks after castration when the experiment was terminated. The inhibitory effects of droloxifene and tamoxifen on mammary tumour growth were similar, but body weight loss of animals treated with tamoxifen was more marked than that of animals treated with droloxifene at the same dose and schedule.
