    Notes: The outer root sheath of hair follicles plays an important role in epidermal regeneration in vivo. Keratinocytes isolated by explantation of outer root sheath tissue have extensive proliferative capacity irrespective of donor age, which probably depends on pluripotent epithelial stem cells residing in the outer root sheath. These keratinocytes can be organotypically grown to epidermal equivalents in vitro. We report here that in a multicenter, randomized phase II study, EpiDex™, a tissue-engineered, fully differentiated autologous epidermal equivalent derived from keratinocytes of the outer root sheath of plucked anagen hair follicles, is as effective as split-thickness skin autografting in the promotion of healing and complete closure of recalcitrant vascular leg ulcers.     Notes: Abstract Because outer root sheath (ORS) cells are valuable substitutes for interfollicular epidermal keratinocytes, we wanted to determine whether epidermal equivalents generated from ORS cells and containing cultured melanocytes can serve as an in vitro model for skin pigmentation. In such epidermal equivalents prepared with ORS cells and melanocytes from donors of phototypes II, III and VI, a stratified epithelium resembling normal epidermis developed within 14 days, as documented by histological, ultrastructural (e.g. basement membrane-like structure, keratohyalin granules, keratinosomes) and immunohistochemical (e.g. keratins, integrins, gp80, involucrin, filaggrin) criteria. The melanocytes were localized in the basal layer and accounted for 10% of the total cell number. Heavily pigmented melanocytes from black donors contained regular melanosomes in all stages of maturation, whereas melanocytes derived from white donors contained predominantly melanosomes of stages I and II. Melanosome-laden dendrites were readily detected extending from the heavily pigmented melanocytes, while they were less conspicuous in melanocytes from white donors. The extent of melanosome transfer was independent of the racial origin of the ORS cells. Melanosomes could also be transferred “through racial barriers”. Melanosomes, mainly of stages III and IV, were detected in the ORS cells, being distributed either as single or compound melanosomes, again irrespective of the racial origin of the ORS cells. In conclusion, pigmented epidermal equivalents generated from ORS cells offer practical advantages over other in vitro pigmentation models: (1) the ORS cells are easily and repeatedly available from any donor regardless of age; (2) primary cultures of ORS cells are free of contaminating melanocytes, a bias if using interfollicular epidermal keratinocytes; (3) a high degree of epidermal differentiation is maintained for 3 weeks in fully defined medium, enabling labelling and stimulation experiments to be performed and compounds interfering with melanin pigmentation to be tested.

    Description / Table of Contents: Summary The healing of wounds by secondary intention is a simple method to close operative defects. The quality of wound healing will depend on the anatomic site on the face as well as on size and depth of the defect. Best functional and cosmetic results are obtained at locations with concave geometry and when the defect lies symmetrically at the interface of two anatomic subunits. Partial reconstruction or guiding sutures can create a symmetric defect in relation to anatomic subunits which will heal by second intention. This simple and efficient operative procedure is ilustrated by two case reports.
    Notes: Zusammenfassung Die Granulation per secundam (GPS) oder die per-secundam-Heilung ist eine einfache Methode um Operationsdefekte zu schliessen. Die besten funktionellen und kosmetischen Resultate erhält man, wenn der Defekt in einer Lokalisation mit konkaver Oberfläche und symmetrisch zwischen zwei anatomischen Untereinheiten zu liegen kommt. Der postoperative Defekt kann durch Teilrekonstruktionen oder Führungsnähte bezüglich den anatomischen Untereinheiten symmetrisiert werden, sodass der Restdefekt der GPS zugeführt werden kann. Anhand von zwei Fallvorstellungen wird dieses einfache und effiziente Operationsverfahren besprochen.

    Description / Table of Contents: Summary A 70-year-old patient developed Mycobacterium chelonae infection at a donor vein graft site following cardiac bypass surgery. The infection presented as fibrinous, necrotic ulcerations in the scar area. Mycobacterium chelonae and mycobacterium fortuitum are atypical mycobacteria and have been described previously causing infections after injections or surgical procedures. Infection of donor vein graft site is a rare complication after cardiac surgery. As mycobacterium chelonae cannot be cultivated on normal culture media, delayed wound healing might be disinterpretated as a primary wound healing disorder. Treatment of atypical myobacteriosis includes antibiotics, local heat therapy and surgical excision. Clarithromycin is the antibiotic of choice. We obtained complete healing after two months of Clarithromycin treatment, combined with heat therapy.
    Notes: Zusammenfassung Wir berichten über einen 70jährigen Patienten, bei dem nach einer Herz-Bypass-Operation eine Infektion mit Mycobacterium chelonae im Bereich der Venenentnahmestelle am Bein auftrat. Die Infektion manifestierte sich als fibrinös-nektrotisch belegte Ulzera im Narbenbereich. Mycobacterium chelonae and Mycobacterium fortuitum sind atypische Mykobakterien und wurden bereits mehrfach als Erreger von Infektionen nach Injektionen und chirurgischen Eingriffen beschrieben. Der Befall der Venenentnahmestelle durch atypische Mykobakterien gilt als eine sehr seltene Komplikation bei Herzoperationen. Da Mycobacterium chelonae in normalen Kulturmedien nicht anzüchtbar ist, kann der verzögerte Heilungsverlauf als primäre Wundheilungsstörung fehlgedeutet werden. Zur Behandlung der atypischen Mykobakteriosen werden neben der Antibiotikatherapie eine Thermotherapie oder die chirurgische Exzision empfohlen. Clarithromycin gilt zur Zeit als Antibiotikum der Wahl. Wir konnten bei unserem Patienten eine vollständige Abheilung nach zweimonatiger Behandlung mit Clarithromycin und begleitender Thermotherapie erreichen.
